SUPPLEMENTARY REQUIREMENTS
PART 1 GENERAL
The work covered by this section of the specifications consists of work common to more than one section of these TECHNICAL SPECIFICATIONS.
1.1 SCOPE OF WORK
This work involves replacing and/or removing existing culverts and headwalls in the Jackson Hole levees as indicated, and reestablishing the levee profile. Headwalls will be replaced with precast headwall. Work will also include creating cofferdams during construction to isolate the work area, and concrete work to build new headwalls for culverts. The river that work will be performed along side of is considered class 1 and 2 waters. Fill and riprap to be government furnished. Contractor to provide the other earthen materials.

1.3 SUBMITTALS

1.4 PROJECT SCHEDULE
The Contractor shall furnish a schedule for the complete project in accordance with Clause 52.236-15 Schedules for Construction Contracts at the pre-work meeting.
1.5 POST AWARD MEETINGS
1.5.1 Pre-Work Meeting
The Contractor shall meet with the Walla Walla District USACE Representative at the Teton County Office Building located in Jackson, Wyoming prior to the beginning of work. The purpose of this meeting is to discuss and develop mutual understandings relative to the general and technical aspects of the work, safety, quality control program, scheduling of work, and environmental protection provisions of the contract.
1.5.2 Levee Closure Sign
Contractor must obtain levee closure signs from Teton County. Coordinate with Contracting Officer for signage.
1.5.3 Contractor's Schedule
Contractor shall furnish a schedule for the complete project detailing the

g4so9drn Highlight g4so9drn Line dates and time that the Contractor will be mobilizing into the area, working at each site, and demobilizing from the area. Submit the actual daily hours the Contractor intends to work. Work can take place between the hours of 0700 and 1800 hours, daily Monday through Saturday.
Exceptions must be discussed Contracting Officer. The Contractor shall furnish notification of any change of work schedule for each site, regular work hours, overtime work hours, and shifts of work crews and personnel at the site. This notification shall be provided a minimum of 48 hours prior to any schedule change to allow suitable scheduling of Government personnel and inspection. Exception to this requirement may be allowed by the Contracting Officer in case of schedule change due to emergency conditions.
1.6 SAFETY
1.6.1 Accident Prevention
In accordance with Contract Clause ACCIDENT PREVENTION the Contractor shall provide and maintain work environments and procedures that will safeguard the public, Government personnel, Contractor personnel, property, materials, supplies, and equipment exposed to Contractor operations and activities.
1.6.1.1 Safety Requirements
The Contractor shall comply with the requirements of EM 385-1-1 .
1.6.1.2 Corrective Action by the Contractor
Whenever the Contracting Officer detects any noncompliance with these requirements or any condition that poses a serious or imminent danger to the health or safety of any personnel, the Contracting Officer will notify the Contractor and request immediate initiation of corrective action. Lack of notice from the Contracting Officer does not relieve the Contractor from compliance requirements and responsibility. After receipt of notice from the Contracting Officer, the Contractor shall immediately take corrective action. Such notice, when delivered to the Contractor at the site of the work, shall be deemed sufficient for the purpose of notification. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or part of the work until satisfactory corrective action has been taken.
1.6.2 Site Safety and Health Officer (SSHO)
Provide an SSHO that meets the requirements of EM 385-1-1 Chapter 2. The SSHO must ensure that the requirements of 29 CFR 1926.16 are met for the project. Provide a Safety oversight team that includes a minimum of one
(1) person at each project site to function as the SSHO. The SSHO or an equally-qualified Alternate SSHO must be at the work site at all times, with the exception of temporary absences less than 24 hours discussed below, to implement and administer the Contractor's safety program and government-accepted Accident Prevention Plan. The SSHO and Alternate SSHO must have the required training, experience, and qualifications in accordance with EM 385-1-1 Chapter 2-3.b, and all associated sub-paragraphs including the following.
The SSHO is required to be employed by the Prime Contractor. The SSHO can be assi gned as t he Pr oj ect Super i nt endent on t hi s pr oj ect . The SSHO must also meet the following requirements:
a. A minimum of 3 years safety work on similar projects.
b. 30-hour OSHA construction safety class or equivalent within the last 5 years.
c. Provide documentation that shows the completion of a minimum of 8 hours of formal, on-line, or self-study safety and health related coursework each year during the last five (5) years.
d. See USACE EM 385-1-1 Chapter 2 for additional qualification requirements.

