INSTRUCTIONS FOR OFFERORS

Section SF 30 - BLOCK 14 CONTINUATION PAGE

Instructions For Offerors

1. Offers shall be prepared in accordance with these instructions, providing all required information in the specified format. Official submission will be documented in accordance with the date and time stamp of the email submission.

2. Proposals shall be submitted via email ONLY to Mesia Luster at Mesia.E.Luster@usace.army.mil no later than due date listed in Box of the Solicitation.

3. The Government will honor Requests for Information (RFI) regarding this procurement. Should the Offeror choose to submit questions, they must be submitted by email to Mesia.E.Luster@usace.army.mil no later than 11:00AM EST on 7 AUG 2019. This will allow responses to be posted on FBO prior to solicitation closing. Questions may not be accepted after the cutoff date/time above.

4. A site visit will be held for prospective bidders on August 5, 2019 at 0800. Prespective bidders are advised to contact Mr. Charles P. Robitaille at (202) 595-4961 or by email at Charles.P.Robitaille@usace.army.mil to register for one.

5. System for Award Management Registration – By submission of a quote, the Offeror acknowledges the requirement that prospective awardee must be registered in the System for Award Management (SAM) database prior to award, during performance, and through final payment of any contract resulting from this proposal. Lack of registration in the SAM database will make an Offeror ineligible for award. Offerors and contractors may obtain information on registration and annual confirmation requirements by calling 1-888-227-2423, or at http://www.sam.gov.

6. Submission Criteria – The offer will be evaluated to ensure all items are provided:

a. Cover Letter on Offeror’s letterhead. Letter shall summarize the pricing total and include Offeror specific details such as CAGE Code, Tax ID#, and DUNS #. Letter shall be signed by the representative managing the account.

b. Capability Statement. The proposed capabilities should include adequate documentation to illustrate the Offeror’s capabilities to successfully complete the work identified within the solicitation.

c. Two (2) Past Performance references for similar projects. Projects must have been completed within the previous five (5) years, and/or

d. Two (2) Past Performance Questionnaires below or references.

e. Bid Schedule – Government template provided.

7. Evaluation Criteria rating – The Government will award a firm-fixed price (FFP) contract to the responsible Offeror whose offer conforms to the requirements as stated in this request for quotation (RFQ).

The award will be made using the Lowest Priced Technically Acceptable (LPTA) method based on three (3) evaluation factors – technical, past performance, and price. The Government intends to evaluate quotes and make award without questions to Offerors.

Therefore, the Offeror's initial quote should contain the Offeror's best terms from a technical and cost standpoint. Unclear quotes may not result in questions for clarifications. Offers with an omission or unclear quote may be judged to mean the contractor does not fully understand the requirements or understand what it takes to meet the requirements, regardless of the proposed cost/price, and will be rated unacceptable. In that case, the Offeror may be found unacceptable, and ineligible for award.

The Government reserves the right to ask questions of Offerors if the Contracting Officer determines them to be necessary.

f. Technical – To be technically acceptable, the Offeror must provide information as specified in the attached Scope of Work and in the above Submission Criteria. Each submittal will be evaluated to ensure requirements are met. The responsiveness and quality of information delivered by the Technical Service Department contacts provided shall also be considered in evaluating technical acceptability.

Table A-1. Technical Acceptability

Rating
Description

Acceptable Proposal clearly meets the minimum requirements of the RFQ (Request for Quotation)

Unacceptable
Proposal does not clearly meet the minimum

requirements of the RFQ

g. Past Performance – Past performance refers to the quality of recent project experience from the customer’s perspective. The Offeror shall arrange for the reference customer point of contact for three projects to fill out the Questionnaire (Form PPQ-0 attached below) and return directly to Mesia Luster Contract Specialist, e-mail at Mesia.E.Luster@usace.army.mil. The questionnaires must be forwarded directly by the references. The Government will not release the questionnaire forms to the Offeror at any time, in order for the Government to solicit candid, unbiased responses and comments. Two (2) references from previously completed jobs must be submitted and include a point of contact and phone numbers. These references must be relevant to the work which will be performed in this contract. Relevant work is defined as Dry Fog Mold Remediation and Prevention Treatment. References must be recent within the past 5 years.

