About this file
This document is an Amendment 01 Questions and Answers file for a Urine Analysis Lease Blanket Purchase Agreement (Solicitation Number W91YTZ25QA099) issued by the Department of the Army Medical Command. The document provides clarifications to three specific vendor questions regarding the solicitation.
Key details include historical test volume information, with the government reporting 2,773 urine chemistry and microscopic tests from August 2024 through July 2025, and 3,161 tests in calendar year 2024. The government confirmed this is a Blanket Purchase Agreement and clarified that the SF 1449 form mentioned in the submission instructions is template language and not an actual included document. The amendment serves to provide official responses to vendor inquiries about the solicitation's requirements and expectations.

Question 1: Would a Fixed Monthly Lease Rate be an acceptable price structure?
Government Response Question 1: This combined Synopsis/Solicitation is for a Blanket Purchase Agreement.
Question 2: Can you please confirm the actual annual Chemistry and Microscopic test volume?
Government Response Question 2: From August 2024 through July 2025, we performed 2,773 urine chemistry tests and 2,773 automated urine microscopic tests.
In calendar year 2024, we performed 3,161 urine chemistry tests and 3,161 automated urine microscopic tests
Question 3: Reading through submission instructions and noticed the requirement to submit response with SF 1449 but I didn't see a SF 1449 included with government provided materials. Can you please advise if there should be a 1449 can you provide?
Government Response Question 3: That is template language. There is no 1449 with a combined synopsis/solicitation.