If the SSHO is off-site for a period longer than 24 hours, an equally-qualified alternate SSHO must be provided and must fulfill the same roles and responsibilities as the primary SSHO. When the SSHO is temporarily (up to 24 hours) off-site, a Designated Representative (DR), as identified in the AHA may be used in lieu of an Alternate SSHO, and must be on the project site at all times when work is being performed.
Note that the DR is a collateral duty safety position, with safety duties in addition to their full time occupation.
The SSHO must comply with EM 385-1-1 Chapter 2-4.b:
1.6.3 Subcontractors
Compliance with the accident prevention and safety requirements by subcontractors will be the responsibility of the Contractor.
1.6.4 Safety Meetings
Shall be conducted and documented as required by EM 385-1-1 . Minutes showing contract title, signatures of attendees and a list of topics discussed shall be attached to the Contractors' daily quality control report. The meeting shall be attended by the following personnel:
(1) Contractor project superintendents
(2) Supervisors and prime contractor employees
(3) Quality control personnel
(4) Safety officer(s)
(5) Subcontractors employees
(6) Contracting officer's QA representatives/safety.
1.6.5 ACCIDENT PREVENTION PLAN (APP)
Prepare the APP in accordance with the format and requirements of EM 385-1-1 A qualified person must prepare the written, site-specific APP on ENG Form 6293, Accident Prevention Plan (APP) Worksheet. The following conditions apply:
1. The Government considers the Prime Contractor to be the "controlling authority" for all work site safety and health of the subcontractors.
2. Contractors are responsible for informing their subcontractors of the safety provisions under the terms of the contract and the penalties for noncompliance, coordinating the work to prevent one craft from interfering with or creating hazardous working conditions for other crafts, and inspecting subcontractor operations to ensure that accident prevention responsibilities are being carried out.
3. If English is not the foreman's primary language, the Prime Contractor must provide an interpreter .
Submit the APP to the contracting Officer Fi f t een ( 15) cal endar days prior to the date of the preconstruction conference for acceptance. Work cannot precede without an accepted APP. Once reviewed and accepted by the contracting Officer, the APP and attachments will be enforced as part of the contract.
1. Disregarding the provisions of this contract or the accepted APP is cause for stopping of work, at the discretion of the Contracting Officer, until the matter has been rectified.
2. Continuously review and amend the APP, as necessary, throughout the life of the contract. Changes to the accepted APP must be made with the knowledge and concurrence of the Contracting Officer, project superintendent, SSHO and Quality Control Manager.
3. Incorporate unusual or high-hazard activities not identified in the original APP as they are discovered. Should any severe hazard exposure ( i.e. imminent danger) become evident, stop work in the area, secure the area, and develop a plan to remove the exposure and control the hazard. Notify the Contracting Officer within 24 hours of discovery. Eliminate and remove the hazard.
4. In the interim, take all necessary action to restore and maintain safe working conditions in order to safeguard onsite personnel, visitors, the public (as defined by ASSP A10.34 ), and the environment.
1.6.6 ACTIVITY HAZARD ANALYSIS (AHA)
AHA requirements are in EM 385-1-1 Chapter 2-6.
Before beginning each activity, task or Definable Feature of Work (DFOW) involving a type of work presenting hazards not experienced in previous project operations, or where a new work crew or subcontractor is to perform the work, the Contractor(s) performing that work activity must prepare an AHA. AHA's must be developed by the Prime Contractor, subcontractor, or supplier performing the work, and provided for Prime Contractor review and approval before submitting to the Contracting Officer. AHA's must be signed by the SSHO, Superintendent, QC Manager and the subcontractor Foreman performing the work. Submit the AHA for review at least five (5)calendar days prior to the start of each activity task.
The Government reserves the right to require the Contractor to revise and resubmit the AHA if it fails to effectively identify the work sequences, specific anticipated hazards, site conditions, equipment, materials, personnel and the control measures to be implemented.
Develop the activity hazard analyses using the Project Schedule as the basis for the activities performed. Any activities listed on the Project Schedule requires an AHA to be completed. Use ENG Form 6206, Activity Hazard Analysis (AHA) to complete an AHA. The form can be viewed, or

