About this opportunity
The Nevada Aging and Disability Services Division (ADSD) is issuing a competitive Notice of Funding Opportunity (NOFO) for transition coordination services under the Money Follows the Person (MFP) Demonstration Grant program. This procurement seeks qualified organizations—including non-profits, public agencies, and for-profit businesses—to provide comprehensive transition coordination services for individuals transitioning from institutional settings (nursing facilities, hospitals, rehabilitation centers) to community-based living environments. Key deliverables and service components include transition intake and eligibility determination, person-centered transition planning, community transition assistance (housing deposits, utility setup, moving expenses), housing coordination, outreach and education, and quality improvement activities. Applicants must demonstrate organizational capacity, cost-effectiveness, and the ability to support approximately 50 transitions during the one-year project period (July 1, 2026–June 30, 2027), with individual community transition services capped at $2,500 per participant. The application deadline is March 20, 2026, at 11:59 PM (PT), with submissions required via email to ADSDGrants@adsd.nv.gov, and an optional informational meeting scheduled for February 20, 2026, at 1:00 PM via Microsoft Teams. Evaluation will be conducted using a 50-point scoring rubric assessing project relevance and need (10 points), capacity and approach (15 points), cost-effectiveness and sustainability (10 points), project impact (10 points), and adherence to instructions (5 points), with subaward decisions announced in June 2026.
Eligible applicants must be in good standing with the State of Nevada and Federal Government, possess a Unique Entity ID (UEI) number, maintain active SAM.gov registration, and hold an Employer Identification Number (EIN); no specific set-aside designations for disadvantaged enterprises are identified in this solicitation, and matching funds are not required. Applicants must comply with Nevada Department of Human Services Grant Instructions and Requirements, ADSD Requirements and Procedures for Grant Programs, Generally Accepted Accounting Principles (GAAP), Generally Accepted Government Auditing Standards (GAGAS), and General Services Administration travel guidelines. Subrecipients must maintain occurrence-based commercial, general, or professional liability insurance and workers' compensation insurance as required by Nevada Revised Statutes, undergo background checks for staff serving vulnerable populations, register with Nevada 211, and enroll in the System Award Management (SAM). Administrative and indirect expenses are limited to 15 percent de minimis or an approved Federal Indirect Cost Rate (FICR) if applicable. Approximately $165,000 in federal MFP Demonstration Grant funds are available for this one-year project period with no renewal options, and reimbursement will be dispersed on a monthly basis in accordance with ADSD reporting schedules.
