About this opportunity
This Federal Grant Opportunity is issued by the Centers for Disease Control and Prevention's (CDC) National Center for Injury Prevention and Control to fund the adaptation, implementation, and evaluation of the STEADI (Stopping Elderly Accidents, Deaths, and Injuries) fall prevention initiative in assisted living facility settings. The key objectives are to: 1) assess the feasibility, acceptability, and usability of implementing STEADI in at least two diverse assisted living facilities; 2) adapt and implement the STEADI algorithm and materials in those settings; and 3) conduct a process and outcome evaluation focused on measures such as falls, fall injuries, medically treated falls, and transfers to the hospital. Eligible applicants include a wide range of organizations such as state/local governments, non-profits, and higher education institutions. Applications will be evaluated on factors including the proposed methodology, expertise of the research team, and plans for dissemination.
The maximum award amount is $700,000 per year for up to 4 years, with a total available funding of $2.8 million. The project period is anticipated to be September 30, 2025 through September 29, 2029, and the work will be conducted in assisted living facility settings across the United States.

Update #2 · Latest ·
This is a published notice (modification) to the NOFO. The NOFO modification adds clarifying language to the Approach section (see highlighted language on page 12 of the NOFO). In addition, a Questions and Answers supporting document was uploaded to grants.gov. - The Centers for Disease Control and Prevention's National Center for Injury Prevention and Control (NCIPC) is soliciting investigator-initiated research proposals to conduct a well-designed study within two or more assisted living facilities serving a diverse patient population to adapt, implement, and evaluate STEADI-based older adult (age 65+) fall prevention. Of particular interest is research that focuses on populations experiencing high rates of older adult falls and fall injuries within these assisted living facilities. These disproportionately affected populations could include: Tribal populations; rural populations; populations disadvantaged by reduced economic stability or limited education attainment; and populations with cognitive (e.g., dementia, Alzheimer's disease), comorbidities, or physical limitations.