AHA's must identify competent persons required for phases involving high risk activities, including confined entry, crane and rigging, excavations, trenching, electrical work, fall protection, and scaffolding.
1.6.6.1 AHA Signature Log
Each employee performing work as part of an activity, task or DFOW must review the AHA for that work and sign a signature log specifically maintained for that AHA prior to starting work on that activity. The SSHO must maintain a signature log on site for every AHA. Provide employees whose primary language is other than English, with an interpreter to ensure a clear understanding of the AHA and its contents.
1.6.7 Sanitation Facilities
The Contractor shall furnish sanitation facilities in accordance with EM 385-1-1 . The location of all facilities will be subject to Contracting Officer approval. All facilities shall be staked to the ground to prevent tip over by winds or placed on a trailer and fastened in such a way as to prevent tip overs. The Contractor shall provide freeze protection during the months of October through March to prevent any spillage.
1.6.8 Reports
1.6.8.1 Accident Reports
a. For recordable injuries and illnesses, and property damage accidents resulting in at least $2,000 in damages shall notify the Contracting Officer within four (4) hours of the accident. The Prime Contractor shall conduct an accident investigation to establish the root cause(s) of the accident, complete the USACE Accident Report Form 3394 and provide the report to the Contracting Officer within five (5) calendar day(s) of the accident. The Contracting Officer will provide copies of any required or special forms.
b. For any weight handling equipment accident (including rigging gear accidents) the Prime Contractor shall conduct an accident investigation to establish the root cause(s) of the accident, complete the WHE Accident Report (Crane and Rigging Gear) form and provide the report to the Contracting Officer within 30 calendar days of the accident.
Crane operations shall not proceed until cause is determined and corrective actions have been implemented. The Contracting Officer will provide a blank copy of the accident report form.
1.6.8.2 Monthly Exposure Reports
Monthly exposure reporting to the Contracting Officer is required to be attached to the monthly billing request. This report is a compilation of employee-hours worked each month for all site workers, both prime and subcontractor.
1.7 FIELD VERIFICATION OF SITE CONDITIONS
The Contractor shall field verify all elevations, dimensions, and