Past Performance will be evaluated utilizing the information provided by the Contractor, the Past Performance Questionnaires and three (3) references and by checking other sources such as the Contractor Performance Assessment Reporting System (CPARS).

Any Offeror who has demonstrated satisfactory past performance will be considered eligible for award. Any Offeror who demonstrated unsatisfactory past performance will not be considered eligible for award. If the Government does not obtain past performance information for the projects identified by the Offeror and cannot establish a past performance record for the Offeror through other sources, or if the Offeror has no past performance record and cannot tie significant, relevant experience of key personnel proposed for this project to cited experience, past performance will be evaluated acceptable, unacceptable or neutral.

ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT YOUR EVALUATION OF THE CONTRACTOR’S PERFORMANCE

RATING
DEFINITION
NOTE
(E) Exceptional
Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO

significant weaknesses identified.

(VG) Very Good
Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub- element being assessed was accomplished with

some minor problems for which corrective actions taken by the contractor were effective.

A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.

(S) Satisfactory
Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than

Satisfactory solely for not performing beyond the requirements of the contract.

(M) Marginal
Performance does not meet some contractual

requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.

A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.

(U) Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone

constitutes an unsatisfactory rating.

(N) Not Applicable No information or did not apply to your contract Rating will be neither positive nor negative.

PERFORMANCE WORK STATEMENT

PERFORMANCE WORK STATEMENT

DRY-FOG MOLD REMEDIATION AND PREVENTION TREATMENT

BUILDING 60, FORT MCNAIR, WASHINGTON DC

1.INTRODUCTION: Mold is a fungus that can grow on virtually any substance, damaging buildings and negatively affecting the health of building occupants. Joint Base Myer-Henderson Hall (JBMHH) recently identified mold in a number of their facilities on JBMHH in Arlington, VA and Fort McNair in Washington DC. The Installation Directorate of Public Works (DPW) requested USACE perform a Hazardous Material (HAZMAT) survey and facility evaluation for all occupied facilities. All of the facilities surveyed thus far have concentrations of mold higher than levels found outdoors. The preferred solution is to control and eliminate the source of moisture that precipitates the mold growth in all affected facilities. However, this is an ever-changing problem with an unachievable immediate long-term solution within JBMHH. This is due, in large part, to building personnel throughout JBMHH adjusting system specific HVAC set points to achieve their immediate comforts. Other physical controls such as windows, doors, etc. are opened or closed to satisfy current employee comforts without any regard to the impact on the larger “system” of controls. These adjustments inevitably create less than optimal operational conditions which frequently enhance the already prime onsite environmental conditions for mold growth. Furthermore, many facilities suffer from deficient architectural, site, and HVAC features that promote mold growth. Deficient maintenance is also a reoccurring issue on JBMHH and often contributes to optimal conditions for mold growth. The immediate goal of the JBMHH DPW is to identify possible solutions that kill existing mold and deter its regrowth in a manner that renders facilities habitable in a one day time period. If this can be achieved, mold remediation procedures could be used in living quarters with minimal impact to service members.
Typical mold remediation technologies require intensive manpower and various levels of personal protective equipment (PPE) to be worn during the removal and prevention processes. The Dry-Fog Mold Remediation and Prevention Process requires no PPE and minimal manpower, as the application system itself performs the work required to destroy existing mold spores and prevent future growth. This application is based on a patented application technology. Dry Fog mold remediation differs from all other known technologies in two ways: 1) the fog is generated via particles small enough that contacted surfaces do not become wet, hence, the “dry-fog” and 2) it is applied in two separate efforts (a) a product formulated and applied to kill/remove the microbial matter and (b) a product formulated and applied to treat/protect the affected surfaces. The goal of this application is to demonstrate and verify the unmatched effectiveness and ease-of-use of this technology on Fort McNair to the JBMHH DPW so they may best shape future remediation efforts on their installations.
2.OBJECTIVE: The objective of this requirement is to perform proof of concept testing of this dry-fog mold remediation and prevention process used to eliminate mold and prevent regrowth on JBMHH and Fort McNair. More specifically, the technical objectives are to:
