The document mandates that all contractors certify their adherence to various Pennsylvania state labor and workplace safety laws, including 13 specific statutes ranging from the Construction Workplace Misclassification Act to the Apprenticeship and Training Act.

The certification requires contractors to confirm compliance with labor regulations, with explicit penalties for false certifications, including potential contract termination and a three-year prohibition on bidding for future Commonwealth contracts. Contractors must sign the form, affirming their understanding of labor law requirements and agreeing to immediately report any changes in their compliance status. The form is officially published by the state on 02/07/2022 and is subject to legal provisions that impose penalties for providing false information to authorities.