g4so9drn Highlight g4so9drn Highlight g4so9drn Line coordinates of existing features indicated on the contract drawings that affect new construction. Any deviations shall be updated as per Section 01 33 00 , SUBMITTAL PROCEDURES, paragraph MAINTAIN WORKING AS-BUILT
CONTRACT DRAWINGS AND FIO DRAWINGS.
1.8 REGULAR CLEANUP AND DEBRIS DISPOSAL
With the exception of materials specifically indicated or specified to be salvaged for reuse in construction, or turned over to the Government, all wastes and demolished materials shall become the property of the Contractor and shall be removed from the job site daily in accordance with Federal, State, and local regulations. The Contractor shall furnish waste containers, all waste containers provided by the contractor shall be emptied daily.
1.9 EQUIPMENT LIST
The Contractor shall furnish a complete list of all equipment to be used on the project. The Contractor shall submit a revised list in the event of change of equipment. Lists shall include rented equipment as well as lease-purchase or sale-leaseback equipment. The initial list and the revised lists shall indicate dates equipment is assigned to or removed from the project and adequate identification or description of each item of equipment.
1.10 TESTING OF EQUIPMENT
Before any machinery or mechanized equipment is put to use on the job, it shall be inspected and tested by a qualified person and determined to be in safe operating condition. Cranes or derricks shall be tested by the Contractor or a qualified testing agency in accordance with EM 385-1-1 .
Equipment shall be large enough to safely handle proposed picks or tasks without exceeding the crane rating established by these tests.
1.11 DISPOSITION OF REMOVED EQUIPMENT
Unless otherwise specified in other sections of the specifications, all existing equipment, such as culvert lift slide gates and lift slide gate frames, removed and not reinstalled shall become the property of the Contractor, removed from the Project site, and disposed of in a legal manner. Materials that cannot be removed daily may be temporarily stored on-site at an approved area. Salvaged materials shall not be sold on the project site.
1.12 PROTECTION AND RESTORATION OF EXISTING FACILITIES
The Contractor shall take precautions to prevent damage to existing facilities while performing work under this contract. Upon completion of the work, all the existing facilities not included as a portion of the work shall be left in a condition better than or equal to the condition existing at time of contract award. Costs for maintenance, repair and restoration of any facilities shall be considered as incidental to and included in The Schedule prices.
1.13 PRE-WORK AND POST WORK CONDITIONS INSPECTIONS
The Contractor shall conduct a pre-work conditions inspection and a post-work conditions inspection, with the Contracting Officer Representative, of each work site. The results of the inspection shall be

g4so9drn Highlight g4so9drn Highlight g4so9drn Line g4so9drn Highlight g4so9drn Highlight documented by digital photographs and descriptive narrative reports. A color copy of the inspectionphotographs and narrative reports shall be submitted to the Contracting Officer.
1.14 FIRE CONTROL
Each piece of internal combustion engine driven equipment used at the work site shall be equipped with a fire extinguisher in accordance with recommendation NFPA as appropriate. The minimum approved rating of extinguishers shall not be less than 5-B:C.
1.15 SUPERINTENDENT
Submit a resume for Contractor's designated on site project superintendent. This individual will be responsible for quality control and ensuring compliance with all environmental regulations.
1.15.1 Employees
A list of names of all employees and authorized site supervisor who will be working for the Contractor, including subcontractors and their employees shall be provided as specified in Section 01 14 00.90 28
INTEGRATING ANTITERRORISM (AT) AND OPERATIONS SECURITY (OPSEC).
1.16 SUPERINTENDENT SUBSTITUTION
If the Contractor plans on replacing any Key personnel (Superintendent, Quality Control etc.) they must have the approval of the Contracting Officer. Submit qualifications of new personnel at least 15 calendar days in advance of date of replacement.
1.17 GOVERNMENT QUALITY ASSURANCE (QASP)
1. The Government will monitor the Contractor's performance under this contract using quality assurance procedures developed by the Government. Typical procedures might include random sampling, checklists, customer complaints, and observations.
2. The Government reserves the right to inspect and test all work called for by the contract, to the extent practicable at all times and places during the term of the contract. The Government will perform inspections and tests in a manner that will not unduly delay the work.
3. If any of the work do not conform to contract requirements, the Government will request the Contractor to perform the work again, where appropriate, in conformity with contract requirements, at no increase in contract amount. When the defects in work cannot be corrected by re-performance, the Government may (1) require the Contractor to take necessary action to ensure future performance conforms to contract requirements and (2) reduce the contract price to reflect the reduced value of the work performed.
4. If the Contractor fails to promptly re-perform the work or to take the necessary action to ensure future performance in conformity with contract requirements, the Government may by contract, or otherwise (1) perform the work and charge to the Contractor any cost incurred by the Government that is directly related to the performance of such work or (2) terminate the contract in whole or in part.
1.17.1 Contractor Daily Report
The Contractor shall prepare a daily report. The daily report shall also be forwarded by electronic means (e-mail) to the COR by the close of business the following work day. Fill out the daily report attached to the end of this section. The Contractor may use their own form as long as the required information below is included, submit a sample of the form to the on-site Government Representative for approval. The daily report shall also contain the following items:
a. Labor hours expended by each trade.
b. Equipment hours.
c. Quantity summary (daily and contract total) for unit price items. (Hard copies of weight tickets to be directly passed to on-site Government representative.)

