QUALITY ASSURANCE

SURVEILLANCE PLAN (QASP)

FOR

Oil/Water Separator Annual Maintenance/Repair Project # 201504xxxx

FY15

23 April 2015

1. PURPOSE. This Quality Assurance Surveillance Plan (QASP) Contractor performs all requirements of the Contract, that the Government receives quality services, and that the Government pays only for services actually provided. The QASP provides a systematic method to evaluate the services the contractor is required to furnish. This QASP is a living document and should be updating accordingly throughout the life of the contract.

2. SCOPE.

2.1. The Contractor is responsible for management and use of a quality control program (QCP) necessary to perform the required services. The Contractor is responsible for management and quality control actions to meet the terms of the contract. The Contractor develops a Quality Control Program (QCP) in compliance with his contract deliverables.

2.2. The role of the Government is to ensure contract standards are achieved. The QASP is put in place to provide Government surveillance oversight of the Contractor’s quality control efforts to assure they are timely, effective and are delivering the results specified in the contract. The QASP is not a part of the contract nor is it intended to substitute for or duplicate the Contractor’s QCP.

2.3. The Government may provide the Contractor an information copy of the QASP as an attachment to the solicitation to support the Contractor’s efforts in developing a QCP that will interrelate with the Government’s QASP.

3. ROLES & RESPONSIBILITIES. The Government resources shall have responsibilities for the implementation of this QASP as follows:

3.1. Contracting Officer. The Contracting Officer ensures performance of all necessary actions for effective contracting, ensures compliance with the terms of the contract, and safeguards the interests of the United States in the contractual relationship.

3.2. Contracting Officer Representative (COR). The COR is responsible for oversight and assures proper performance of the contract by the contractor. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. Any changes that the contractor deems may affect contract, price, terms, or conditions shall be referred to the Contracting Officer for action.

4. METHODS OF QA SURVEILLANCE.

4.1. The method of surveillance used for the administration of this QASP is 100 % surveillance, though some areas will not always be applicable and will be noted as such. The 100% evaluation method requires that a complete inspection of the quality of work is completed by the COR/inspector. 100% Inspections will involve work performed for the specific unit being surveillanced and as requiredbasis. 100% Inspection is used when the service is considered critical and/or the quantity of work is relatively small.

4.2. The methods of surveillance recognize that unforeseen and uncontrollable problems and issues do occur. The Contracting Officer’s Representatives (COR) are expected to be objective, fair, and consistent in evaluating Contractor performance against the standards.

5. IDENTIFIED COR SURVEILLANCE TASKS. The following performance requirements are identified within the Statement of Work and are to be monitored under this QASP.
