2RFP p. 51, Sec. 4.3: This section states, “This volume requires at least three references but shall not exceed five references and 15 pages in total, excluding CPARS reports.” Later, the same paragraph states, “The prime offeror shall submit at least two but no more than three past performance references.”The latter reference has been removed from the RFP Sec . 4.3 to avoid the conflict
4RFP p. 51, Sec. 4.2: Please verify that Section 4.2 is duplicated in error.Duplicate entry has been removed
5RFP p. 51, Sec. 4.1: “Key Point #1 (Identify Licensure/certification.” Please specify what information is being requested here. Does the Government require copies of licenses/certifications for proposed personnel? If so, are resumes for proposed personnel also required? This is also referenced on p. 50The government does not require resumes from contractor personnel. Key Point 1 requires acknowledgement from the offeror that their proposed personnel are licensed or will be licensed at time of performance.
6In section 3.5.1 the Government states “Three copies of the entire and complete proposal, composed of Volumes I through III, shall be delivered in person to Purchasing and Contracting, 106520 Mather Blvd, Sacramento, CA 95655…” Can the Government please clarify what they mean by “in person”? Will the Government accept hard copies of the proposal to be that were received via FedEx, USPS, or UPS submissions?"in person" includes via courier, company representative, USPS, Fed-Ex, etc. Some maps list 95655 as "Mather" instead of "Sacramento"; either are acceptable if received.

3. Past Performance Questionnaires are not required and will not be evaluated. 52.212-1 Addendum is amended.

4. 12190 Medical Records Technician (Care Coordinator) x 15 on contract. RN ll 12312 (Case Manager) x 5 on contract. (The number of FTE is determined by the contractor.)

5. We do not track tenure of contracted employees

6. The current contract is operating under SCA+C2 10 1. Do the medical technician and case manager travel together?

2. When traveling, do they have access to a government credit card? Or do they get reimbursed after providing receipts?

3. Is there Per Diem rate automatically affiliated with the travel by the government.

4. How many days at one time can they expect to be on travel?

5. How often is travel involved? 1. They will travel together or seaparate as allowed per JTR.

2. They do not have a Government Travel Card. They will be reimbursed as allowable per the JTR.

3. The JTR and PWS section 1.4.12 addresses per diem

4. Normally 3-4 days but SRPs could last 6-8 days

5. PWS section 1.4.4 addresses this

111. Do we need to provide coverage for the entire 0700-1700 or 8 hours/day within the 0700-1700. Are the weekend work hours the same as the M-F?1. 8 hrs/day within the 0700-1700 window. Weekend hours depend on the start time of the event but will not exceed 8 hrs.
12In Section 3.5.2 refers to government “confirmation of receipt of the proposal from GSA e-buy”. There was no mention of this solicitation being bid under a particular GSA schedule. This section also contradicts Section 3.5.1 instructions to deliver our proposal “in person”.Section 3.5.2 has been amended to substitute AMRDEC for GSA. Note that both electronic AND hardcopy are required.
13Please verify if the requirement for Medical Liability Insurance is for Professional Liability Insurance or for Medical Malpractice Insurance.This questions is pending answer
14Relative to the SCA, how many years have each of the incumbent staff been there, regardless of the management firm? SCA requires increased benefits for longevity, beginning after one year of service. Offerors must include those Costs in Fringe Benefits for incumbent staff for the life of the contract. Please indicate the current longevity of each incumbent staff person.The tenure of each current employee is not available.
151.4.12 Travel. May offerors add Handling Fee to Travel Charges? If yes, please create a CLIN for this Fee.Travel-related fees will be reimbursed as allowed per JTR and invoiced against the travel CLIN. Non-covered costs should be folded into your overhead pricing.
161.4.15 Phase In. May the contractor bill for services provided during the

Phase In? For instance, both the Contract Manager and the Key Person may be

working during this time.The Phase-In period has been removed to accommodate the extended solicitation close date. The Period of Performance will begin sometime between Sep 19th and Sep 30th.
17Pg 51 SUB-FACTOR 3: Key Personnel (Case Managers are identified as Key

