STATEMENT OF WORK (SOW) FOR SUPPLIES

ILLINOIS AIR NATIONAL GUARD (ILANG) UNIT/DIRECTORATE

DATE: 24 October 2018___________

TITLE: Walk In X-Ray Vault

1. PURPOSE. Provide an enclosed safe facility for industrial X-ray operations.

2. SCOPE OF WORK. The contractor shall provide all materials and support necessary to meet the required specifications listed below.

3. DELIVERY LOCATION AND DELIVERY DATE. This equipment shall be delivered to the Illinois Air National Guard NDI Lab, Building P-26, located at 3101 J David Jones Parkway., Springfield, Illinois 62707-5001 and not later than 31 December 2019.

4. QUANTITY REQUIRED: Quantities are listed below.

5. SPECIFICATIONS REQUIRED. The X-Ray vault shall be of modular design with outer dimensions of 10 feet in width by 12 feet in length and 9 feet high. The walls, ceiling and door must be of steel/lead/steel construction with a minimum of ¼ inch thick lead sheeting. Ensure lead thickness meets or surpasses Federal safety regulations in CFR 21.1020.40 and all other applicable State, Federal and Military safety requirements, to include Technical Order (T.O.) 33B-1-1. There is to be no exposed lead. All walls, ceiling and door shall provide primary shielding for 160 kVp 5 mA X-ray generator located at 45 inches from wall or door. Government furnished TSG 50105D Interlock System, Contractor installed.

5.1 One sliding door, minimum clear opening size of 60”W X 84”H with safety reversing switch. Door operation will be electric motor driven. One CDRH X-Ray dual interlock switch on door. Door shall have a safety bumper strip on both leading and trailing edges, to either stop the closing of the door or to reverse direction. Door will be painted safety orange. Doorway shall be constructed with a “roll-over” style threshold. Door will be located at one corner of the vault and while door is opened, the door will not travel past the envelope of the vault. Location of the door will be on the 12 feet, broad side. Sliding door shall have locks installed. Door controls shall be on the outside of the door and shall be in steel conduit to the interface panel. A second door shall be installed on the 10 feet wide side and suitable for personnel to enter and exit. This door shall swing open outwards. Personnel door will also have a lock installed. Door shall have one CDRH X-Ray dual interlock switch installed.

5.2 One load center with a minimum of 16 circuits (120 / 240 rated for at least 60 amps total load)

5.3 One exterior junction box with terminal block for interface of X-Ray on lights, door interlock switches and interior alarm. Location of junction box to be determined.

5.4 Overhead LED lighting to be the equivalent of 8 ea. 4’ fluorescent tubes, with wall mounted switch.

5.5 20A duplex outlets in steel conduit. 120V/240V 30A outlets on inside and outside near pass through.

5.6 All safety lights shall be wired with a 4 wire plus ground. Lights shall be wired in such a manner that should a light be defective the X-Ray cycle will not begin.

5.7 Government supplied Emergency Shut off switch (E-Stop) with integrated audible alarm will be wired in series with interlock mounted on interior wall opposite of the Personnel door. E-Stop switch shall be mounted at eye level, approximately 5 feet above the floor. One government supplied Federal Signal Model 121S-120R red rotating safety light(s) mounted inside the room on a ½ inch threaded NTP pipe above the Emergency Shutoff switch, and two government supplied Federal Signal Model 121S-120R red rotating safety lights mounted on ½ in threaded NTP pipe on the exterior of the room. Location of the exterior light(s) to be above each door. Wiring for the lights shall terminate on the exterior where the government supplied TSG interlock is installed.

5.9 Cable entrance baffle must have the same effective shielding as the wall it is installed on. Location to be determined.

5.10 60 amp 2 pole 240 VAC fused safety disconnect.

5.11 Holders for 2 ea. X-Ray control cables, 2 ea. cooler control cables, and 2 ea. cooler hoses, installed on one wall.

5.12 Fire Protection Sprinkler System to be installed in the interior of the vault with an external connection to the local water supply. Sprinkler to meet the requirements of NAFP.

5.13 Exterior color shall be grey and interior shall be white.

5.14 Vendor will be responsible for providing the installation of the vault and the installer will be present during the Bioenvironmental Radiation Protection Survey to correct any deficiencies identified during the survey.

5.15 Quotes and designs shall be reviewed by the Air Force NDI Program Office, 183rd Bioenvironmental Engineering Service Office and the 183rd Civil Engineering Office prior to contract award.

5.16 Suggested Product is Newco 160 kV Cabinet with Lorad LPX 160KV portable X-ray system.
