SECTION SF 30 BLOCK 14 CONTINUATION PAGE
SUMMARY OF CHANGES
SECTION SF 30 - BLOCK 14 CONTINUATION PAGE
The following have been added by full text:
RESPONSE TO QUESTIONS
1) Two relevant projects (past 3 years) with client reference past performance questionnaires (to be sent directly from the client to Baltimore District)
Three references within the past 5 years
2) Completed Price Schedule
Yes
3) Can you please clarify if electronic submission of proposals via email will suffice? The SF1449 indicates that contractors are to submit 2 copies of the proposal to the issuing office however, page 3 indicates that proposals can be submitted electronically via email.
Proposals for Solicitation No. W912DR-15-T-0100 may be submitted electronically via email.
4) The RFP includes a requirement for Past Performance Questionnaires (PPQs) to be submitted for two projects in the past 3 years. However, Factor 1 Past Performance only requires a 5 page written description of contractors relevant experience on 3 projects in the past 5 years. The requirement for PPQs are different than for the experience requirements for Factor 1 and the PPQs are not mentioned in the Factor 1 Proposal Evaluation Criteria. Can the USACE please clarify if
a. PPQs are to be submitted and if so, are they required for the projects to be provided in Factor 1?
Yes, three references within the past 5 years
b. If PPQs are required, can PPQs that have been prepared by our clients for previous USCE proposals using the same NAVFAC/USACE PPQ format be submitted with our proposals in lieu of asking our clients to complete and submit the forms again?
Recent PPQs pertaining to the required services will be acceptable.
5) Does the USACE expect unit rates to be fully loaded and include project management time, per diem, travel time to sites, EM385-1-1 HASP....? Or will these items be able to be negotiated per task order?
Contractor should use fully loaded rates for purposes of this RFQ.
6) The bid sheet asks for bidders to include equipment transportation costs in unit rates. Is there a certain amount of mileage to assume and then any additional will be included in task orders as mobilization? For example, the transportation cost is different for New York than South Carolina.
Per the “**” notation: Transportation cost is daily rate to haul maximum of 20 tons. Bidders should provide maximum daily rate to haul up to 20 tons.
7) At sites where MPPEH is needed to be classified as MDAS is it acceptable to have a larger crew than 1-operator and 2 technicians? Or will task orders allow for site specific personnel needs?
For the purposes of this RFQ, unit price bids should assume 1-operator and 2 technicians.
8) Can the USACE separate the transportation to 2 columns 1) transportation of equipment to the site and 2) transportation of 20 tons to the disposal facility?
No. Per the “**” notation: Transportation cost is daily rate to haul maximum of 20 tons. Bidders should provide maximum daily rate to haul up to 20 tons of material.
9) Price Schedule- Transportation Cost- Is that for transportation of product, or does that include Mob & De-Mob of equipment and personnel? If it does not include Mob & De-Mob of equipment and personnel, where is that to be included?
This is a unit price to either haul equipment or Material. Unit is cost per day for up to 20 tons
10) 52.212-2 Evaluation- Commercial Items a. Project References-
Will CPARS be accepted in lieu of Past Performance Questionnaires?
Yes
11) 52.212-2 Evaluation- Commercial Items a. Project References- Do you want Past Performance broken out by Task Order or total contract?
By Project
12) In the Past Performance Submission the requirement is two references within the past 3 years, however, in the evaluation criteria you ask for three references within the past 5 years, please clarify what is required.
Three references within the past 5 years
13) Performance Work Statement- 4. Performance Requirements “MPPEH Shall be documented by authorized and technically qualified personnel as Material Documented as Safe (MDAS) after 100% inspection and an independent 100% re-inspection to determine that it is free of energetic material.”
Are you referring to an independent 3rd Party QA, and if so does the cost for independent inspection need to be included in the pricing that is submitted or is the installation going to handle the independent inspection?
Contractor will be required to conduct MPPEH Inspection per guidance outlined in DoDI 4140.62.
14) Regarding the Past Performance Questionnaires (PPQs), do all PPQs need to represent the offeror’s prime contractor team member or can PPQs representing the offeror’s other team members be submitted?
All team members can be submitted
15) Between pages 3 and 4 of 44 if the pdf of the solicitation package, Evaluation Criteria: There is reference to “relevant experience of key personnel proposed”. However, there is no requirement to identify key personnel and their experience. Are identification of key personnel and their relevant experience required proposal submittals and/or will they be part of the evaluation criteria?
No
16) Section 52.212-2: Regarding the three project references to be included in Factor 1 – Past Performance, do all project references need to represent the offeror’s prime contractor team member or can project references representing the offeror’s other team members be submitted?
Others may be submitted
17) PWS Section 2: Regarding MDAS awaiting bulk demilitarization and processing, will this material have supporting documentation certifying and verifying it as MDAS and be properly containerized/sealed prior to the offeror arriving on site?
MDAS will have appropriate documentation and be held in a secure location. Containerization is the responsibility of the contractor
18) Will the on-site USACE POC sign 1348-1 documentation? If so, under what circumstances?
MDAS certification will be prepared in accordance with DoDI 4140.62.
19) PWS Section 3.3: For the purposes of this bid, should the offeror complete the Bid Sheet based on execution of work within the fourteen states listed in Section 3.3 and DC or anywhere in the Continental United States?
Offeror should complete Bid Sheet in accordance with information listed in Section 3.3 of Performance Work Statement
20) PWS Section 4: This section states, “MPPEH shall be documented by authorized and technically qualified personnel as Material Documented as Safe (MDAS) after 100% inspection and an independent 100% re-inspection to determine that it is free of energetic material.”
a. Must the “independent 100% re-inspection” be performed by an independent third party UXOQC?
Inspection/certification of MDAS will be in accordance with DoDI 4140.62
21) Bid Sheet: Is it up to the offeror to factor in the net proceeds from any metals recycling into their price or does the government have any requirements for returning the funds back to the contract or other recipient?
Government has no requirements for returning the funds back to the contract or other recipient
22) Bid Sheet: The “Notes” sections refers to an “average production rate”. Is this rate based on a daily rate or some other unit?
Daily rate
23) Bid Sheet: The Bid Sheet appears to require pricing for shearing, shredding, crushing, etc. of MPPEH, which has not been fully evaluated and determined as MDAS/MDEH. Conversely, Section 2 of the PWS states, with regard to MPPEH, “processing, inspection, and certification of MPPEH as MDAS/MDEH”.
Given that shearing, shredding, crushing, etc. of MPPEH are typically cost-prohibitive compared to explosively treating, it is assumed that MPPEH will be evaluated and determined MDAS or MDEH and that only the resulting MDAS will be subjected to shearing, shredding, etc. Is this the correct interpretation of Section 2 and the Bid Sheet and/or can the government clarify?
Bid sheet clearly identifies mechanical destruction for MDAS and additional treatment/inspection for
MPPEH
