GENERAL REQUIREMENTS
PART 1 GENERAL
1.1 SCOPE
This section of the Specifications provides general information and requirements. The work covered by these Specifications consists of furnishing all equipment, labor, and materials required for the fabrication and delivery of two steel bridges as shown on the drawings and as described herein.
1.2 SUBMITTALS
Government approval is required for submittals with a "G" designation;
submittals not having a "G" designation are for information only. When used, a designation following the "G" designation identifies the office that will review the submittal for the Government. The following shall be submitted in accordance with Section 01 33 00 SUBMITTAL PROCEDURES:
SD-01 Preconstruction Submittals
Fabrication Shop Storage Plan; G RE
A plan describing in detail the proposed layout of fabricated items at the Contractor's plant, for which partial payments are requested, shall be submitted for approval. The plan shall be submitted within 15 calendar days after Notice to Proceed.
Delivery Work Plan; G RE
A plan describing in detail the proposed delivery methods of the fabrications shall be submitted for approval. The plan shall be submitted at least 60 calendar days prior to the proposed delivery of any item.
1.3 INSPECTIONS AND ACCEPTANCE OF WORK
1.3.1 General
Not withstanding other provisions of this contract, all materials and work to be performed under this contract shall be inspected and accepted by the Contracting Officer or his authorized representative(s). The Contracting Officer or his authorized representative shall be the individual(s) solely authorized to reject materials, disapprove or approve any work in progress, or order or direct any revision or change to the plans and specifications as presently shown and stated.
1.3.2 Access to Work
The Contractor shall provide safe access to the fabrication facility and any other areas where work on this contract is performed to permit
SECTION 01 00 00 Page 2 inspection by the Government.
1.4 FABRICATION SHOP STORAGE
Mill steel, fabricated items, and appurtenances shall be stored in a manner to prevent permanent deflection, distortion, or other damage.
The Contractor is required to submit a fabrication shop storage plan for approval by the Contracting Officer. The plan shall include a layout of the proposed locations of all items stored at the fabrication shop, for which partial payments are requested.
1.5 STORAGE YARD
The storage site is located at the TVA Muscle Shoals Reservation in Alabama. Site operations at this storage yard will impact or be impacted by other organizations. The Contractor is responsible for coordinating with others in scheduling activities in order to minimize or eliminate conflicts. The point of contact for TVA is Wesley Jaynes Phone:
256-386-3752.
Site preparation will be performed by others and availability is projected to be 180 days from NTP. Contact the Contracting Officer for the status of the site plan and storage yard plan. The storage yard plan will show the proposed location of all items to be delivered to the storage yard, including miter gates, valves, bulkheads and appurtenances which will be fabricated under a separate contract.
1.5.1 Site Access to Storage Yard
The Contractor is advised that access restrictions will apply to both land and water based activities in the storage yard area. These restrictions are defined in the following paragraphs.
1.5.1.1 Marine Access
This site is accessible by water upstream of Wilson Dam, as shown in the contract drawings. The use or construction of other marine access locations near the storage yard area require the necessary environmental clearances and must be approved by TVA and the Contracting Officer. It is likely that permission to use an alternate marine access location will depend, at least partially, on an environmental assessment as described in Section 01 57 20 ENVIRONMENTAL PROTECTION of these Specifications.
1.5.1.2 Land Access
Land access to the TVA storage yard is accessible off Highway 133, as shown in the contract drawings. Use of any TVA lands outside the designated area requires approval from the Contracting Officer and TVA. In addition, the Contractor shall adhere to all load restrictions and speed limits on public roads when hauling material to the storage yard.
1.5.2 Storage Requirements
Fabricated components will be constructed, painted and stored on lagging (blocking) which will support them in a safe and transportable manner for up to 5 years. A corridor from gate to gate shall be left open such that components and appurtenances may be stored for easy removal. All components should be accessible without the need to rearrange them.
SECTION 01 00 00 Page 3
Adequate turning radius and distance should be preserved to access the components fabricated under this contract.
