About this opportunity

The National Institutes of Health, with participation from other NIH Institutes and Centers through the NIH Common Fund, is soliciting applications for the Precision Medicine with Artificial Intelligence - Integrating Imaging with Multimodal Data (PRIMED-AI) program Playbook initiative, with applications due October 9, 2026.

Eligible applicants include higher education institutions (public and private), state and local governments, 501(c)(3) and non-501(c)(3) nonprofits, small businesses, for-profit organizations, tribal governments and organizations, special district governments, independent school districts, and housing authorities. Non-domestic entities and foreign components of U.S. organizations are not eligible.

The program funds development and initial testing of discrete projects that deliver playbooks—collections of actionable guidelines, standardized protocols, and standard operating procedures—for responsible use of multimodal AI models, error mitigation, data and algorithm management, data ontology and linkage, and preparation for regulatory approval including FDA clearance. Applications must demonstrate expertise in AI-driven integration of clinical imaging with complex, multiscale health data. Collaborative investigations combining harmonization expertise for development and validation of multimodal AI tools integrating clinical imaging with other health data types are encouraged.

The funding instrument is a UG3 Cooperative Agreement under CFDA 93.310 (Trans-NIH Research Support). Recent awards under this program range from approximately $25,000 to $8.0 million in total obligated costs, with project durations typically spanning 2–5 years. Applications undergo scientific merit review by peer review groups and program relevance assessment by national advisory councils, with review cycles lasting four to nine months. Grantees must comply with OMB 2 CFR 200 audit requirements and retain financial and programmatic records for three years from annual Financial Status Report submission.

Synopsis

The overarching goal of this notice of funding opportunity (NOFO) and its companion opportunities is to establish the Precision Medicine with AI: Integrating Imaging with Multimodal Data (PRIMED-AI) Program to support development of innovative, reliable, cost-effective, and sustainable multimodal AI-based clinical decision support (CDS) tools. PRIMED-AI CDS tools are based on the integration of clinical imaging with other types of multimodal health data to enhance care for patients with a wide range of health conditions. The PRIMED-AI Program seeks to catalyze the adoption of AI-based CDS tools into clinical workflows to enable novel personalized medicine strategies that address significant health challenges. The purpose of this NOFO is to solicit applications for the design, development, and preliminary validation of robust frameworks for the application of multimodal-artificial intelligence (AI) models for clinical use. Frameworks developed through these awards would populate a "playbook", defined as a collection of actionable guidelines, standardized protocols, and/or standard operating procedures (SOPs) for reliable and effective development and deployment of multimodal AI tools. It is expected that the frameworks delineated in this playbook will directly address PRIMED-AI objectives and needs, while also remaining flexible enough to enable sufficient extensibility and interoperability for use across a broad spectrum of multimodal biomedical AI applications, both internal and external to the PRIMED-AI Program.

Eligibility

Refer to Section III. Eligibility Information in the NOFO for additional information on eligibility.Foreign Organizations/International Collaborations:Non-domestic (non-U.S.) Entities (Foreign Organizations) are not eligible to apply.Non-domestic (non-U.S.) components of U.S. Organizations are not eligible to apply.Foreign components, as defined in the NIH Grants Policy Statement, are not allowed.
