About this file

The Worker Protection Form is a certification document issued by the Commonwealth of Pennsylvania, specifically addressing worker protection and investment requirements under Executive Order 2021-06. The form mandates that contractors and grantees certify compliance with numerous Pennsylvania state labor and workforce safety laws, including regulations related to workplace misclassification, employment of minors, minimum wage, prevailing wage, equal pay, and various industry-specific labor protections. The certification requires authorized officials to affirm their organization's adherence to these legal standards and understand potential penalties for false declarations.

The document outlines significant legal consequences for non-compliance, including contract termination and a three-year prohibition on bidding for state contracts under debarment provisions. Contractors must proactively report any changes in their compliance status to the purchasing agency and acknowledge that false certifications are subject to legal penalties under Pennsylvania's unsworn falsification statutes. The form serves as a critical mechanism for ensuring workplace safety, fair labor practices, and legal accountability among contractors and grantees doing business with the Commonwealth of Pennsylvania.
