About this opportunity
The Michigan Department of Health and Human Services (MDHHS), in partnership with the Michigan Alliance for Innovation on Maternal Health, is procuring 5,000 Urgent Maternal Warning Lanyard Tags through the State of Michigan's Print & Mail Management division. The finished lanyard tags must measure 2.25" x 3.5" with 1/8-inch rounded corners, feature a centered lanyard hole approximately 5mm x 15mm along the 3.5" edge, and be printed in 4-color (4/4c) on both sides using plastic credit card thickness material (0.76mm). The tags serve as educational resources designed for both patients and healthcare providers, incorporating QR code functionality that enables users to access digital content on mobile devices, including urgent maternal warning signs information, advocacy language, warning signs follow-up guides, and a sepsis in obstetrics score calculator. Deliverables must be completed by March 1st, 2024, and shipped via carton to Bradley M. Schumann at the General Services Building in Dimondale, Michigan; the bid request was posted on January 22, 2024, with a response deadline of January 25, 2024. Bidders must submit detailed cost estimates with their proposals, provide both electronic PDF and hard copy proofs prior to production, and follow a head-to-head layout design as specified in the bid documentation, with failure to provide a detailed estimate resulting in disqualification.
Only pre-qualified vendors registered with the State of Michigan and capable of accepting Electronic Funds Transfer (EFT) payments are eligible to respond to this procurement; interested vendors must contact Jerry Monroe (517-636-0297, monroej1@michigan.gov) or Andrew Hale (517-636-0092, halea3@michigan.gov) for pre-qualification, which requires approximately one week. Bids must be submitted online with all costs included on an F.O.B. Destination, prepaid basis to the state's dock at 7461 Crowner Drive, Lansing, Michigan 48913, and late bids will be rejected. The award will be made by the Department of Technology, Management & Budget based on best price and value rather than lowest bid alone, and past quality concerns or inability to meet specified requirements may result in rejection of the lowest bid. Pricing terms are established on a per-unit or per-job basis as submitted in online bids, with no underruns accepted and any overruns charged to the vendor. The bid documentation does not contain information regarding set-asides for disadvantaged enterprises, incumbent contractors, or specific budget ranges for this procurement.
