Subject: Hazardous Materials Survey Report Addendum: Yakima Training Center, Building
Mr. Brad Endres, Innovex Environmental Management, Inc. (INNOVEX) is submitting this addendum to the previously issued Hazardous Materials Survey Report for Building 223 at Yakima Training Center (YTC) dated November 9, 2016. Per the request of Jacobs Engineering Group (JACOBS), INNOVEX collected additional suspect asbestos containing materials (ACM) inside the boilers and water tanks at Building 223 on November 16, 2016. The boilers and water tanks were previously not opened to access for sampling. Additionally, INNOVEX identified an additional paint coating to be sampled. This addendum includes the following documents:
Table 1 (Revised) — Asbestos Containing Materials Sample Inventory, YTC Building 223.
Table 1 has been revised to include the additional sampling of suspect ACM inside the boilers and water tanks at Building 223. Fifteen suspect ACM samples were collected. The samples are labeled as 223-19A, 223-19B, 223-20A, 223-20B, 223-21A, 223-21B, 223- 22A, 223-22B, 223-23A, 223-23B, 223-24A, 223-24B, 223-25A, 223-25B, and 223-25C.
Table 2 (Revised) — Lead-Containing Paint Sample Inventory, YTC Building 223. Table 2 has been revised to include the sampling of an additional paint coating identified during sampling on November 16, 2016. The additional sample is labeled as 223-Pb-12.
Revised Appendix C — The samples identified above were submitted to EMSL Analytical, Inc. on November 17, 2016 for Asbestos Analysis of Bulk Materials by EPA 600/R-93/116 using Polarized Light Microscopy (PLM) and Lead in Paint Chips by Flame Atomic Absorption Spectroscopy (FAAS). No asbestos or lead was detected in any of the samples collected as identified above.

4.0 RECOMMENDATIONS
Based on the information gathered during the hazardous materials survey, INNOVEX makes the following recommendations.
4.1 ASBESTOS
Planning and coordination of ACM removal should begin prior to renovation and/or demolition activities. The ACM identified in this report should be removed by certified, trained, and protected personnel using appropriate work practices and engineering controls prior to disturbance by renovation or demolition, as outlined in WAC 296-2, I-1.
Any additional, previously unsampled, suspect materials encountered during renovation and/or demolition activities should be sampled by a certified AHERA Building Inspector and analyzed by a NVLAP-accredited laboratory to evaluate asbestos content prior to disturbing the material.
4.2 LEAD
A representative sample of the demolition waste associated with samples 3423-Pb-04, 3423-Pb- 06, and 3423-Pb-07, each of which contained elevated concentrations of lead, should be collected and analyzed using a toxicity characteristic leaching procedure (TCLP) to characterize the waste for disposal. Demolition waste streams with leachable lead concentrations exceeding 5.0 milligrams per liter when prepared for analysis using the TCLP method are considered hazardous and require special handling according to federal and state regulations, including 40 CFR 247.
Any contractor who may come in contact with materials containing lead at any detectable concentration is required to address worker exposure responsibilities as outlined in WAC 296- 155-176.
Should additional, previously unsampled, painted surfaces be revealed through renovation and/or demolition activities, INNOVEX recommends the coatings be sampled by a WSDOC-certified inspector and analyzed to evaluate lead content prior to destruction, removal, or personnel exposure.
4.3 POLYCHLORINATED BIPHENYLS
During demolition activities, any light ballast without “No PCBs” labeling should be handled and disposed of according to state and federal regulations (i.e., workers should employ proper personal protective equipment when handling the ballasts and properly store the ballasts in sealed plastic bags or buckets to minimize potential contact with any exposed or leaking PCB-containing oil). All PCB waste must be labeled, manifested, transported, and disposed of according to federal and state regulations, including 40 CFR 761.
4.4 MERCURY
Fluorescent and HID light tubes may contain mercury vapors. These tubes and mercury-containing heat thermostats should be removed without breakage and disposed of property in accordance with WAC 173-303-573, the Standards for Universal Waste Management.
Hazardous Materials Survey Report_JBLM_F Page 6 of 6
4.5 CHLOROFLUOROCARBONS
During demolition activities, HVAC systems should be evacuated of refrigerant prior to being impacted by demolition activities. Any discharge of spent CFCs to the environment constitutes disposal and is subject to full regulation under WAC 173-303, Dangerous Waste Regulations. All refrigerant must be properly transported, recycled, and reclaimed in accordance with all applicable federal, state, and local regulations by an authorized hazardous waste handler.
4.6 BUILDING OCCUPANT-DERIVED WASTE
During demolition activities, any BODW identified should be separated from the general demolition waste stream, transported by an authorized hazardous materials transporter, and disposed of at an authorized waste facility.

INNOVEX was not provided access to the Utility Room or the roof. The Utility Room was locked at the time of the survey. Manuel Camacho (JBLM, Maintenance) was contacted during the survey to access the Utility Room and the roof. According to Mr. Camacho, the lock on the Utility Room door was stuck in-place and was inaccessible. INNOVEX was subsequently unable to sample suspect ACM in the Utility Room (Photo 8, Appendix A). Mr. Camacho was also unable to locate a ladder to access the roof and the inclement weather outside prevented safe access to the roof.
INNOVEX was subsequently unable to sample suspect ACM on the roof.
INNOVEX performed non-destructive sampling of suspect ACM, per the request of Mr. Camacho.
INNOVEX was therefore unable to access inside the wall cavities inspecting for suspect ACM.
Hidden suspect ACM such as vermiculite may be present in the cement masonry walls, along with insulation piping inside the interior walls.
Opinions and recommendations contained in this report apply to conditions existing when services were performed and are intended only for the client, purposes, locations, time frames, and project parameters indicated.
