Additionally, in light of the current COVID-19 Pandemic, it is requested that contractors limit the amount of individuals coming for a site visit to no more than two (2) individuals. The contractor assumes responsibility for all quantities, materials, critical dimensions, and site conditions used to calculate their quote and perform the requested work. All contractors may call the Technical Point of Contact (TPOC) for an appointment. The TPOC’s contact information is as follows:
Ellen K. Radowski, IT Chief Phone: (502) 315-6548 Ellen.K.Radowski@usace.army.mil

Note: Page numbering in this document may not be accurate.
All contractors must be registered in the System for Award Management (www.SAM.gov) prior to award of a contract. All proposed contractors are highly encouraged to review FAR Clause 52.232-33 Payments by Electronic Funds Transfer – System for Award Management, which indicates “All payments by the Government under this contract shall be made by electronic funds transfer (EFT).” Those not currently registered can obtain registration by going to the website http://www.SAM.gov. The process can usually be completed from 24 to 48 hours after submission. Contractors will need to obtain a Unique Entity Identifier (formerly DUNS number) for processing their registration. If you do not already have a Unique Entity Identifier, one can be obtained from http://www.SAM.gov.
Refer to www.SAM.gov for information formerly found in CCR, EPLS, ORCA and FedReg.
Please begin the registration process immediately in order to avoid delay of the contract award should your firm be selected.
ALERT: You must submit a notarized letter appointing the authorized Entity Administrator before your registration will be activated. This requirement now applies to both new and existing entities. Effective 29 April 2018, the notarized letter process is now mandatory on all CURRENT registrants at SAM who have a requirement to update data on their SAM record. The notarized letter is mandatory and is required before the GSA Federal Service Desk (FSD) will activate the entity's registration.
Effective 29 June 2018, vendors creating or updating their registration can have their registration activated prior to the approval of the required notarized letter. However, the signed copy of the notarized letter must be sent to the GSA Federal Service Desk (FSD) within 30 days of activation or the vendor risks no longer being active in SAM.
Vendors can check whether an account is active by performing a query by their CAGE or Unique Entity Identifier (known as DUNS). The new registration process may now take several weeks, so vendors are highly encouraged to begin registering as soon as possible to avoid any possible delays in future contract awards. Remember, there is no cost to use SAM.
To find out additional information about the changes of the SAM registration process, contractors should visit the Frequently Asked Questions (FAQ) link located at the top of the SAM homepage (www.sam.gov).
EVALUATION CRITERIA
Evaluation for Contract Award
The contract will be awarded on the basis of “Best Value to the Government”. This means that factors other than price will be considered during the Government selection of the Contractor to be awarded the contract. Quotes shall include the following:
Completed Contract Rate Pricing and Price Breakout Schedule for the Sample Project Completed Standard Form 1449 (signed) Completed Project Details Sheet and Past Performance Questionnaires attached to this document
The Offeror provided information contained in the above listed items is necessary for the Government to assess and evaluate potential contractors under the “Best Value to the Government-Tradeoff Process” criteria described below.
Evaluation Criteria
Content and Rating. Quotes shall be specific and complete in every detail and shall contain the information necessary to properly assess and evaluate the Contractor's capability to perform the specified contract services, based upon the following factors:
Past Performance/Experience Price
In addition to a complete and properly prepared price breakout schedule, the Contractor shall also complete the “Past Performance Questionnaires” and “Project Detail Sheets” forms attached to this document. The price breakout schedule and information provided on the forms will be used to evaluate each Offeror’s quote. The Government will evaluate the quotes in accordance with the criteria described herein, and award a firm fixed price IDIQ contract to the responsible offeror, whose quote conforms with all the terms and conditions of the solicitation and whose quote is determined to represent the overall best value to the Government, all factors considered. Quotes will be evaluated in accordance with the factors contained in the following paragraphs.
Past Performance. Offerors will be evaluated on the following:
Project Detail Sheets shall be used by the offeror to provide up to five (5) previous projects which are similar in size and/or scope to this solicitation. Projects considered similar to this project include installing or removing copper outlets, floor mount pedestals, fiber cables, or new audio/video cabling. Projects considered similar in size to this project include installation of at least 15 units. Past Performance Questionnaires attached to this document shall be used by the Offeror to provide business references that can be contacted for the purpose of verifying past performance.
At least one (1) reference for each project identified for project experience. Reference information must include project name, location, points of contact, telephone number, and email address (for the point of contact). Also include any ratings, letters, awards, etc. that support past performance on these projects. A Past Performance Questionnaire Template is attached for your convenience. If used, the Past Performance Questionnaire must be submitted by the offeror with the quote submission and not sent directly to the agency from the reference.
The SSEB will next review how well the offeror performed on those projects. The Government reserves the right to check any or all cited references to verify supplied information and to assess owner satisfaction. The Government also reserves the right to not contact the provided references. In addition to the information submitted by the offeror, the Government reserves the right to review any other sources of relevant information for evaluating past performance, including projects other than those submitted by the offeror. The Government will, at a minimum, review past performance information retrieved through the Contractor Performance Assessment Reporting System (CPARS), using all CAGE/Unique Entity Identifier numbers. Other sources may include, but are not limited to, past performance information retrieved from inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and any other known sources not provided by the offeror.
