DESCRIPTION OF SERVICES
This statement of work describes contractor requirements for the project titled “Building 135 – WING HEADQUARTERS – Replace Flooring”. All work will occur at the Fresno Air National Guard Station, Fresno, California. The contractor is responsible to furnish and install all labor and materials to complete the items listed in this Statement of Work. The contractor shall provide the government a complete and usable product that will be inspected/accepted by the government at the completion of the project. All information included in this document, along with the instructions and standards referenced in the document, should be considered as part of the Statement of Work.
SITE OF WORK
All work will occur at the Fresno Air National Guard Base 5323 McKinley Ave, Fresno, CA 93727
STATEMENT OF WORK
Objectives: The purpose of this project is to remove the existing carpet and tile in the Wing building and install with new carpet and tile.
| Directions: |
| Complete all work according to the manufacturer’s specifications. |
| Contractor shall be approved by the manufacturer to install any of their manufactured products. |
| Perform necessary demolition. Once the old flooring has been removed, the contractor will ensure the surface is clean of any old adhesives, glue and any other debris before new flooring is installed. |
| Install approximately 11,000ft2 of new 24” X 24” Tandus INFINITY #05849 (or approved equal) carpet tile as indicated in the drawings. Available color list or color samples to be submitted for approval. Approved equal must be 24” x 24” carpet tile. Approved equal must meet or exceed attachment, Tandus Carpet Specifications, in terms of warranty, stain protection, face weight, stiches per inch, and gauge. |
| Install approximately 5,000-6,000ft2 of new 12” X 24” Centiva vinyl tile, Event Series (or approved equal) as indicated in the drawings. Available color list or color samples to be submitted for approved. Approved equal must be 12” x 24” carpet tile. Approved equal must meet or exceed attachment, Tandus Centiva Spec, in terms of warranty, classification, total thickness, and wear layer thickness. |
| Install approximately 4500lf of new 6” vinyl cove base of all walls where new flooring was installed. Color samples to be submitted for approval. |
| All existing bases where work is completed should be converted to cove base. The contractor is instructed to cut cove base or use a shorter cove (of the same color and style) in areas where furniture restricts a 6” height. Wall brackets that support furniture may be notched to allow the 6” base. Notching must be square, minimized and look finished in areas around brackets if utilized. |
| Install approximately 300lf of transitions where the carpet tiles and vinyl tile meet. |
| The bathroom on the East side of Bldg. 135, Room 1 will remain the same ceramic tile, however the contractor will cut the tile and install a transition to the new vinyl tile in the lobby, Room 2. |
| Demolition |
| Contractor must remove all materials completely off 144th FW property. |
| Remove all carpet and glue as indicated in the drawings. |
| Remove all vinyl composite tile and adhesive as indicated in the drawings. |
Remove all tile and adhesive as indicated in the drawings.
Remove all of the carpet base board in all rooms and hallways indicated in the drawings.
| Finishing |
| All flooring, transitions, and adhesives will be installed to the manufacturer’s specifications. |
Any damage to the wall paper, walls, doors, glass, furniture, and wall mounted electronics will be repaired or replaced by the contractor.
| If applicable, seal and wax the vinyl tile floor per the manufacture specs. |
| Furniture Moving |
| Furniture moving will be a separate line Item on the contractor’s estimate. The Government will choose to execute or not execute line item 2 after receipt of offers. |
| The Government will be responsible to remove all non-mounted electronics. All mounted wall TVs and electronics, outside of the workable surface will remain in place. |
| The government will be responsible to remove all files, documents, and items out of furniture prior to the arrangement of furniture moving. |
| If desired, a storage conex or container may be brought on base and stored in an allowed parking area. The conex will be the responsibility and at the cost of the contractor. |
| Furthermore, if damage occurs to structures, buildings, or asphalt during the transport of a storage container the contractor will be responsible for repairs. |
CONTRACTOR LICENSE REQUIREMENTS
The contractor will possess a valid California “C‐15” license and be able to provide proof of competence in the area of work to self‐perform sections of the project. However, a California “B” license is acceptable if work is sub‐contracted.
All listed sub‐contractors working for the contractors will possess the applicable California Contractors license (C‐15) license. Refer to the Project Specific Requirements section for the required license from the Contractor.
