3.3. VOLUME 1 FACTOR 4: PAST PERFORMANCE – PRIME CONTRACTOR

3.3.1. Submission Requirements

The Offeror shall submit past performance evaluations for the project or projects that it has identified in its submission in response to Factor 2 “Past Experience – Prime Contractor” (Paragraph 3.1.1).

The Offeror may also submit up to two (2) additional past performance evaluations on construction projects that the Offeror considers to be similar in purpose, size, scope (including dollar value) and complexity. Joint Ventures must submit at least one project for each Joint Venture partner if the joint venture itself does not have past performance of a similar type project as defined below. Projects that are substantially similar to the following may be considered more relevant:

a. Renovation or construction of SCIF/CAA or their equivalent or similar commercial or institutional type.

b. Typical project size should reflect a minimum of 2,000 square feet per building.

c. Construction value of at least $1,000,000.00.

The following information may be considered:

If the Offeror submits more than two (2) additional past performance evaluations, the Government will evaluate only the two (2) most recent ongoing or completed projects.

Projects completed for DOD, Department of the Army and USACE customers will be considered more relevant than projects completed for other customers.

Past performance information for unfinished projects may be submitted, however to be considered recent, the unfinished projects must be at least 50% complete by the time of the issue date of this Solicitation. Past performance information about unfinished projects may not be considered as relevant as information about completed projects.

Past Performance information about projects completed closer in time to the closing date for submission of proposals under this Solicitation will be considered to be more recent and relevant than past performance information about earlier projects.

If available submit Contract Performance Assessment Reporting System (CPARS) Performance Evaluations ratings. For projects which were constructed for other government entities, submit the performance appraisal documentation used by that government entity if available. For projects which are not covered in the CPARS database or other Government Performance Rating System, submit a Past Performance Questionnaire (Form

PPQ-0).

3.3.1.1 CPARS Evaluations

Offerors are requested to retrieve their CPARS Past Performance information directly from the Past

Performance Information Retrieval System (PPIRS) at http://www.ppirs.gov. PPIRS is an electronic repository of performance information collected by all the major federal performance reporting systems.

Logging onto PPIRS will require the following: All Offerors must have purchased and installed a Public Key

Infrastructure (PKI) certificate. If the Offeror does not have this certificate, it cannot access its information.

Additional information about the PKI certificate is posted in red at the top, center portion, of the http://www.ppirs.gov web page. The Offeror will also need its DUNS number and Marketing Partner

Identification Number (MPIN) to log onto PPIRS. The MPIN number was selected by whomever registered you’re the Offeror in the Central Contractor Registry at http://www.sam.gov. If the Offeror does not know its

MPIN number, it will need to contact the SAM help desk by emailing them from the email link on the https://fsd.gov/fsd-gov/home.do web page. Please be aware that the SAM help desk will only release the MPIN number to the person who originally registered the Offeror.

There are two other ways to obtain a copy of CCASS evaluations as follows: (1) Contact the Offeror's

Government point of contact for the project it has mentioned, and request that the point of contact if can send the

Offeror a copy; or (2) Apply for “Contractor Corporate Senior Management Representative” access on the following CCASS web page: http://www.cpars.csd.disa.mil/. This type of access is issued to only one person within the firm, typically a member of senior management. In addition to access to the Offeror's completed

CCASS performance evaluations, the Offeror will also be able to view status of evaluations which have not yet been completed. Following receipt of the Offeror’s faxed application request, the Offeror’s point of contact will be emailed logon and access instructions. This type of access will only let the Offeror see CCASS information, so it is not as complete as PPIRS access which allows access to evaluations prepared by non-DOD federal agencies, as well as the DOD agencies which use CCASS. It is a DOD requirement that the Offeror will need to have a PKI certificate to access the CCASS system.

3.3.1.2. Past Performance Questionnaire (PPQ) (Form PPQ-0)

The PPQ is provided for the Offeror or its team members to submit to clients. The PPQ should be completed by a client or client’s representative who is not affiliated with the Offeror's firm. Ensure correct phone numbers and email addresses are provided for the client point of contact.

Clients must submit completed directly to the following Government points of contact by e-mail prior to proposal closing date:

Mr. Nicholas I. Johnston, Nicholas.I.Johnstone@usace.army.mil; and Ms. Kim, Tok-

Kyong Tok-Kyong.Kim@usace.army.mil.

(Note: The PPQ Form is the ONLY submission which the Government will accept by e- mail. Please refer to paragraph 3.5.1.2, above.)

The Offeror is solely responsible for ensuring that the evaluating agency has forwarded the PPQ prior to the closing date of the solicitation.

If it appears that a client will be unable to submit (a) completed PPQ(s) to the Government before the proposal closing date, the Offeror should complete and submit with its proposal the first page of the PPQ, which must provide contract and client Information for the respective project(s).

The Government will not evaluate any PPQs, previously submitted in response to other solicitations, which the

Offeror incorporates by reference into its proposal PPQs.

However, this does not preclude the Government from otherwise utilizing previously submitted PPQ information in its past performance evaluation.

3.3.1.3 Government Utilization of Alternate Information Sources for Past Performance

The Government will assess the past performance on projects submitted under the Past Experience factor and http://www.ppirs.gov/ http://www.ppirs.gov/ http://www.sam.gov/ https://fsd.gov/fsd-gov/home.do http://www.cpars.csd.disa.mil/ mailto:Nicholas.I.Johnstone@usace.army.mil mailto:Tok-Kyong.Kim@usace.army.mil.

reserves the right to conduct telephone interviews with points of contact identified in the proposal or to review personal knowledge. In addition, the Government may review any other sources of information for evaluating past performance of the Offeror on the submitted projects and any other previous work.

Other sources may include, but are not limited to, past performance information retrieved through the Past

Performance Information Retrieval System (PPIRS), using all CAGE/DUNS numbers of team members (joint venture, or parent company/ subsidiary/ affiliate) identified in the Offeror's proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic

Subcontract Reporting System (eSRS), and any other known sources not provided by the Offeror.

3.4. VOLUME 1 FACTOR 5: KEY SITE PERSONNEL QUALIFICATIONS

3.4.1. Submission Requirements