The Contractor shall follow the storage yard plan developed by others in using the storage yard. Items to be stored at this site by others include:
miter gates, culvert valves, and culvert valve bulkheads. The Government will provide the storage yard plan to the Contractor after it's approval.
1.6 DELIVERY OF FABRICATIONS
The Contractor is required to submit a delivery work plan for approval by the Contracting Officer. The plan shall describe in detail the proposed methods of delivery of the fabricated items. The plan shall address equipment, machinery and site limitations identified in the specifications and discussed at a pre-delivery meeting between the Contractor and the Contracting Officer. The plan shall be submitted at least 60 calendar days prior to proposed delivery of any item. The plan shall include, but not be limited to the following:
a. Type and size of barge(s) to be used for delivery.
b. Propelling unit to be used for delivery.
c. Location and description of intermediate assembly or handling areas (if any) between the fabrication location and the delivery location.
d. Cranes and other lifting and positioning equipment to be used.
e. Packaging of components.
f. Blocking.
g. Full assembly after delivery.
1.7 ACCIDENT PREVENTION
Accident prevention requirements that are applicable to this project during site operations are stated in Section 01 35 26 GOVERNMENTAL SAFETY REQUIREMENTS. Site operations include delivering, unloading, assembling, and blocking at the TVA storage yard.

Contractor Guidelines For:
1. The Preparation of the Accident Prevention Plan (APP)
2. The Preparation of the Activity Hazard Analysis (AHA)
APPENDIX A
HELPFUL HINTS FOR THE PREPARATION OF THE
CONTRACTOR’S
ACCIDENT PREVENTION PLAN (APP)
1. The following are minimum considerations for developing the Contractor’s Accident Prevention Plan. These helpful hints raise a number of basic questions that need to be answered for the Contractor’s accident prevention plan to be an effective management tool for use by on-site supervision. This plan shall be specific for this job.
a. Time Of Submittal. The accident prevention plan and the activity hazard analysis for the first phases of the job shall be acceptable prior to start of work. The plan and analysis shall be submitted for review within ten (10) calendar days after notice of award so that discussion can take place at the pre-construction conference. Job hazard analysis for later phases of work shall be acceptable prior to the start of that phase. It is recommended that the activity hazard analysis for the next phase of work be submitted twenty days before scheduled phase start in order to give ample time for review. The accident prevention plan shall contain a list of the phases to complete the works. Each phase shall have an anticipated start date. On short jobs one submittal covering the total job will be sufficient.
b. Responsible Individual(s). Who will be responsible for enforcing the accident prevention program and what are the basic duties? How will this person be held accountable? Include a statement that there will be compliance with pertinent provisions of the U.S. Army Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1.
c. Subcontractor Supervision. What procedures will be followed to assure that Subcontractor activities are fully integrated into the project accident prevention plan and activity hazards analysis?
d. Indoctrination of New Employees before Start of Work. Every employee is required to receive an initial safety briefing prior to starting work. The accident prevention plan shall establish the procedure for ensuring the following items are covered:
(1) General safety policy and pertinent provisions of EM-385-1-1.
(2) Requirements for employee and project safety.
(3) Employee’s responsibilities for property and safety of others.
(4) Employee’s responsibilities for reporting all accidents.
(5) Medical facilities and required treatment.
(6) Procedures for reporting or correcting unsafe conditions or practices.
LRN 385-1-2
(7) Safe clearance procedures.
(8) Fire fighting and other emergency procedures.
(9) Activity hazard analysis.
(10) Personal protective equipment.
e. On-the Job Safety Meetings.
(1) When and where will monthly safety meetings for all supervisors be held? Who will conduct the meetings and what will be covered?