The Government will consider information in the Past Performance Questionnaires completed by personnel for whom the Offeror has performed work, and any information obtained from other sources. Evaluation of past performance will be based on subjective assessment based on a consideration of all relevant facts and circumstances.
It will not be based on absolute standards of acceptable performance. The Government's conclusions about the overall quality of the Offeror's past performance may influence the selection of the quote considered “Best Value to the Government”.
NOTE: For purposes of evaluating past performance, the Prime Contractor is defined as the contractor identified in Block 17a of the Standard Form 1449.
Price. The completed Contract Rate Pricing and Sample Project Price Breakout Schedule will be reviewed for completeness and compatibility with the estimated costs to perform the contract services. The reasonableness of the quoted prices will be evaluated and compared with the Government's cost estimates. All price and non-price factors when combined will be weighted approximately equal when evaluating an Offeror’s quote, the lowest price shall not by itself determine which Offeror is awarded the Contract, except as mentioned below. The quoted prices shall demonstrate an understanding of costs to perform the specified contract services. This understanding should be reflected in the Offeror’s quoted prices for individual work items.
Overall Price Evaluations. The total evaluated price will be determined by adding the Grand Total (base plus all options) SATOC Price from the Contract Rate Pricing sheet with the Total Price for the Sample Project. The price will be reviewed for reasonableness through the use of price analysis. Price will also be checked for unbalancing of line items. Offerors are cautioned to distribute costs appropriately. Award may not be made to offerors who do not provide reasonable and realistic prices for all items listed in the Contract Rate Pricing quote, and for all line items in the Sample Project Price Breakout Schedule. Quotes that do not utilize the binding rates proposed in the Contract Rate Pricing sheet to calculate the price for the Sample Project may not be considered for award.
Discounts. Prompt payment discounts will not be considered in the evaluation of quotes. However, any offered discount will form a part of the award, and will be taken if payment is made within the discount period indicated by the Offeror.
Evaluation Process
A Source Selection Evaluation Board (SSEB) comprised of representatives of the Corps of Engineers, User/Customer, and other required personnel will evaluate the quotes. Offerors are advised that the evaluation and rating of quotes will be conducted in strict confidence. Reviews of non-price factors will be conducted without knowledge of the price offered. The number and identities of offerors will not be revealed to anyone who is not involved in the evaluation and award process or to other offerors.
The evaluation process will consist of the following four parts:
(1) Quote compliance review
a. This is an initial review to ensure that all required forms and certifications are complete.
(2) Past-performance and experience evaluation
a. The SSEB will evaluate and rate the quotes against the solicitation requirements.
i. Using the relevancy ratings and confidence assessments defined in the tables below, these non-price factors will be evaluated based on the extent of recent and relevant past experience, and the quality of the offeror’s performance during that experience.
Past Performance Relevancy Ratings Rating Definition
Very Relevant Present/past performance effort involved essentially the same scope and magnitude of effort and complexities this solicitation requires.
Relevant Present/past performance effort involved similar scope and magnitude of effort and complexities this solicitation requires.
Somewhat Relevant Present/past performance effort involved some of the scope and magnitude of effort and complexities this solicitation requires.
Not Relevant Present/past performance effort involved little or none of the scope and magnitude of effort and complexities this solicitation requires.
Performance Confidence Assessments
Rating Definition
Substantial Confidence Based on the offeror’s recent/relevant performance record, the Government has a high expectation that the offeror will successfully perform the required effort.
Satisfactory Confidence Based on the offeror’s recent/relevant performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort.
Neutral Confidence
No recent/relevant performance record is available or the offeror’s performance record is so sparse that no meaningful confidence assessment rating can be reasonably assigned. The offeror may not be evaluated favorably or unfavorably on the factor of past performance.
Limited Confidence Based on the offeror’s recent/relevant performance record, the Government has a low expectation that the offeror will successfully perform the required effort.
No Confidence Based on the offeror’s recent/relevant performance record, the Government has no expectation that the offeror will successfully perform the required effort.
(3) Price evaluation
a. The SSEB will evaluate price quotes independent of the evaluation of the non-price factors.
(4) Trade-off Analysis
a. After evaluations are complete, the Contracting Officer will compare the relative advantages and disadvantages of the quotes and prices. The Contracting Officer will then consider all factors to select the quote offering the best value to the Government.

ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT YOUR
EVALUATION OF THE CONTRACTOR’S PERFORMANCE
RATING DEFINITION NOTE
(E) Exceptional
Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating.
Also, there should have been NO significant weaknesses identified.
(VG) Very Good
Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory
Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.
(U) Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
(N) Not Applicable
No information or did not apply to your contract
Rating will be neither positive nor negative.