Personnel). Also Q&A answer #9 indicating there are five Case Managers on

staff currently, but quantity to be determined by Contractor. What is the minimum number of Case Managers to be proposed as Key Personnel?The contractor may take any action to ensure performance of services as defined in the PWS, including making adjustments to local staffing levels, as this is a non-personal service contract.
18Pg 81 requires the QCP 30-days post award; pg 56 at 1.4.1, requires QCP 10-

days post-award; pg 49 at 4.1 and pg 48 at 3.3 requires QCP as part of proposal

and is evaluated as Key Point #3. Please clarify.An acceptable Quality Control Plan is an evaluating factor due at time of proposal. It remains a deliverable within 10 days of award. Page 81 of solicitation is amended.
19Pg 57 at 1.4.4 Place. Please clarify whether these Readiness Services are to be

provided a) Only at the intermittent events, or b) Are to be provided on a regular basis at CA ARNG HQ – with a 40hr work week - and also at the intermittent

events.PWS section 1.4.4 specifies: "MRE’s and Drill weekends are typically held on a monthly basis, and could result up to 15-18 weekends per year. Contractors may flex the work week before or after the weekend that was worked to remain in a 40 hour work week".
20Q&A #9. The answer lists quantities of personnel on staff today.

a. Is this solicitation for full time Case Management services (40 hours per week) or is it for only 18 weekends per month?

b. To assist offerors in building the staff model, please provide the Manpower reports from the past two years.

21Price Volume. Does the government want pricing for 1920hrs annual, or 2080hrs,
or other?PWS section 1.4.3 and 1.4.4 outline the hours of operation. How the contractor staffs this requirement is their decision.
22In the Q&A yesterday it is mentioned that the incumbent is Compass Medical Group (61UK6), W912LA-19-P-0020; However, with the original Sources Sought that was released (#W912LA-18-R-0001 on 2/9/18), Lifehealth LLC (#W912LA17P0061) is listed as the incumbent. Was the Lifehealth LLC the incumbent to the Compass Medical contract? Is there a J/A or any reason why the Compass Medical Group (61UK6), W912LA-19-P-0020 contract was sole source?Lifehealth performed services from 27 Sep 2017 until 21 Aug 2018 through contract W912LA-17-P-0061. Compass Medical performed services from 14 Jan 2019 to 13 Sep 2019 under contract W912LA-19-P-0020. The sources sought W912LA-18-R-0001 was for a different period than that covered by W912LA-19-R-0009. W912LA-19-P-0020 was an approved 8(a) Sole Source.
231. If we have an approved 8a JV, can we use Past Performance from either JV partner to meet the minimum 3 required references?

2. If we propose as a 8a Prime and non 8a Sub, can we provide all 3 minimum required references from the SUB only or does the Prime still need to provide at least 1 reference?

3. Section 3.3 indicates the Technical Factor is allowed 25 pages not including Resumes. Can you please share what resumes are required?

4. Is there any travel anticipated outside of Sacramento? If yes, will we be able to provide daily Per Diem and lodging?

5. Section 4.1 SUB FACTOR 2 – Key Point #2 On site Staff Hierarchy– How is this item different than SUB FACTOR 3 Key Personnel Key Point #3: Site Specific team Structure? We feel both are almost the same thing – requesting information about the site team composition and managerial structure. If so, may we include the exact same data in both areas?

6. For SUB FACTOR 3 Key Point#2 – Company Structure – What exactly are you needing in this section? What are you looking for? If our company is a corporation or LLC, etc? or are you looking for other information? Please clarify.

1. Per section 4.2 of Addendum 52.212-2 "...the prime Offeror can submit up to two (2) past performance references for each major subcontractor for recent performance."

2. Refer to section 4.2 of Addendum 52.212-2 and clause 52.219-14 to see if this is feasible.

3. Resumes are not required per this amendment.

4. Travel will be reimbursed as allowed per JTR. MREs commonly occur outside a 50-mi radius of Sacramento.

5. Sub-Factor 3, Key Point #2 has been removed with this amendment

6. Sub-Factor 3, Key Point #3 has been removed with this amendment

24Since PPQs are not being accepted, what recourse is being afforded those companies without CPARS records? Are there other forms of past performance documentation that will be accepted for this RFP?
Vendors without relevant Past Performance experience will be treated as "neutral" and allowed to proceed. Those with negative ratings will be deemed "Unacceptable".
