About this opportunity
The City of Joliet Fire Department is requesting proposals for medical and fitness evaluation services for the 2026–2027 fiscal years under RFP 2991-0726, with proposals due July 2, 2026 at 10:00 a.m. Questions must be submitted through the Euna Procurement Bonfire platform by June 25, 2026 at 4:00 p.m.
The scope covers annual physical examinations, laboratory testing, fitness assessments, and cardiac evaluations for all department members in compliance with NFPA Standard 1582 and OSHA requirements. Services must be performed at locations within Joliet's corporate limits, with evaluations conducted at approximately 10 per day to allow adequate physician consultation. The provider must conduct spirometry testing and blood draws at four designated fire stations over three days and accommodate scheduling flexibility including available Saturdays. Pricing is structured as unit costs per test across multiple categories: physical examinations and consultations, comprehensive metabolic and lipid profiles, lead and zinc protoporphyrin testing, electrocardiograms, spirometry, and fitness evaluations. Separate unit pricing is required for supplemental tests such as hepatitis boosters and x-rays. Proposals must provide unit costs for 2026, 2027, and yearly extension periods, with actual invoices calculated based on the number of tests performed.
The lead physician must hold an MD or DO degree with occupational medicine experience, board certification in relevant specialties, and demonstrated knowledge of NFPA 1582 standards and the International Association of Fire Fighters Wellness Fitness Initiative. Proposals must include facility location details, contact information, and references from at least three other fire departments currently utilizing the provider's services. All medical evaluation information is confidential under HIPAA; results are reported in general departmental terms unless specific diagnostic information is necessary. A comprehensive departmental health report must be submitted electronically within 30 days of payment. Proposals are submitted and opened at City Hall, 150 W. Jefferson Street, Joliet, Illinois 60432.