PROJECT SPECIFIC REQUIREMENTS
The project will be completed no later than 110 working days post Notice to Proceed (NTP). Cabinets and fixtures will remain in place during the project. The contractor and government will negotiate the final phasing (if applicable), and schedule during a review of the project at the pre‐construction conference.
| Project is to be phased in 3 sections. Phases should take approximately 30 days each. |
| Materials. The following lists out materials requirements for this project. Contractor may request substitutes for an approved equal during the pre‐construction conference. All substitution requests shall be made in writing on the AF Form 3000. Requests for material substitutions made after the pre‐construction conference may not be granted by the Government. |

GENERAL REQUIREMENTS
When required, the Contractor will attend a Pre‐Construction Conference. The Pre‐ Construction Conference will generally be scheduled after award but prior to the NTP. At a minimum, the contractor shall have their project manager and site superintendent attend the meeting. The Contractor will provide the following documents at the Pre‐ Construction Conference Project Schedule Contract Schedule of Values Safety Plan

Material Safety Data Sheets Draft DD 1354 – Final draft to be submitted at conclusion of project
| Government Furnished Property and Services. The government will provide water, sewer and electrical connections. Secured storage is the responsibility of the contractor. |
| Quality Control. Contractor shall develop and maintain a quality program to ensure work is performed as described in this Statement of Work and in accordance with all local, state and federal rules. |
Quality Assurance. The Government will maintain a Quality Assurance Program to ensure work is performed as described in this Statement of Work and in accordance with all local, state, and federal rules. The Project Manager will manage the Government’s QA program.
| Hours of Operation. The hours of work shall be between 7:00 a.m. and 5:00 p.m. Monday through Friday unless otherwise prescribed by this statement of work. NOTE: Select Mondays are scheduled as off‐days throughout the year. This schedule can be made available to the contractor after award. Work shall not be performed on Weekends and Federal Holidays without prior coordination from the Project Manager. |
| Base Access. Base security personnel require that all personnel entering the installation are cleared for access. The contractor will provide first name, last name, driver’s license number and applicable vehicle data for all personnel accessing the base. The contractor shall submit this information to the base Contracting Office NLT two working days prior to the date of required access. Personnel must maintain a valid driver’s license, vehicle registration and proof of insurance at all time while on the base. |
| The Contractor, and all listed sub‐contractors, must be licensed within the State of California. Refer to the Project Specific Requirements section for the required license from the Contractor. |
| Technical Competence and Ability. If requested by the Government, the Contractor shall provide proof of technical competence and experience in the area of expertise required by this Statement of Work. The contractor shall be prepared to submit documents providing proof of successful competence such as training certificate or accreditations from recognized industry leaders. Contractors shall also be prepared to provide proof of successful project experience through references of former clients in related project types. |
| Dimensions and Quantity. All dimensions and quantities provided in the Statement of Work are estimates only. They are provided as a courtesy for contractors to estimate the magnitude of work or materials necessary to complete a task. They are not intended to be final measurements. The Contractor is responsible for verifying all dimensions and quantities in the field prior to starting work. |
Submittals. All submittals will be transmitted with an AF Form 3000. When submitting on a finish, texture or other material characteristic where a photograph is unacceptable, the Contractor will provide a sample of the material for review and approval along with the product cut‐sheet.
Warranty. The contractor shall guarantee all workmanship and materials covered under this statement of work for a period of no less than one (1) year from the date of project acceptance. This contractor’s warranty shall be included with any manufacturer’s warranties that accompany materials or systems installed as part of this statement of work.
SUBMITTALS
All submittals will be transmitted with an AF Form 3000. When submitting on a finish, texture or other material characteristic where a photograph is unacceptable, the Contractor will provide a sample of the material for review and approval along with the product cut‐sheet.
CLOSEOUT DOCUMENTATION
10% Retainage will be released upon receipt of the following items:
| Contractor workmanship letter of warranty up to one year. |
| Certificates of no debts to suppliers and vendors. |
| Electronic pictures showing the area before and after repair. |
| Recycling or Landfill proof of disposition document (if available). |
All applicable O&M manuals
"Contractor Manpower Reporting The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force collection site. The Contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/.
Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data should be reported no later than October 31 of each calendar year.
