Combined Synopsis/Solicitation
Health and Wellness Program Support Services at DSCR
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number SP4703-15-R-0018 is issued as a request for Proposal (RFP).
This acquisition is set-aside 100% for small businesses. The North American Industry Classification System (NAICS) Code is 621999. The small business size standard is 15M. This is a firm fixed-price (FFP) contract for one year from the date of contract award. The period of performance will be 12-month base period and four 12-month option periods. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-24. For applicable clauses and provisions, refer to list of applicable clauses attached. This solicitation contains provisions for Reverse Auction. See DLAD 52.215-9023 on clauses section.
Pricing for all Line Items must be submitted with the offer. Partial proposals will not be accepted. The estimated start date for any contract resulting from this solicitation is September 30, 2015. Contractors should use this date for the development of their pricing proposals. Sample Pricing Sheet attached.
Award will be made to the lowest priced offeror that meets the requirement stated in the Statement of Work and this combined synopsis/solicitation.
This requirement is to provide support for the Installation Support Richmond Health Promotion Program at DSCR, 8000 Jefferson Davis Highway, Richmond, Virginia 23297. The contractor shall assist in the development and provide evaluation of the Health Promotion Program. Refer to attached Statement of Work for further details.
The contractor must be registered in the Central Contract Registration (CCR) and Online Representations and Certifications Application (ORCA) prior to award. If not already registered in CCR and ORCA, go to System for Award Management (SAM) website www.sam.gov to register prior to submitting your quote.
All inquiries must be submitted 5 working days prior to closing date.
Proposals are due on August 31, 2015, 5:00 PM, EST.
