Mr. Greg Nespor, AIA WJA Design Collaborative 119 South Main Street, Suite 400 Seattle, Washington 98104 Subject: CLIN 0001 Repair Sanitary Sewer System

MTEF Facility, Yakima Training Center (YTC) Joint Base Lewis-McChord, Washington Med-Tox Northwest Project No. A-8321.9

Dear Mr. Nespor:

On March 13, 14 and 15, 2017 Anthony Fullerton and Ingrid Holznagel of Med-Tox Northwest (MTNW) conducted an asbestos survey of the underground water and sewer lines associated with CLIN 0001 Repair Sanitary Sewer System MTEF Facility YTC –Joint Base Lewis McChord, Washington. The existing water and sewer lines associated with the YRS compound were included in this survey. This survey was conducted to identify whether the pipes associated with both systems were suspect asbestos-containing materials.

MTNW employs inspectors certified by an Environmental Protection Agency (EPA)-approved training provider to provide Asbestos Hazard Emergency Response Act (AHERA) building surveys, including renovation and demolition surveys. A copy of Mr. Fullerton’s and Ms. Holznagel’s AHERA certificates are attached.

Applied Professional Services, Inc. (APS) provided potholing which included using water and air suction to excavate dirt to locate underground water and sewer lines. APS was contracted to locate several utilities while on site (water, sewer, electrical). MTNW sampled materials associated with the water and sewer pothole locations. Subsurface pipes were approximately four to ten feet underground. All potholing locations were identified by APS and shown on the attached site figure. Due to the destructive nature of the testing and to the risk of breaking a pipe, most sewer pipe samples were collected from manholes directly adjacent to identified potholing locations. The water pipe that was sampled came from an exposed pipe located next to the SCIF building. Conversations with the base water department representative indicated that the exposed pipe is typical of the buried pipe. Pothole locations and findings are as follows in Table 1:

The seven bulk samples collected in Table 3 were analyzed by Polarized Light Microscopy (PLM) dispersion staining EPA Method 600/R-93/116 by Seattle Asbestos Test, LLC (SAT). SAT is accredited through the National Voluntary Laboratory Accreditation Program (NVLAP) of the U. S. Department of Commerce. This accreditation does not constitute endorsement, but rather a finding of laboratory competence. The NVLAP participant number for SAT is 200768-0 laboratory competence. Their NVLAP participant number is 102021 (certification is attached). A copy of the laboratory analytical report is also attached.

Med-Tox Northwest recommends, and state law requires, that all asbestos materials identified in Table 1 be removed prior to renovation or demolition activities that may impact them. MTNW recommends that this survey report be placed on-site during site activities and copies provided to the contractor(s) bidding and performing work. Washington Industrial Safety and Health Act (WISHA), Occupational Safety and Health Administration (OSHA) require that the report be on-site and available for review during the entire project duration.

Sampling was performed consistent with the level of care and skill ordinarily exercised by professionals currently practicing under similar conditions in the area. No other warranty, expressed or implied, is made.

This report has been prepared for the exclusive use of WJA Design Collaborative and its’ designates for this project only. The analyses, conclusions, and recommendations presented in this report are based on conditions encountered at the time of our study and our experience and judgment. MTNW cannot be held responsible for interpretation by others of the data contained in this report; any use of this report shall include the entire document. This survey is not intended for use as abatement plans and/or specifications.