a.Evaluate the efficiency of the dry fog remediation process and determine the length of time required to treat a facility.
b.Determine the efficacy and performance (via sampling and analysis) of the dry-fog process.
c.Verify initial remediation impact(s) and non-reoccurrence of mold/mildew (via sampling and analysis).
3.MAJOR REQUIREMENTS: In order to accomplish the work, it shall be necessary for the Contractor to complete the following tasks:
a.Site Validation. Prior to initiation of any onsite activities, the Project Delivery Team (PDT), to include Government and contractor personnel, shall conduct a telecon to discuss the installation location and determine if the project goals and objectives are attainable given existing site conditions. The PDT shall meet with local personnel, and the details of the requirements of the project shall be discussed and verified within the tasks scheduled to be performed.
b.Baseline Sampling and Analysis. An initial visual assessment via onsite inspection and photographs shall be conducted. Additionally, sampling and analysis for mold shall be performed to document the existing conditions and abundance of mold. Samples shall be analyzed via two methods 1) air quality, and 2) tape surface pull. Indoor and outdoor air sampling shall be performed in order to determine onsite baseline parameters for comparative analysis. Prior to sampling, a sampling plan shall be developed to ensure proper and adequate sampling as well as the appropriate analytical methods are performed in order to meet all project requirements.
c.Application. Perform the application of their two-step dry-fog mold remediation and prevention process on a total square footage of office space, at Fort McNair, Washington DC, (with a total treated area not to exceed 22,500 square feet). Application shall occur over a one (1)-day period and shall be coordinated with JBMHH DPW and Fort McNair personnel.
d.Verification Sampling and Analysis. Once application is complete, verification testing shall be performed and sent to an independent third party for analysis and report.
e.Reporting. Prepare report detailing the results and findings of the project. Both DRAFT and FINAL versions of a Final Report are also required. The FINAL version of the Final Report shall summarize the activities performed and due at the end of the performance period. The calendar due date shall be determined during the onsite meeting identified in Task 3.a above.
4.GOVERNMENT FURNISHED INFORMATION/MATERIALS: The government will provide the following:
a.Government personnel to support access to JBMHH and Fort McNair as well as escort while on JBMHH and Fort McNair property.
b.Government personnel to evaluate and analyze the sampling results and additional findings as they are realized during the demonstration.
5.No Government personnel, other than the Contracting Officer, shall have the authority to do other than clarify technical points or supply relevant information to the Contractor regarding this contract. Specifically, no requirement in these specifications may be altered through verbal clarification. Only a warranted Contracting Officer (either a Procuring Contracting Officer (PCO), or an Administrative Contracting Officer (ACO)), acting within their delegated limits, has the authority to issue modifications or otherwise change the terms and conditions of this contract. If an individual other than the Contracting Officer attempts to make changes to the terms and conditions of this contract you shall not proceed with the change and shall immediately notify the Contracting Officer.
6.NON-PERSONAL SERVICES: The Government and the Contractor hereby understand and agree that the services to be delivered under this contract by the Contractor to the Government are non-personal services, and the parties recognize and agree that no employer-employee or master-servant relationships exist or will exist under this contract between the Government and the contractor's employees.

b. UNACCEPTABLE PERFORMANCE.

When unacceptable performance occurs, the Technical Monitor shall inform the contractor. This will normally be in writing unless circumstances necessitate verbal communication. In any case the Technical Monitor shall document the discussion and place it in the Technical Monitor file.

When the Technical Monitor determines formal written communication is required, the Technical Monitor shall prepare a Contract Discrepancy Report (CDR), and present it to the contractor's task manager or on-site representative. A CDR template is attached to this QASP.

The contractor shall acknowledge receipt of the CDR in writing. The CDR will specify if the contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the KO. The Government shall review the contractor's corrective action plan to determine acceptability.

Any CDRs may become a part of the supporting documentation for contract payment deductions, fixed fee deductions, award fee nonpayment, or other actions deemed necessary by the KO.
