PAST PERFORMANCE INFORMATION SHEET
PROJECT NO. _____, Page 1 of up to 4 pages (project number is taken from column 1 on Attachment 2) Must have at least 2 projects, more than 5 will not be evaluated. The Offeror as the prime contractor must have performed at least 2 of the 5 projects submitted for experience and 2 projects must be DB.

1. Building renovations that include floors, doors, walls, windows, external facades, repairs to roofs;
1. Repair/replacement of major building systems such as electrical, mechanical, HVAC;
1. Civil work, that includes concrete/asphalt roads and pavements, earthwork, and drainage structures.
1. Environmental work, such as hazardous material abatement and disposal.

Offeror: A contractor and/or company submitting a proposal for this solicitation.
Prime Contractor: A contractor that performed at least 15% of the work, and managed all other work for the project.
Sub Contractor: A contractor that performed part of the project work, but did not manage the entire project.
Joint Venture: More than one contractor with a legal binding agreement to work together.
Key Subcontractor: A contractor that has signed a letter of commitment to perform work for the duration of the contract for the Offeror, and the Offeror is allowed to take credit for the Key Subcontractor’s experience the project.

e. Ensure you provide a letter of commitment signed by both the Offeror and the TEAMING Subcontractor in order to obtain credit for the project work. Besides the signed letter, no additional information is required other than what is on Page 1 of this form.
