About this file
This document is a Past Performance Questionnaire (PPQ) issued by the Iowa National Guard's U.S. Property & Fiscal Officer (USPFO) for a solicitation (W912LP-26-R-0001) seeking a contractor to provide breakfast, lunch, and dinner meals for the 185th RTI Schoolhouse from 15 October 2025 to 4 August 2026. The PPQ is a five-page evaluation form designed to collect detailed performance feedback from previous clients, with sections covering customer service, meal quality, schedule adherence, menu compliance, and overall customer satisfaction.
The questionnaire requires respondents to rate the offeror's performance across multiple dimensions using a confidence scale ranging from "Substantial Confidence" to "No Confidence," with mandatory written rationale for each rating. Completed questionnaires must be submitted to Hunter Maeder (hunter.d.maeder.mil@army.mil) and Kelsey Letcher (kelsey.l.letcher.civ@army.mil) no later than 23 September 2025. The evaluation is specific to the North American Industry Classification System (NAICS) Code 722310, which relates to food service contractors, and will be used to assess the potential contractor's past performance for the upcoming meal service contract.
