About this file
This is a Worker Protection and Investment Certification Form (BOP 2201) issued by the Commonwealth of Pennsylvania, applicable to the District 3 Uniform/Shop Towel Rental contract opportunity for the Pennsylvania Department of Transportation (PennDOT). The form serves as a mandatory certification requirement for contractors and grantees engaging with the Commonwealth to demonstrate compliance with Pennsylvania labor and workforce safety standards. All contractors bidding on this PennDOT uniform rental and laundering services contract for Engineering Districts 3-5 (Snyder County), 3-8 (Union County), and 3-4 (Northumberland County) must complete and submit this certification as part of their proposal.
The certification requires an authorized contractor representative to affirm compliance with thirteen specific Pennsylvania labor and workplace safety laws, including the Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, and various other worker protection statutes. Contractors must also verify compliance with Pennsylvania's Unemployment Compensation Law and Workers' Compensation Law. The form carries significant legal consequences for false certification, including potential contract termination and three-year debarment from bidding on Commonwealth contracts. Additionally, contractors must report any changes to their compliance status to the purchasing agency immediately and understand that all certifications are subject to penalties under Pennsylvania law for unsworn falsification to authorities.