(2) How will the weekly “tool box” meetings be conducted?
f. Accident Reporting. The contract requires prompt reporting of injuries, fire, and property damage. Initial reports must be made immediately to the on-site Government representative and written reports shall be submitted within one to four working days. How does the accident prevention plan reflect responsibilities assigned for immediate oral reporting, accident investigation, determining proper corrective action, and preparation of reports?
g. Sanitary Facilities. What toilet facilities will be provided considering the number and distribution of employees? What other considerations are planned for drinking water and washing facilities?
h. First Aid and Medical. Describe first aid facilities and qualifications of attendant. List telephone numbers of physician, ambulance, and hospital.
i. Housekeeping. How will access ways to work areas be maintained during work hours? What procedures will be followed to assure daily cleanup?
j. Fire Protection. Considering the availability of existing fire protection, what general types and size of extinguishers and fire barrels will be required to protect buildings, shops, and storage areas as well as to deal with special hazards such as welding and flammable liquids? Name the local professional fire fighters. List their telephone number.
k. Machinery and Mechanized Equipment. How will inspection of cranes, trucks, and other mechanical equipment be accomplished? Frequency, by whom, what type of records will be kept?
2. Posters, contests, safety awards help develop positive attitudes toward safety rules. What methods, if any, will be used on this project? Most accidents are preventable by well thought out and executed accident prevention plans.
APPENDIX B
GUIDELINES FOR THE PREPARATION OF
ACTIVITY HAZARDS ANALYSIS (AHA)
1. Activity Hazards Analysis Development. Before starting any major phase of work an activity hazards analysis shall be developed and reviewed with the Government representative. This analysis will evaluate anticipated hazards and outline the proposed methods and techniques which will be utilized to accomplish the work in a safe manner.
2. Phases of Work. Listed are examples of major phases of work, but this list is not all inclusive. Phases of work shall be tailored to the specific characteristics of the contract.
Clearing and Grubbing Earthwork Trench Excavation Blasting Concrete Placement Steel Erection Masonry Electrical Work, Exterior Mechanical Work Carpentry
3. Sample Activity Hazards Analysis. The enclosed sample shows a possible format for a phase accident prevention plan that might be submitted on a representative project. This sample incorporates a phase of work, the safety hazards that may be encountered, and precautionary actions that will be taken to overcome these hazards.
Each safety hazard identified in the third vertical column must be accompanied by the appropriate paragraph reference number from EM 385-1-1. If none exists in EM 385-1- 1, so state in the third vertical column of the Activity Hazard Analysis.
4. Indoctrination. Employees performing the work must be made aware of the activity hazard analysis. For this reason, an important part of any phase plan is the indoctrination of all employees who will be performing the work.

NETWORK ANALYSIS SCHEDULES (NAS)
PART 1 GENERAL
1.1 DESCRIPTION
The network analysis system shall consist of the network analysis schedule (diagram) and associated reports. The scheduling of all procurement and construction shall be the responsibility of the Contractor. Construction increments will be interrelated on a single schedule that represents the entire project duration from Contract Award to the Contract Completion Date. Schedule updates will build upon each other and will include construction increments as they are detailed, submitted and accepted.
Submission of progress and revision data will be used to measure work progress, aid in the evaluation for requests for time extensions, and to provide the basis of all progress payments. The Critical Path Method (CPM) of network calculation shall be used to generate the project schedule and will utilize the Precedence Diagram Method (PDM) to satisfy both time and cost applications. All progress payment amounts will be derived from and tied to the cost-loaded schedule activities.
For consistency, when scheduling software terminology is used in this specification, the terms in Primavera's scheduling programs are used.
Primavera Project Planner, P3, Primavera Project Manager, SureTrak and PrimeContract are registered trademarks or service marks of Primavera Systems, Inc. Adobe and Acrobat are registered trademarks of Adobe Systems Incorporated.

