COMBINED SYNOPSIS/SOLICITATION:
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/quotes are being requested and a written solicitation is attached. Solicitation RFQ FA4600-16-Q-0017 incorporates provisions and clauses effective through Federal Acquisition Circular (FAC) 2005-83, dated 2 Jul 15. This requirement is set-aside 100% for Small Business Owned Business. North American Industry Classification System Code (NAICS) is 811219 - Other Electronic and Precision Equipment Repair and Maintenance, with a size standard of $20.5M. All qualified small businesses under this industry are encouraged to submit quotes.
DESCRIPTION OF REQUIREMENT:
Steris Sterilizer Maintenance Service
The 55th Medical Group Biomedical Equipment Office is seeking a potential qualified contractor that can provide comprehensive recommended preventative maintenance inspection services for (3) Steris System 1E Germicidal Liquid Sterilizing Units, (2) Steris Chimeron 20 Bulk Steam Sterilizers, and (2) Steris Reliance Vision Sterilizing Unit Washers. The contractor is responsible for providing all parts, labor, travel, tools, telephone support, and preventative maintenance inspections. Any prospective contractors must be authorized/certified to provide the required service from the manufacturer and must show proof with any provided documents at the time of response.
APPLICABLE LABOR LAWS:
This solicitation is subject to the Service Contract Labor Standards FAR 52.222-41
Wages under this contract are subject to the attached prevailing wages. REFERENCES:

SPECIAL INSTRUCTIONS:
Offerors shall be required to return the attached Standard Form 1449 Solicitation FA4600-16-Q-0017, signed and filled in appropriately as their offer along with any additional information required to be submitted by FAR 52.212-1 Instructions to Offerors Addenda and any additional fill-in provisions.
All contractors must be registered in the System for Award Management http://www.sam.gov database prior to any contract award.
Offerors MUST provide offers via e-mail ONLY
QUESTIONS DUE DATE AND TIME:
All questions concerning this solicitation shall be sent by 15 July 2016 at 1200 Noon CST.
A question and answer (Q&A) will be posted approximately 2 business days after all questions are in.
OFFER DUE DATE AND TIME:
Offers are due 28 July 2016 at 1400 (2pm) CST.
LATE OFFERS:
Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in this solicitation by the time specified in the solicitation. Quotations/offers or modification of quotes/offers received at the e-mail address specified for the receipt of offers after the exact time specified for receipt of offers WILL NOT be considered (ref. FAR 52.212-1(f)).
