About this opportunity
The Office of the National Coordinator for Health Information Technology is soliciting applications for cooperative agreements under the Leading Edge Acceleration Projects (LEAP) in Health Information Technology program, with an application deadline of July 16, 2026.
ONC will award up to three cooperative agreements total—up to $1 million for Area of Interest 1 (agentic artificial intelligence solutions in clinical care and clinical trials) and up to $500,000 each for Areas of Interest 2 and 3—distributing $2 million in fiscal year 2026 across one award per area of interest. The initial project and budget period runs September 26, 2026 through September 25, 2028; applicants may propose three-year projects with potential year-three funding contingent on available funds and satisfactory progress.
Eligible applicants include federally recognized tribal governments, 501(c)(3) nonprofits, public higher education institutions, private higher education institutions, and state governments. For-profit organizations and foreign institutions may participate only as sub-recipients or consortium members.
The program addresses gaps in leveraging electronic health record data for population-level analyses and integrating clinical knowledge into routine clinical practice. Required activities include determining fundamental questions that identify barriers to nationwide interoperability and electronic exchange of health data; engaging the health IT industry and academic researchers to develop innovative solutions addressing interoperability barriers; and disseminating findings while fostering collaboration and implementation of lessons learned.
Merit review evaluates Understanding of Project Purpose (10 points), Proposed Approach and Activities (40 points), Applicant Capabilities (30 points), and Budget Narrative (20 points). Recipients must maintain monthly check-ins with ONC, submit quarterly programmatic progress reports, develop open-source software solutions, and publish peer-reviewed findings.
The Assistance Listing / CFDA number is 93.345.

Update #2 · Latest ·
While working to implement Cures Act provisions, ONC identified gaps with respect to leveraging EHR data to support population-level analyses and delivery of services, as well as integrating clinical knowledge into routine clinical practice. The reasons for these gaps range from a lack of data standards and interoperability to the digitization, integration, and presentation of new evidence into clinical workflows in safe, useful, and useable ways. Therefore, this funding opportunity will support innovative and breakthrough solutions critical to maximize the potential of health IT and achieve the goal of a transformed health care delivery system through various methods, such as:• Determining the fundamental questions, the answers to which will identify barriers to nationwide interoperability and electronic exchange of health data.• Engaging the health IT industry, along with academic researchers, to identify and develop innovative solutions that address barriers to interoperability.• Disseminating findings from research while fostering collaboration, advancement, and implementation of solutions and lessons learned with the health IT industry.
