About this file

This is a Worker Protection and Investment Certification Form issued by the Commonwealth of Pennsylvania in compliance with Executive Order 2021-06, applicable to the contract opportunity 23HN Howard Nursery Garage Doors. The form requires contractors and grantees to certify compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and all applicable state labor and workforce safety laws, including the Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, and eleven additional labor statutes. The project itself involves replacing four existing overhead garage doors at Howard Nursery facilities, including installation of new door operators and all electrical requirements. Door specifications vary by location: Door #1 measures 12'2" wide by 12' high with white commercial smooth ribbed panels; Door #2 measures 10'2" wide by 12' high with similar specifications; Door #3 measures 8' wide by 7' high; and Door #4 measures 8' wide by 8' high with flush wood grain panels and polyurethane insulation. Technical questions regarding project specifications should be directed to Brian Stone, PGC Howard Nursery Manager, at 814-355-4434 Ext. 31101 or bstone@pa.gov.

The certification form mandates that an authorized official of the contractor or grantee execute the certification under penalty of law, with understanding that false certifications subject the contractor to contract termination and three-year ineligibility to bid on future Commonwealth contracts under 62 Pa. C.S. 531. Certifying officials must also report any changes in compliance status to the Purchasing Agency immediately and acknowledge that the certification is subject to provisions and penalties under 18 Pa. C.S. 4904 regarding unsworn falsification to authorities. All quantities for the garage door project are estimated, and no specific pricing terms, award dates, or contract duration information is provided in the available documentation.
