The base year period of performance runs September 30, 2026 through September 29, 2027. Key deliverables include regional outreach workplans, assessment tools, training curriculum modules for CHWs, a functional database system, and quarterly progress reports. Contractors must attend monthly meetings with VA CPIVH Co-Leads.
Responses must include company socioeconomic status, legal name, SAM registration, primary point of contact, capabilities statement, relevant experience, project references, and subcontracting plans. SDVOSB and VOSB firms must be SBA-verified. Submit responses to Lydia Anderson (Lydia.anderson@va.gov).
