About this opportunity

This notice of funding opportunity announces grants from the Health Resources and Services Administration to establish a National Coordinating Center on Transition and fund Autism and Epilepsy Transition Demonstration Projects. Eligible applicants include U.S. public and private organizations to implement innovative strategies and evaluate transition outcomes for youth with autism and/or epilepsy. The NCCT grant will provide up to $735,000 annually over five years, while demonstration project grants of up to $450,000 per year will be awarded. Applications are due by March 13, 2024 and will be assessed based on need assessment, program design, partnership plans, evaluation methodology, applicant capabilities, and budget appropriateness.

The demonstration projects will be performed throughout the United States to improve transition outcomes and systems of care for youth nationally. It is expected that the work of both the demonstration projects and coordinating center will improve the system of services for children and youth with autism and epilepsy as they transition into adulthood, through evidence-informed strategies implemented with state/local agencies, community organizations, health care professionals, youth, and their families.