1.3 SUBMITTALS
Government approval is required for submittals with a "G" designation;
submittals not having a "G" designation are for information only. When used, a designation following the "G" designation identifies the office that will review the submittal for the Government. Submit the following in accordance with Section 01 33 00 SUBMITTAL PROCEDURES:

Monthly Network Analysis Updates; G
SD-11 Closeout Submittals
As-Built Schedule; G
1.4 SCHEDULE ACCEPTANCE
Review comments made by the Government on the Contractor's schedule(s) will not relieve the Contractor from compliance with requirements of the Contract Documents. The Contractor is responsible for scheduling, sequencing, and prosecuting the Work to comply with the requirements of the Contract Documents. Government acceptance extends only to the activities of the Contractor's schedule that the Government has been assigned responsibility and agrees it is responsible. The Government will also review for contract imposed schedule constraints and conformance, and cost loading of the CPM activities. Comments offered on other parts of the schedule, which the Contractor is assigned responsibility, are offered as a courtesy and are not conditions of Government acceptance; but are for the general conformance with established industry schedule concepts.
1.4.1 Schedule Acceptance Prior to Start of Work
The Baseline Network Analysis Schedule described in the paragraph entitled "Baseline Network Analysis Schedule" must be submitted and accepted by the Government before the Contractor will be allowed to start work on the construction stage(s) of the contract. Examples of construction stages are, but not limited to; demolition, site work, temporary work for construction, etc.
1.4.2 Acceptance
a. When the Construction Network Analysis Schedule is submitted and accepted by the Contracting Officer, it will then be considered the "Baseline Network Analysis Schedule". The Baseline Network Analysis Schedule will then be used by the Contractor for planning, organizing, and directing the work; reporting progress;
and requesting payment for work accomplished. The schedule will be updated monthly by the Contractor and submitted monthly with the progress pay request to reflect the current status of the work. Submittal and acceptance of the Baseline Network Analysis Schedule and accurate updated schedules accompanying the pay requests are both conditions precedent to processing pay requests. Only bonds will be paid prior to acceptance of the Baseline Schedule(s).
b. Submittal of the Baseline Network, and subsequent schedule updates, will be understood to be the Contractor's certification that the submitted schedule meets all of the requirements of the Contract Documents, represents the Contractor's plan on how the work will be accomplished, and accurately reflects the work that has been accomplished and how it was sequenced (as-built logic).
SECTION 01 32 17 Page 4
1.5 SOFTWARE
The scheduling software that is to be utilized on this project is Primavera Project Planner (P3) by Primavera Systems, Inc. Notwithstanding any other provision in the contract, schedules submitted for this project must be prepared using Primavera P3 (files saved in Concentric P3 format). The contractor shall provide the government with two licensed copies of the software (including any software or hardware keys). These copies are for the sole use of the government during the contract period. The contractor will retain ownership of the licenses and the government's use of the software product will stop following the closeout of the contract. The contractor is responsible for providing the government with all updates/upgrades necessary in order to keep the version the government is using current with the version the contractor is using.
The Contractor shall provide electronic files of schedule updates to the government as required by the contract. Submission of data from another software system where data conversion techniques or software is used to import into Primavera's scheduling software is not acceptable and will be cause for rejection of the submitted schedule.
1.6 QUALIFICATIONS
The Contractor shall designate a Scheduler that will be responsible for the development, preparation, and maintenance of an accurate, computerized Network Analysis Schedule. The Scheduler shall have previously developed, created and maintained at least 2 previous computerized schedules of similar size and complexity of this contract. A resume outlining the qualifications of the Scheduler and their SureTrak or P3 training certificate from an authorized Primavera trainer shall be submitted for acceptance to the Contracting Officer. If at a later date, the Contracting Officer considers the Contractor's Scheduler to be incompetent or objectionable, the Contractor will propose a new Scheduler, meeting the qualification requirements. Payments will not be processed until an acceptable Scheduler is provided.
1.7 NETWORK SYSTEM FORMAT
The system shall consist of time scaled logic diagrams and specified reports.
1.7.1 Diagrams
Show the order and interdependence of activities and the sequence in which the work is planned to be accomplished. The basic concept of the network analysis diagram will be followed to show how the start of a given activity is dependent on the completion of preceding activities and how its completion restricts or restrains the start of following activities.
Activity durations shall not be resource-driven, activities shall start according to network logic and finish when its duration has elapsed.
