A duly authorized official of the bidding contractor must execute the certification, confirming the contractor's compliance status with all applicable Pennsylvania state labor and workplace safety laws.

The certification form requires the executing official to provide their signature, printed name, title, and the contractor/grantee's name. Pennsylvania law establishes significant penalties for false certifications, including contract termination and three-year ineligibility to bid on Commonwealth contracts under 62 Pa. C.S. 531 (Debarment or Suspension). Certifying officials must understand that the certification is subject to the provisions and penalties of 18 Pa. C.S. 4904 (Unsworn Falsification to Authorities). Additionally, contractors must immediately report any changes in their compliance status to the Purchasing Agency. Electronic bid responses are accepted through the PA Supplier Portal, and all bidders must be registered vendors to submit bids for the fluorescent reflective materials solicitation.
