This is a notice that Request for Quote (RFQ) 36C263-19-Q-0031 has been amended. This Amendment, 36C263-19-Q-0031 A00001, dissolves the small business set aside on the original RFQ, opening the procurement to quotes from large businesses. It also extends the proposal due date to Monday, December 17, at 5:00PM CST. The original notice for the RFQ follows below.

The Department of Veterans Affairs (VA) and the VA Sioux Falls Health Care System, (VASFHCS) plans to issue a Request for Quote (RFQ) to obtain Mobile Lithotripsy services to eligible beneficiaries. The VA anticipates one fixed-price IDIQ contract award. The Contractor shall provide Lithotripsy Technologist and Equipment to provide Mobile Extracorporeal Shock Wave Lithotripsy (ESWL) Services on-site in accordance with the specifications contained in the Performance Work Statement in the attached RFP 36C263-19-Q-0031.
This is a COMBINED SYNOPSIS/SOLICITATION notice for commercial services prepared in accordance with FAR Parts 12 Commercial Item and 13 Simplified Acquisitions, supplemented with additional information included in this notice. This announcement constitutes the only solicitation.
This solicitation will be an unrestricted acquisition, small business will be preferred. The solicitation will be issued on or about November 16, 2018, with an anticipated proposal due date of November 29, 2018 at 5:00 PM CST via Federal Business Opportunities: https://www.fbo.gov/
The solicitation contains requirements for a 12-month base year with four (4) twelve-month options to renew. Service will begin January 1, 2019 (or date of award) through December 31, 2019 for the Base Year.
The Offeror is responsible for downloading all documents including amendments without further notification from the VA Contracting Officer. The RFP must be completed in its entirety and included in the response. Proposals must be submitted in writing to the issuing office, be in accordance with the preparation instructions outlined in the solicitation, and be timely. It is the responsibility of the offeror to ensure their proposal is received in full by the Contracting Office before the solicitation closing. Late proposals will not be accepted.
The North American Industry Classification System (NAICS) code assigned to this procurement is 621999 "All Other Miscellaneous Ambulatory Health Care Services," with a size standard of $15.0 million. Offerors will be evaluated on technical ability, past performance, socio-economic status and price.
Contractors must be registered in the System for Award Management (SAM). Lack of registration in the System Award Management (SAM) database (www.sam.gov) will make a vendor ineligible for award. https://www.sam.gov/
This solicitation incorporates provisions and clauses in effect through FAR, VAAR and Federal Acquisition Circular 2005-101. It is the contractors responsibility to be familiar with the applicable clauses and provisions. The clauses may be accessed in full text at these addresses:

The contractor(s) selected, will be required to undergo a security background screening which will include completion of fingerprint cards before beginning performance under the contract.
