SOLICITATION
I. GENERAL INFORMATION
The Puerto Rico Army National Guard has a requirement to provide Medical and Dental Non-clinical
Case Management and administrative Care Coordinator staffing in support of Individual Medical
Readiness (IMR) for the Puerto Rico Army National Guard as described in the Performance Work
Statement (PWS)(Attachment 1). The case management services shall maintain communication as required with the Service Member; all shall be documented in the Medical Readiness Portal and
MEDCHARDT Applications to include Medical Operational Data System (MODS). The Contractor shall serve as liaison between the chain of command and the chain of concern, providing assistance to Soldiers with all aspects of medical readiness and mobilization to include Periodic Health
Assessments and individual Soldiers readiness to determine deployability status of the Individual
Soldier. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only notice. A written solicitation will not be issued. This requirement is issued as a request for quote (RFQ). The solicitation documents and incorporated provisions and clauses are those in effect in the Federal Acquisition Circular 2005-95.
A Firm Fixed Priced Award will be made in accordance with the following:
NAICS Code: This acquisition is being solicited as a 100% set aside for small business -
Competitive (8a). The North American Industrial Classification System (NAICS) code for this acquisition is 541611 with a size standard of $16.5 million dollars.
II. DESCRIPTION OF ACQUISITION:
The Puerto Rico ARNG has a requirement to purchase the below listed services. This procurement will be awarded under Simplified Acquisition Procedures. Quotes must be received no later than
20 July 2020, 1pm, EST, daylight savings time, Email quotes to SSG Maria del Mar Ruiz at maria.d.ruizperez.mil@mail.mil or to Capt Rolando Perez at rolando.perez20.mil@mail.mil. If there are any questions, please email SSG Maria del Mar Ruiz
All questions and requests for information (RFI) must be received not later than 29 June 2020 at
3:00pm EST.
III. SCHEDULE OF SUPPLIES/SERVICES:
Period of Performance (PoP): One year after contract award.
All work is to be performed in accordance with (IAW) the attached Performance Work
Statement. The contractor shall provide price for all services listed in the PWS attached.
IV. QUOTE PREPARATION INSTRUCTIONS
To ensure timely and equitable evaluation of quotes, offerors must comply with instructions contained herein. Quotes must be complete, self-sufficient, and respond directly to the requirements of this RFQ.
a. The offeror must submit a signed and dated quote on company letterhead with unit prices and extended prices in response to this solicitation. In doing so and without taking any exception to the requirements of this synopsis/solicitation, including specification, "Schedule of Supplies/Services" and terms and conditions, the offeror agrees to accede to all solicitation requirements. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale.
b. Offerors must provide a unit price and extended price for CLIN 0001 referenced in the
"SCHEDULE OF SUPPLIES/SERVICES."
c. Interested parties capable of providing the requirement must submit a written quote to include discount terms, tax identification number, cage code, and DUNS.
d. To be eligible to receive an award resulting from this RFQ, contractors must be registered in the
System for Award Management (SAM) database, no exceptions. To register, please visit http://www.sam.gov or by calling 1-866-606-8220.
e. Quote Contents:
PART I – TECHNICAL:
Provide quote that complies to all the requirements of the synopsis/solicitation.
PART II – PRICE:
The price factor will be evaluated for reasonableness.
IMPORTANT NOTE: Multiple Offerors nor Multiple Awards will be considered. A single award will be made in the resultant contract.
V. BASIS FOR AWARD
This is a 100% Competitive 8(a) Small Business Set-Aside firm-fixed price contract. The small business contractor must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Award will be made to the lowest priced quote that meets all required services technical capabilities/requirements.
PART I – TECHNICAL:
The offeror by acceding to all of the requirements of this synopsis/solicitation (see Quotation
Preparation Instructions) will be determined technically acceptable.
PART II– PRICE:
The Government will evaluate the price of all quotes that meet all the items technical capabilities as follows:
Quotes will be determined fair and reasonable based on competitive quotes, comparison with similar services in a related industry, comparison to an independent Government estimate, or any other reasonable basis in accordance with FAR 13.106-2.
The Government intends to award a contract without discussions with respective offerors. Offerors shall submit sufficient information and in the format specified in Quote Preparation Instructions.
Offeror’s may be asked to clarify certain aspects of their quote to which the offeror has not previously had an opportunity to respond.

VII. CONTRACT TERMS AND CONDITIONS:
The following provisions and clauses apply to this acquisition and can be viewed through Internet access at the Air Force Federal Acquisition Regulation (FAR) Site, http://www.farsite.hill.af.mil in accordance with 52.252-1 and 52.252-2.
This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available, IAW FAR 52.107 or view in the sites mentioned in provision 52.252-1 and Clause
52.252-2.
