About this opportunity
The Department of Agriculture Forest Service seeks quotations for pre-season Self-Contained Breathing Apparatus (SCBA) physical and fit testing services across the Shasta-Trinity, Modoc, and Klamath National Forests, with quotes due May 13, 2026, and questions due May 8, 2026.
This is a Total Small Business Set-Aside (100 percent), with NAICS code 624230. Place of performance is Redding, California, with travel required to six specified locations across the three forests. The contractor must provide on-site medical evaluations, SCBA mask fit testing, and spirometry testing. Required personnel include a Physician, Registered Nurse, Nurse Practitioner, or Physician Assistant, with a Medical Director overseeing the program.
Scope comprises 52 new user physicals and health screenings plus 143 fit tests and 143 spirometry tests at Shasta-Trinity; 35 new user physicals, 35 fit tests, and 35 spirometry tests at Modoc; and 45 new user physicals, 99 fit tests, and 99 spirometry tests at Klamath. The contract period runs May 18, 2026, to September 1, 2026, on a firm-fixed-price basis. Award may be made by location grouping, potentially to separate contractors: Shasta-Trinity (items 1–6), Modoc (items 7–11), and Klamath (items 12–15).
The contractor must provide clinical-grade spirometry equipment, quantitative SCBA fit test machines, and medical equipment to measure physiological biometrics. Travel costs are charged per trip; a single travel fee applies regardless of services performed that day. An optional Cancelled Appointment Fee may be charged if 24-hour notice of cancellation is not provided.
Quoters must be registered in SAM and submit technical proposals demonstrating past performance on three relevant contracts within the preceding five years, along with detailed pricing. Service Contract Act prevailing wage rates apply (Shasta County and Siskiyou/Modoc/Trinity County determinations).