PART 1 GENERAL
1.1 CONTRACTOR'S WORKING HOURS AT THE JOB SITES
Work at the job sites shall be accomplished between the hours of 7:00 a.m.
to 6:30 p.m. each workday unless otherwise specified herein, or authorized in advance by the Contracting Officer.
1.2 LEVEE ACCESS
The levees are located on private property. There is a speed limit of 15 mph while traveling on the Levee. Contractor vehicles shall follow Activity Hazard Analysis (AHA) speed limit, including speeds recommended by AHA when encountering pedestrians, on all levee's.
1.2.1 Vehicle Access
For work on the Public Levee, the Contractor shall post signs at the egress areas of this levee stating days the levee will be closed to vehicles. Notification signs shall be installed a minimum of 3 days prior to closure. Signage stating "Levee Closed" can be obtained from the Teton County Road and Levee Department.
1.2.2 Pedestrian Access
Due to public pedestrian access on the Walton Levee, the main access to the main stockpile site, immediately adjacent to Highway 22 right after Emily Stevens Pond Park, the Contractor is responsible to maintain a safe speed to ensure public safety while in proximity to pedestrians. The first 3 miles of Public Levee is also heavily used by the public.
1.2.3 Road and Bridge Restrictions
There are road and bridge restrictions in the area. The Contractor is responsible for following the local road and bridge restrictions in the Project area. Contact the Teton County Roads and Levees Department for current information.
1.3 GATE SECURITY AT STOCKPILES AND LEVEE REPAIR LOCATIONS
Each work site is protected by one or more gates. To prevent cattle and other livestock from escaping, unless otherwise instructed the Contractor shall immediately close gates after passing though them.
1.4 STOCKPILE LOCATIONS FOR RIPRAP AND FILL
The Contracting Officer will designate areas within the stock pile site from which to draw riprap and fill for levee reconstruction.
1.4.1 Main Stockpile Site
The main stockpile site is located approximately five four miles northwest of the city of Jackson, Teton County, Wyoming, in the NE 1/4 of Section 724, Township 41 North, Range 116117 West, Lat N43 3230' 0528", Long. W110 4849 ' 3950". Access to the main stockpile site is via Walton Levee immediately east of Snake River Bridge on Highway 22. Travel north on the levee road approximately 1 mile, turning east through a locked gate into the main stockpile site.
1.5 CONTRACTOR STORAGE
The Contractor may request to store equipment during the contract performance period at the main stockpile site. The Contractor may store equipment at individual levee work sites only during periods of work activity at that site. The Government will not be liable for the theft or destruction of any Contractor's tools or equipment.
1.6 OTHER CONTRACTS
The Government may undertake or award other contracts for additional work, and the Contractor shall fully cooperate with such other contractors and Government employees and carefully fit his own work to such additional work as may be directed by the Contracting Officer. The Contractor shall not commit or permit any act which will interfere with the performance of work by any other contractors or Government employees.
1.7 TRUCK SCALES
The contractor may use the truck scales located in the main stockpile site for obtaining weight of riprap and fill material loads for use in this contract. The government will validate the accuracy of the scales. Keys for accessing the scale house and ticket printing system may be obtained from the Teton County Roads and Levees Superintendent. The contractor will be required to furnish a portable generator to power the scales for use in this contract.

