Micah Kruse Contract Execution Checklist 03 June 2015 Our organic unit members do the MEDPROS data entry, HRR, EKG and Immunizations.
No dental is included in this contract.
There is not dental solicitation being posted in FY18. Please watch FBO.gov for updates on separate requirements An Audiologist may work inside our facilities, but an individual booth will not be supplied. Based on locations and space shortages, we may be able to provide a small office, but that is not guaranteed.
Only Benson Audiometers are authorized. They are the only system that works with DOEHRs.
Yes- the ORARNG will provide laptops, and Military VPN to MODS-MHA module. Contractors must have current CACs to use laptops as a DoD visitor.
No prescriptions have been provided by medical providers in the past, and none are expected to be needed.
Column Q is for putting the total price (provider and audiology ) for executing the Exhibit 3 based on the proposed pricing for that year.
Fiscal Year 2018 Yes. Organic enlisted unit members and RNs will be working the stations.
Yes.
For the Audiology portion, all Laptops, Internet access, Power source (generators and related supplies) and Benson audiometers are contractor furnished.
There is not a separate CLIN just for audiology services. There is a separate pricing structure for audiology under Annex A which should encompass all audiology services required by the PWS.
Exhibit 3 is the governments best estimation of the workload. However, the minimum guarantee for the contract is $2,500.
If the offerer feels that a Project Lead is required, they should include the price of a Project Lead into the overall pricing schedule. Adding a separate Project Lead CLIN is not possible at this point.
