Performance Assessment Questionnaire

This document is a Performance Assessment Questionnaire issued by the U.S. Army Contracting Command–Aberdeen Proving Ground (ACC-APG) dated October 27, 2026, in support of Request for Proposal (RFP) W91CRB-27-R-A002. The questionnaire is designed to gather past performance information from references regarding offerors' performance on recent and relevant federal, state, local government, or commercial contracts comparable to North American Industry Classification System 333992. Completed questionnaires must be returned to Contracting Officer Todd M. Strasavich or Contract Specialist Noah R. Lane no later than 15 days after receipt or November 12, 2026, whichever occurs first.

The questionnaire consists of three parts: Part I requires the offeror to provide contract identification details including contractor name, address, program title, contract number, contract type, award date, completion date, nature of effort, and total contract value, along with offeror representative information. Part II requires the point of contact (respondent) to evaluate the contractor's performance across five areas—compliance with specification requirements and workmanship standards, effectiveness of project management including subcontractor control, timeliness of performance, effectiveness in forecasting and controlling costs, and commitment to customer satisfaction—using four confidence ratings (Substantial, Satisfactory, Limited, or No Confidence), with required rationales for each response. Part III requests return of the completed questionnaire to the identified Contracting Officer. The questionnaire includes detailed definitions for each confidence rating level and provisions for general comments and identification of other relevant past performance sources.