MEASUREMENT AND PAYMENT
PART 1 GENERAL
In each instance, the contract price for an item shall constitute full compensation for furnishing all plant, labor, equipment, and materials, and for performing all operations required to complete the work included in the item as herein specified, or as otherwise approved.
1.1 SUBMITTALS
Government approval is required for submittals with a "G" designation;

Retainage in the amount of 10 percent of original contract award amount will be withheld until the final submittals, listed below, have been approved by the Government. The 10 percent retainage for final submittals will start once the Contractor has earned 90 percent of the original contract value.
1.3 SITE WORK
Site work will be measured for payment as a complete pay item, (JOB).
Payment will be made at the job price for Item No's. 0001 through 0010 and 0013. Features included with Item No's. 0001 through 0010 and 0013 will include, but not be limited to:

Payment will be made at the unit price for CLIN 0011, "Load, transport, and place fill," which price and payment shall be full compensation for all work required for loading, transporting, and placing of the fill, complete, as specified and approved.

1.15 CLIN 0012 LOAD, TRANSPORT, AND PLACE RIPRAP
Load, transport, and place riprap will be measured for payment as the number of tons (TN) of riprap transported and placed. 1.16 CLIN 0013 STAGING TO THE WALTON STOCKPILE SITE
Measurement and payment for staging to the Walton Stockpile site will be measured for payment as a complete pay item, JOB. Payment will be made at the job price for Clin 0014, "Staging to the Walton Stockpile site", which price and payment shall be full compensation for required for performance of the work, as specified and approved.
1.17 MEASUREMENT
Transporting and placing of government furnished property (GFP), fill and riprap, will be measured for payment by the ton (2,000 pounds) by weighing each truckload to the nearest 0.1 ton, and the final quantity of the whole sum will be rounded to the nearest whole ton. The GFP will be measured for

payment by being weighed on approved state certified scales before being transported to the site and placed in the work. Contractor shall submit a copy of each load weight certificate to the Government Representative at time of placement. Weight certificate shall have the initials of the scale operator, date and time of weighing, truck number, name of truck operator. Each truck shall be weighed empty once every day and the empty weight ticket submitted. Submit a current copy of the Wyoming State Certification of Scales being used by the Contractor for weighing of GFP, unless the government scales located at the main stockpile site are to be used, in which case no certification is required. Submit certificate at the pre-work meeting. Scales shall be certified for the current year contract work is being performed.

SUBMITTAL PROCEDURES
PART 1 GENERAL
The Contracting Officer may request submittals in addition to those specified when deemed necessary to adequately describe the work covered in the respective sections. Make certain that each submittal is complete and in sufficient detail to easily determine contract compliance.
a. Use the same units of weights and measures as the contract drawings.
b. Contractor's Quality Control (CQC) System Manager shall check and approve all items prior to submittal and stamp, sign, and date indicating action taken.
c. Clearly identify any proposed variations from the contract requirements.
d. Include drawings; literature (catalog cuts, diagrams, operating charts or curves); test reports; samples; O&M manuals (including parts list); certifications; warranties; and other necessary information.
e. Obtain submittal approval prior to acquiring the covered material or equipment.
1.1 DEFINITIONS
1.1.1 Submittal Descriptions (SD)
Submittals requirements are specified in the technical sections.
Submittals are identified by Submittal Description (SD) numbers and titles as follows:
SD-01 Preconstruction Submittals
Submittals which are required to be approved prior to commencing work on site. Submittals required prior to the start of the next major phase of the construction on a multi-phase contract. Schedules or tabular list of data or tabular list including location, features, or other pertinent information regarding products, materials, equipment, or components to be used in the work.
SD-02 Shop Drawings
Drawings, diagrams and schedules specifically prepared to illustrate some portion of the work.
Diagrams and instructions from a manufacturer or fabricator for use in producing the product and as aids to the Contractor for integrating the product or system into the project.
Drawings prepared by or for the Contractor to show how multiple systems and interdisciplinary work will be coordinated.
SD-03 Product Data
Catalog cuts, illustrations, schedules, diagrams, performance charts, instructions and brochures illustrating size, physical appearance and other characteristics of materials, systems or equipment for some portion of the work.
Samples of warranty language when the contract requires extended product warranties.
SD-06 Test Reports
Report signed by authorized official of testing laboratory that a material, product or system identical to the material, product or system to be provided has been tested in accord with specified requirements. (Unless otherwise specified testing must have been completed within three (3) years of date of contract award for the project.)
Report which includes findings of a test required to be performed by the Contractor on an actual portion of the work or prototype prepared for the project before shipment to job site.
Report which includes finding of a test made at the job site or on sample taken from the job site, on portion of work during or after installation.
Investigation reports.
Daily logs and checklists.
Final acceptance test and operational test procedure.
SD-07 Certificates
Statements printed on the manufacturer's letterhead and signed by responsible officials of manufacturer of product, system or material attesting that product, system or material meets specification requirements. Must be dated after award of project contract and clearly name the project.
Document required of Contractor, or of a manufacturer, supplier, installer or subcontractor through Contractor, the purpose of which is to further quality of orderly progression of a portion of the work by documenting procedures, acceptability of methods or personnel qualifications.

