About this opportunity

The National Institutes of Health Common Fund, in partnership with other NIH Institutes and Centers, is soliciting applications for the Precision Medicine with Artificial Intelligence – Integrating Imaging with Multimodal Data (PRIMED-AI) program with an application due date of October 19, 2026.

Eligible applicants include domestic and foreign 501(c)(3) nonprofits, higher education institutions (public and private), non-501(c)(3) nonprofits, state and local governments, for-profit organizations, tribal governments and organizations, and special district governments. Non-domestic components of U.S. organizations and foreign organizations are eligible to apply.

The program operates through a biphasic cooperative agreement mechanism using UG3/UH3 activity codes. Phase 1 (UG3) focuses on rigorous AI model building through retrospective analysis and testing of large, interoperable multimodal datasets to improve diagnostic accuracy, prognostic prediction, or therapeutic guidance. Phase 2 (UH3) advances projects demonstrating strong potential and model readiness to evaluate real-world clinical utility of the AI-driven Clinical Decision Support tool in actual clinical workflows.

The program seeks to develop innovative, reliable, and cost-effective AI-based tools that integrate clinical imaging with other health data types to enhance personalized medicine for patients with chronic and other health conditions. Projects must address unmet health challenges with high potential for demonstrable, positive impact on patient outcomes or healthcare processes. Applicants with expertise in analysis and testing of large, interoperable multimodal datasets are particularly encouraged.

Applications are reviewed for scientific and technical merit by peer review groups, with funding decisions based on significance, approach, innovation, investigator competency, and resource adequacy. The review cycle typically spans four to nine months. Grantees must retain financial and programmatic records for three years from the date of annual Financial Status Report submission. This opportunity is funded under Trans-NIH Research Support (CFDA 93.310).

Synopsis

The overarching goal of this notice of funding opportunity (NOFO) and its companion opportunities is to establish the Precision Medicine with AI: Integrating Imaging with Multimodal Data (PRIMED-AI) Program to support development of innovative, reliable, cost-effective, and sustainable multimodal AI-based clinical decision support (CDS) tools. PRIMED-AI CDS tools are based on the integration of clinical imaging with other types of multimodal health data to enhance care for patients with a wide range of health conditions. The PRIMED-AI Program seeks to catalyze the adoption of AI-based CDS tools into clinical workflows to enable novel personalized medicine strategies that address significant health challenges. The purpose of this NOFO is to catalyze the translation of Artificial Intelligence (AI)-enabled, image-centered, multimodal CDS tools, developed as Software as a Medical Device, from training and testing of prototypes towards clinical applications that address unmet health challenges in precision medicine. These projects are expected to have high potential for demonstrable, positive impact on patient outcomes and/or healthcare processes.

Eligibility

Refer to Section III. Eligibility Information in the NOFO for additional information on eligibility.Foreign Organizations/International Collaborations:Non-domestic (non-U.S.) Entities (Foreign Organizations) are eligible to apply.Non-domestic (non-U.S.) components of U.S. Organizations are eligible to apply.Foreign components, as defined in the NIH Grants Policy Statement, are allowed.
