About this opportunity
The National Institute on Aging, with co-participation from six other NIH institutes and offices (NICHD, NIMH, NINDS, NINR, NIMHD, NCI) and the Tribal Health Research Office, is soliciting R01 applications for palliative care research across the lifespan with an earliest submission date of January 5, 2027 and a NOFO expiration of November 6, 2029. Stage I intervention development (creation, adaptation, modification, and pilot-testing) is explicitly out of scope; applications must propose hypothesis-driven, milestone-defined clinical trials at Stages II through V of the NIH Stage Model (traditional efficacy, real-world efficacy, effectiveness, and dissemination/implementation) or non-clinical trial research such as health services research, methodologic research, natural experiments, and observational studies focused on palliative care access, use, quality, and disparities for persons with serious illnesses and their care partners.
Eligible applicants include higher education institutions (public and private), 501(c)(3) and non-501(c)(3) nonprofits, for-profit organizations, state and local governments, tribal governments (federally recognized and not), faith-based and community-based organizations, regional organizations, independent school districts, public or Indian housing authorities, and foreign organizations (though foreign subawards and subcontracts are prohibited). No cost share is required. Award budgets are not limited but must reflect actual project needs. Maximum project period is five years. Recipients must become members of the NIH-funded Advancing the Science of Palliative Care Across the Lifespan (ASCENT) Consortium and budget for mandatory consortium interactions including annual conference attendance and collection of agreed minimum common data elements.
All applications must include a Data Management and Sharing Plan compliant with the 2026 NIH format, recruitment and retention plans, and for clinical trials, a detailed Milestone Plan with feasibility assessments and contingency strategies. Clinical trial applications must demonstrate adequate statistical power for mediation and moderation analyses. Standard NIH due dates are February 5, June 5, October 5, and February 5 of subsequent years. Applications undergo initial scientific peer review by non-NIH scientists followed by secondary review by the appropriate NIH Advisory Council; funding decisions consider scientific and technical merit, program relevance, and availability of funds. The opportunity operates under seven Assistance Listing numbers: 93.866, 93.242, 93.393, 93.307, 93.853, 93.865, 93.361.
Synopsis
The purpose of this Notice of Funding Opportunity (NOFO) is to invite R01 applications proposing research on palliative care across the lifespan for persons with serious illnesses and/or their care partners relevant to the missions of participating NIH Institutes, Centers, and Offices (ICOs), including projects focused on palliative care access, use, and quality. Applications may propose hypothesis-driven, milestone-defined palliative care traditional efficacy (Stage II), real-world efficacy (Stage III), effectiveness (Stage IV), and/or dissemination and implementation (Stage V) clinical trials in alignment with the NIH Stage Model (Stages II-V) and relevant Institute or Center policies. Applications may propose non-clinical trial palliative care research, such as health services research, methodologic research, natural experiments, or observational studies. Recipients will become members of the research community facilitated by the NIH-funded Advancing the Science of Palliative Care Across the Lifespan (ASCENT) Consortium.
NOTE: This NOFO is not appropriate for applications proposing new intervention generation, refinement, modification, adaptation, and/or pilot testing (Stage I of the NIH Stage Model) clinical trial research. The intervention(s) to be tested must be in a developed and testable form, with Stage I activities already completed and demonstrated.
