QUALITY ASSURANCE SURVEILLANCE PLAN

Chief Joseph Dam Grounds Maintenance

Contract Number:

Contract Description: Services include but are not limited to: Lawn mowing, edging, trimming, pruning, debris collection and garbage removal. The Contractor shall meet all the requirements of this Performance Work Statement (PWS) to perform the general grounds maintenance services.

Contractor’s name: Chief Joseph Dam Grounds Maintenance Services (hereafter referred to as the contractor).

1. PURPOSE. This Government Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for this contract. The QASP explains the following: (1) what will be monitored; (2) how monitoring will take place; (3) who will conduct the monitoring; and (4) how monitoring efforts and results will be documented.

This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance. In addition, the QASP should recognize that unforeseen and uncontrollable situations may occur.

This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government will coordinate changes with the contractor. Updates will ensure that the QASP remains a valid, useful, and enforceable document.

This QASP was developed to comply with the Federal Acquisition Regulation (FAR), Subpart 37.6, Performance Based Acquisition and 46.4, Government Contract Quality Assurance. In addition, FAR, Subpart 46.103, Contracting Officer Responsibilities, states that agencies will develop quality assurance surveillance plans when acquiring services.

2. GOVERNMENT ROLES AND RESPONSIBILITIES.

Contracting Officer (KO) - The KO will ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and will safeguard the interests of the United States in the contractual relationship. The KO will also assure that the contractor receives impartial, fair, and equitable treatment under this contract. The KO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.

Contract Specialist - The Contract Specialist acts as an acquisition consultant and serves as the liaison between the contractor and the KO.

Contracting Officer’s Representative (COR) The COR is responsible for technical administration of the contract and will assure proper Government surveillance of the contractor’s performance pertaining to the recreational and vault toilets of the contract. The COR is not empowered to make any contractual commitments or to authorize any contractual change.

3. PERFORMANCE REQUIREMENTS SUMMARY (PRS): The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable quality levels (AQL) of service required for each requirement. The Government will use these requirments to determine contractor performance and shall compare contractor performance to the PRS thresholds.

3.1 ACCEPTABLE QUALITY LEVEL (AQL): Predetermined value used by the Government to distinguish between satisfactory and unsatisfactory performance. AQL may be stated in percent and is the maximum number of defects per 100 units that can be considered satisfactory. As long as the defective performance does not exceed the AQL, the total service will not be rejected by the Government. The contractor, however, must reperform the defective service at no cost to the Government.

3.2 DEDUCTIONS: Deductions will be made on a monthly basis during the billing cycle when the observed acceptance rate (OAR) fails to meet AQL. Deductions will be made to specific line items in the following manner:

a 10% line item deduction will be made when the OAR is within 5% below the AQL for that specific line item; a 20% deduction will be made when the OAR falls 6-10% below the line item AQL; a 50% deduction will be made when performance falls 11-25% below the line item AQL; and a 100% deduction will be made when performance falls 26% or below the line item AQL. OAR deduction calculations will always be rounded up to the nearest percentage point.

PRS 6: Edging 5.1.3 95% Random Sampling PRS 7: Trimming 5.1.4 95% Random Sampling PRS 8: Pruning 5.1.5 95% Random Sampling

4. METHODS OF QA SURVEILLANCE: Various methods exist to monitor performance. The Government will use the surveillance methods listed below in the administration of this QASP.

100% Inspection. Evaluates all outputs. Most applicable to small quantity, but highly important services. May be used where there are written deliverables and stringent requirements such as tasks required by law, safety, or security.

Random Sampling. Designed to evaluate the outputs of the award requirement by randomly selecting and inspecting a statistically significant sample. Highly recommended for large quantity repetitive activities with objective and measurable quality attributes.

Surveillance results may be used as the basis for actions (to include payment deductions) against the contractor. In such cases, the Inspection of Services clause in the Contract becomes the basis for the KO’s actions.

5. DOCUMENTING PERFORMANCE: The contractor’s performance will be compared to contract requirements. The contractor’s progress report accompanying invoices will be the primary basis, using the standards established as performance thresholds by this QASP to guide evaluations, surveillance methods, and document findings. Work that is deficient, and does not meet contract requirements, shall be re-done by the contractor at no cost to the Government.

Acceptable Performance. The Government will document positive performance. A report template is attached.

Any report format chosen by the COR may become a part of the supporting documentation for firm fixed fee payments or other actions.

Unacceptable Performance. When unacceptable performance occurs, the Government will inform the contractor.

This will normally be in writing unless circumstances necessitate verbal communication.

When the Government determines formal written communication is required, a Contract Discrepancy Report (CDR), will be prepared and furnished to the Contractor's primary or alternate point of contact. A CDR template is attached to this QASP. The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR. The Government will review the contractor's corrective action plan to determine acceptability.