Diagrams shall be organized by Work Phase, sorted by Early Start Date and will show a continuous flow from left to right with no logic (relationship lines) from right to left. With the exception of the Contract Award, Start Project and End Project milestone activities, no activities will be open-ended; each activity will have predecessor and successor ties. The diagram shall clearly show the activities of the critical path and must be red in color. Once an activity exists on the schedule it may not be deleted or renamed, and must remain in the logic. No more than 20 percent of the activities may be critical or near critical. Critical will be
SECTION 01 32 17 Page 5 defined as having zero days of Total Float. "Near critical" will be defined as having Total Float in the range of 1 to 30 days. Show the following information on the diagrams for each activity:
a. Activity ID
b. Activity Description
c. Original Duration in Work Days
d. Remaining duration
e. Actual Duration in Work Days

Provide network diagrams on ANSI E sheets. Updated diagrams shall show the date of the latest revision.
1.7.2 Schedule Activity Properties and Level of Detail
Numbering shall be assigned so that, in general, predecessor activity numbers are smaller numerically than the successor activity numbers. Skip numbering shall be used on the network to allow insertion of additional activities for contract modifications and logic changes. The minimum number of construction activities in the final network diagram shall be 2500. Activity categories included in the schedule are specified below.
1.7.2.1 Activity Categories
a. Procurement Activities: Tasks related to the procurement of material or equipment shall be included as separate activities in the project schedule. Examples of procurement activities include, but are not limited to; Material/equipment submittal preparation, submittal and approval of material/equipment; delivery of O&M manuals; material/equipment fabrication and delivery, delivery of extra parts, extra stock, special tools, notification of Government Furnished Material/Equipment delivery requirement, etc. As a minimum, separate procurement activities will be provided for every specification section. If the Contractor intends on using Just-In-Time (JIT) delivery methods, the schedule will show each JIT delivery with relationship tie to the Construction Activity specifically for the JIT delivery. Material and equipment for which payment will be requested in advance of installation shall be cost-loaded with the procurement costs (e.g.; the delivery milestone(s)). All activities within a procurement process/cycle will have a unique identifier in the activity code to show their relationships and will extend to the related construction activities (i.e., CSI Code).
If the Government's action on any submittal is "Disapproved" or "Revise and Resubmit", a new series of Procurement Activities will be inserted into the schedule. Predecessor for the new submittal preparation activity will be the original approval activity and the successor of the new approval activity will be the
SECTION 01 32 17 Page 6 fabrication/deliver activity for the equipment or material.
b. Government Activities: Government and other agency activities that could impact progress shall be clearly identified.
Government activities include, but are not limited to; Government approved submittal reviews, Government conducted inspections/tests, environmental permit approvals by State regulators, utility outages, Notice(s) to Proceed and delivery of Government Furnished Material/Equipment. Show activities indicating Government furnished materials and equipment utilizing delivery dates indicated in "FAR 52.245-2, Government Property (Fixed-Price Contracts)." Government activities will be driven by calendars that reflect Saturdays, Sundays and all Federal Holidays as non-work days.
c. Construction Quality Management (CQM) Activities: CQM Activities will identify the Preparatory Phase and Initial Phase for each Definable Feature of Work identified in the Contractor's Quality Control Plan. These activities will be added to each 3-Week Look Ahead Schedule referenced in the paragraph entitled "THREE-WEEK LOOK AHEAD SCHEDULE" and will also be included in each monthly update referenced in the paragraph entitled "Monthly Network Analysis Updates". The Follow-up Phase will be represented by the Construction Activities in the Baseline Schedule and in the schedule updates.
d. Construction Activities: Construction activities shall include, but are not limited to: Tasks related to mobilization or demobilization; the installation of temporary or permanent work by tradesman; testing and inspections of installed work by technicians, inspectors or engineers; start-up and testing of equipment; commissioning of building and related systems;
scheduling of specified manufacture's representatives; Punch Out Inspection; Pre-Final Inspection, Final Acceptance Inspection;
final clean-up; training to be provided; and administrative tasks necessary to start, proceed with, accomplish or finalize the contract. No onsite construction activity shall have a duration in excess of 20 working days. Contractor activities will be driven by calendars that reflect Saturdays, Sundays and all Federal Holidays as non-work days.
e. Hammock (Summary) Activities: The Contractor shall include special activities that are a summary of a chain of activities.
