About this opportunity

The Health Resources and Services Administration is soliciting applications for FY 2027 State and Regional Primary Care Association cooperative agreements to develop and deliver technical assistance supporting health center program recipients and look-alikes, with applications due on a date not specified in this forecast.

Eligible applicants include domestic public entities, private nonprofits (501(c)(3) and non-501(c)(3)), for-profit organizations, higher education institutions, county and city governments, tribal governments (federally recognized and not federally recognized), and special district governments. Applicants may be new organizations or current PCA funding recipients. Individual applicants are ineligible. Applications may originate from the 50 states, District of Columbia, Puerto Rico, Northern Mariana Islands, American Samoa, Guam, U.S. Virgin Islands, Federated States of Micronesia, Republic of the Marshall Islands, or Republic of Palau.

PCAs will improve health of medically vulnerable individuals and communities by increasing access to preventive and primary health care services; preventing and managing chronic disease; recruiting, retaining, and training health center workforce; preparing for, responding to, and recovering from emergent health events; implementing value-based care delivery and payer strategies; and accelerating data-informed operational and clinical quality improvements.

Applicants are evaluated on ability to provide technical and non-financial assistance to health centers in specified areas, with activities supporting stated objectives and advancing the HRSA mission. Applications undergo independent objective review. Awards to support projects beyond the first budget period are contingent on congressional appropriation, compliance with applicable requirements, satisfactory progress toward objectives, and government determination that continued funding is in the government's best interest.

The program operates under CFDA 93.129. This opportunity is subject to E.O. 12372 intergovernmental review where applicable. Entities expending $1 million or more in federal awards annually must undergo single or program-specific audit; those expending less than $1 million are exempt. Award recipients must maintain grant accounting records for three years after submitting the Federal Financial Report, or longer if litigation or claims are pending.
