Synopsis: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.
This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-04. The associated North American Industrial Classification System (NAICS) code for this procurement is 621330 Offices of Mental Health Practitioners (except Physicians).
The Government contemplates an award of a Firm-Fixed-Priced contract resulting from this solicitation. This is a Service-Disabled Veteran-Owned Small Business Set-Aside (SDVOSB). Offerors must be listed as verified in the VIP database (www.vip.vetbiz.gov) when an offer is submitted and at the time of contract award. Furthermore, offerors must be registered in the System for Award Management (SAM) at www.sam.gov, and in active status, at the time of submittal in order to be considered.
The Northern Arizona VA Healthcare System is seeking a contractor to provide annual and initial psychological assessments for current and potential police officers. The Request For Quote (RFQ) is attached to this notice.
Responses to this notice shall include sufficient evidence that they possess adequate resources, (virtual) capability, experience, responsibility, and integrity to meet the technical capabilities to comply with the SOW requirements.
Responses shall be e-mailed to the attention of Tanya Torres at tanya.torres2@va.gov. Telephone responses will not be accepted. Responses must be received in writing no later than 10:00 AM PST on Friday 3 June 2022.