The start of the activity will be the start date of the first activity in the chain and the finish date will be the finish date of the last activity in the chain. Generalized work sequences, Area Codes and Phase Codes will be summarized.
1.7.2.2 Project Milestones
Dates shall be shown on the diagram for the start of the project, any contract required interim start and completion dates, contract completion date and other significant milestones.
a. Project Start Date Milestones: The schedule shall start no earlier than the Contract Award Date and the project duration (Day
1) will start on the Notice-to-Proceed (NTP) date. The Contractor shall include as the first milestone in the schedule, an activity named "Contract Award". Another milestone shall be included that
SECTION 01 32 17 Page 7 will be named "Start Project". The Contract Award and Project Start milestones shall have mandatory start constraint dates equal to the Contract Award and NTP dates, respectively.
b. Constraint of Last Activity Milestone: The Contractor shall include as the last activity in the project schedule, an activity named "End Project". The "End Project" activity shall have a mandatory finish constraint equal to the contract completion date for the project. Calculation of project updates shall be such that if the finish of the last activity falls after the contract completion date, then the float calculation shall reflect negative float on the critical path and if the finish of the last activity falls before the contract completion date, the float calculation shall reflect positive float on the critical path. The only predecessor activity to this activity will be either the "Contractor Early Completion" or the "Substantial Completion" milestone, whichever is used by the Contractor.
c. Early Project Completion: In the event the Contractor's project schedule shows completion of the project prior to the contract completion date, the Contractor shall include an activity named "Contractor Early Completion". The activity shall be a milestone with an unconstrained date representing the Contractor's Early Completion date. The only successor activity to this activity will be the "End Project" milestone.
d. Substantial Completion: If the Contractor elects to include an activity for Substantial Completion, then it is agreed that Substantial Completion will be the point in time that the Government considers the project is complete and ready for its intended use. The activity will be named "Substantial Completion". The activity shall be a milestone with an unconstrained date representing the Contractor's Substantial Completion date. The only successor activity to this activity will be the "End Project" milestone.
1.7.2.3 Activity Identification (ID) and Description
a. Standard Activity Coding Dictionary: The Contractor shall use the activity coding structure defined in the Standard Data Exchange Format (SDEF) in ER 1-1-11, Appendix A. This exact structure is mandatory, even if some fields are not used.
b. Standard Activity ID Dictionary: The Contractor shall submit the alphanumeric coding scheme for Schedule Activity Numbers that shall be used throughout the project. The coding scheme submitted shall list the values for each activity code and translate those values into project specific designations. Code length shall not exceed 10 characters. Once accepted, the coding scheme will be used for the duration of the project.
c. Activity Description: Each activity shall have a narrative description consisting of a Verb or work function (e.g.; form, pour, excavate), an Object (e.g.; slab, footing, under floor plumbing), and Area (e.g.; 3rd floor, northeast quadrant, basement).
SECTION 01 32 17 Page 8
1.7.2.4 Activity Code Dictionary and Values
The Contractor shall establish the activity codes identified in this specification. The codes will have values assigned that will allow the scheduling program to sort, select, group and organize the activities in the schedule. Activity codes include, but are not limited to, the following codes:
a. Area Code: All activities shall be identified in the project schedule by the Area Code in which the activity occurs.
Activities shall not be contained in more than one Area Code.
Area is defined as distinct separations in construction, such as a story of construction, separate structure, usage or function difference, utility distribution systems, etc.
b. Responsibility Code: All activities in the project schedule shall be identified with the party responsible to perform the task.
Responsibility includes, but is not limited to; the Prime Contractor, subcontracting firm, or Government agency performing a given task. Activities shall not belong to more than one responsible party.
