About this file

This document is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for contractors and grantees. It outlines that contractors and grantees must certify they are in compliance with various Pennsylvania labor and workplace safety laws, including the Unemployment Compensation Law, Workers' Compensation Law, and other applicable state laws. The certification requires the contractor/grantee representative to affirm their authority to execute the certification and that the contractor/grantee is compliant with the listed laws. Failure to provide truthful certification can result in contract termination and three-year ineligibility to bid on Commonwealth contracts.

The document provides details on the specific labor laws that contractors/grantees must comply with, including the Construction Workplace Misclassification Act, Minimum Wage Act, Prevailing Wage Act, and others. It outlines the penalties for providing false certifications under Pennsylvania's Unsworn Falsification to Authorities statute. The certification must be signed, dated, and include the printed name and title of the certifying official, as well as the name of the contractor/grantee.
