PAST PERFORMANCE INFORMATION SHEET
PROJECT NO. _____, Page 1 (project number is taken from column 1 on Attachment 2A) Must have a total of 5 projects, more than 5 will not be evaluated. The Offeror as the prime contractor must have performed at least 2 of the 5 projects submitted for experience.

1. Building renovations that include floors, doors, walls, windows, external facades, repairs to roofs;
1. Repair/replacement of major building systems such as electrical, mechanical, HVAC;
1. Civil work, that includes concrete/asphalt roads and pavements, earthwork, and drainage structures.

Offeror: A contractor and/or company submitting a proposal for this solicitation.
Prime Contractor: A contractor that performed at least 25% of the work, and managed all other work for the project.
Sub Contractor: A contractor that performed part of the project work, but did not manage the entire project.
Joint Venture: More than one contractor with a legal binding agreement to work together.
Key Subcontractor: A contractor that has signed a letter of commitment to perform work for the duration of the contract for the Offeror, and the Offeror is allowed to take credit for the Key Subcontractor’s experience the project.
