It is the intent of the Phoenix Area Indian Health Service to acquire fully qualified Child and/or Adult Psychiatrist Physician Services for the Ft. Yuma Service Unit (FYSU) medical facilities located in and around Phoenix, Arizona. Services shall be provided on an as needed basis as scheduled by the Chief of the Department or appointee. The anticipated schedule is included in Attachment D-Statement of Work. Hours and length assignment are not guaranteed. The attached Rate Schedule is provided and highly encouraged to be used. If not your submission must clearly identify the necessary pricing elements.
All responses to this RFQ must contain, a complete profile and current work history of the candidates(s) presented, including licensure and certifications. Selected candidates shall undergo IHS mandated security clearance PRIOR TO COMMENCEMENT OF WORK on-site at the PIMC. The security pre-clearance requirement must be adhered to. The security pre-clearance process includes a fingerprint check and a screen check of the candidate(s) through the Office of the Inspector General's exclusion list at http://exclusions.oig.hhs.gov/. Documented evidence of the clearances shall be provided to the Contracting Officer at the Phoenix Area Indian Health Service, 40 North Central Ave., Phoenix, AZ 85004.
The Government reserves the right to issue multiple awards against this solicitation for a period of 60 days after the issuance of the first contract until the needs and scheduling requirements are achieved or can award a single Purchase Order (PO). The Period of Performance stated assumes all requirements can be met within that period in regards to onboarding and background checks. It is subject to change based on availability of candidate(s) and the background check process. Award(s) will be made based on the best value considering the candidate(s) qualifications, availability and price for the Government. This solicitation closes on 02/01/16 at 12:00PM, Phoenix, Arizona, Local Time.
