CAUTION PAGE:
CAUTION!

1. AMENDMENTS: Have you acknowledged receipt of ALL Amendments? If in doubt as to number of amendments issued, please contact our office.
2. QUOTES: Shall be marked to show the Contractors name and address, the solicitation number, title of project, amendments received, and the date and time quotes are due.
3. AMENDED QUOTE PAGES: If any of the Amendments furnished amended quote pages, the amended quote page(s) must be used in submitting your quote.
4. LATE QUOTES: In order for a late mailed quote to be considered, generally it must have been sent by either registered or certified mail not later than 5 calendar days before the receipt of quotes due date.
5. GUARANTEE: Sufficient guarantee in proper form must be furnished with your quote, if your quote exceeds $150,000 pursuant to FAR Clause 52.228-1.
6. MISTAKE IN QUOTE: Have you reviewed your quote prices for possible errors in calculations or work left out?
7. FACSIMILE QUOTES AND MODIFICATIONS: Will be considered. Facsimile withdrawals will be considered.
8. HAND-DELIVERED QUOTES: If quotes are hand-delivered, you must be aware of enhanced security requirements in effect in the Federal Building and allow for extra time to clear security. Please ensure that the solicitation number and title of project appear or can be identified on the outside of quote package.
9. BUY AMERICAN ACT: All Contractors are cautioned that, prior Government conduct notwithstanding, the Contractor's selection of a domestic construction material which would require the subsequent selection of a foreign construction material for compatibility is not a justification for waiver of the Buy American Act. It is the Contractor's responsibility to verify, prior to submitting the materials for approval that each system can be built to meet the contract specifications without the use of foreign construction materials.
10. SYSTEM FOR AWARD MANAGMENT (SAM): In accordance with FAR 52.204-7: By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Offerors may obtain information on registration and annual confirmation requirements via the SAM database accessed through https://sam.gov/portal/SAM/#1 . It is highly encouraged that all prospective Offeror’s start the SAM’s registration process as soon as possible.
OFFEROR MUST BE REGISTERED IN THE SYSTEM FOR AWARD MANAGEMENT WITHIN 7 DAYS
OF THE CLOSE OF THE SOLICITATION OR THE CONTRACT OFFICER MAY PROCEED WITH
AWARD TO THE NEXT SUCCESSFUL CONTRACTOR.
If you were not previously registered, please go to https://www.sam.gov. Instructions for new registration are also located at the following link https://www.sam.gov/sam/transcript/Quick_Guide_for_Contract_Registrations_v1.7.pdf to register.
If you have questions about Government procurements in general or need assistance in the preparation of your proposal, a local Procurement Technical Assistance Center (PTAC) may be able to help. The Procurement Technical Assistance Program was authorized by Congress in 1985 in an effort to expand the number of businesses capable of participating in the Government marketplace. To locate a PTAC near you, go to http://www.aptac-us.org/sam-registration/ THIS A FREE SERVICE BY A NON PROFIT PAID FOR BY THE US GOVERNMENT
Alleged Fraudulent Activity in SAM
GSA has taken action to address alleged fraudulent activity in the System for Award Management (SAM). The measures GSA already put in place to help prevent improper activity in SAM include masking specific data elements in the entity registration even for authorized entity users; requiring “parent” approval of new registrations for their “child” entities; and requiring the formal appointment of the Entity Administrator by original, signed notarized letter.
Additional enhanced controls have been deployed. These controls include implementing multi-factor authentication using login.gov and notifying Entity Administrators when there is a change in the entity’s bank account information.
NOTARIZED LETTER PROCESS CHANGES
How has the notarized letter review process changed?
All non-Federal entities who create or update their registration in SAM.gov will no longer need to have an approved Entity Administrator notarized letter on file before their registration is activated.
This applies to you if your SAM.gov Purpose of Registration is either Federal Assistance or All Awards.
Check SAM.gov to find your Purpose of Registration.
All non-Federal entities still must mail the original, signed copy of the notarized letter to the Federal Service Desk.
Where can I find the notarized letter templates?
Read the instructions for domestic entities or instructions for international entities posted at the Federal Service Desk.
Where do I send the notarized letter?
You must mail the original, notarized letter to:

When an offeror submits its proposal in response to a Request For Proposals and that proposal contains any work, materials, methods or other type of betterment that provides an advantage to the Government that is above the minimum requirements contained in the RFP, the terms of the proposal will be the basis for evaluation and, if successful, will become the terms of the contract upon award. If a change in one of those terms is made to one that is less favorable to the Government than the one on which the award of the contract was based, a credit to the Government will be negotiated by the contractor. ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT
YOUR EVALUATION OF THE CONTRACTOR’S PERFORMANCE
RATING DEFINITION NOTE
(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.
(VG) Very Good Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.
(U) Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
(N) Not Applicable