SD-11 Closeout Submittals
Documentation to record compliance with technical or administrative requirements or to establish an administrative mechanism.
Special requirements necessary to properly close out a construction contract. For example, Record Drawings and as-built drawings. Also, submittal requirements necessary to properly close out a major phase of construction on a multi-phase contract.

1.1.3 Work
As used in this SECTION, on- and off-site construction required by contract documents, including labor necessary to produce submittals, construction, materials, products, equipment, and systems incorporated or to be incorporated in such construction.
1.2 SUBMITTAL CLASSIFICATION
Submittals are classified as follows:
G - This code is for submittals requiring government approval that were listed in the original government-generated specs.
I - This code is used for both Government-generated submittals and Contractor-generated submittals. This code is reserved for submittals that don't require approval, e.g. test reports.
1.2.1 Government Approved (G)
Government approval is required for extensions of design, critical materials, variations, equipment whose compatibility with the entire system must be checked, and other items as designated by the Contracting Officer. Government approval is required for any variations from the Solicitation or Accepted Proposal and other items as designated by the Contracting Officer. Within the terms of the Contract Clause entitled "Specifications and Drawings for Construction," submittals are considered to be "shop drawings."

Submittals not requiring Government approval will be for information only
(FIO).
1.3 PREPARATION
1.3.1 Transmittal Form
The attached sample transmittal form (ENG Form 4025) is in RMS and shall be completed in RMS 3.0 Contractor Mode. Use ENG Form 4025 for submitting both Government approved and information only submittals in accordance with the instructions on the reverse side of the form.
Properly complete this form by filling out all the heading blank spaces and identifying each item submitted. Exercise special care to ensure proper listing of the specification paragraph and sheet number of the contract drawings pertinent to the data submitted for each item.
1.3.2 Identifying Submittals
When submittals are provided by either the Prime Contractor or subcontractor, the Prime Contractor is responsible to prepare, review and stamp with Contractor's approval all specified submittals prior to submitting for Government approval.
Identify submittals, except sample installations and sample panels, with the following information permanently adhered to or noted on each separate component of each submittal and noted on transmittal form. Mark each copy of each submittal identically, with the following:
a. Project title, location, and date.
b. Contract number.
c. Date of the drawings and revisions.
d. Name, address, and telephone number of subcontractor, supplier, manufacturer and any other second tier Contractor associated with submittal.
e. Section number of the specification section by which submittal is required.
f. Submittal description (SD) number of each component of submittal.
g. When a resubmission, add a number suffix on submittal description, for example, submittal 18 would become 18.1, to indicate the resubmission.
h. Product identification and location in project.
The sequence of submission of drawings shall be such that all information is available for checking each drawing when it is received. Contract number and project shall be shown directly above the revision space on all drawings submitted. Drawing details and notes shall be of such quality and clarity and of sufficient definition and line weight as to permit sharp and totally legible scanning.
