The QC system covers all construction operations, both onsite and offsite, and be keyed to the proposed construction sequence. The project superintendent will be held responsible for the quality of work and is subject to removal by the Contracting Officer for non-compliance with the quality requirements specified in the Contract. In this context the highest level manager responsible for the overall construction activities at the site, including quality and production is the project superintendent.
3.2 CONTRACTOR QUALITY CONTROL (CQC) PLAN
Submit no later than 15 days after receipt of notice to proceed, the Contractor Quality Control (CQC) Plan proposed to implement the requirements FAR 52.246-12 Inspection of Construction. The Government will consider an interim plan for the first seven days of operation.
Construction will be permitted to begin only after acceptance of the CQC Plan or acceptance of an interim plan applicable to the particular feature of work to be started. Work outside of the accepted interim plan will not be permitted to begin until acceptance of a CQC Plan or another interim plan containing the additional work.
3.2.1 Content of the CQC Plan
Include, as a minimum, the following to cover all construction-operations, both onsite and offsite, including work by subcontractors fabricators, suppliers and purchasing agents:

When production of the mixture can be maintained and when practical, pavers may be used in echelon to place the wearing course on adjacent lanes. The roller shall pass over the unprotected edge of the freshly laid mixture only when the laying of this course is to be discontinued for such a length of time as to permit the mixture to become chilled.
1.6.2.5 SUBSECTION 411.04 J. Joints
Append to the third paragraph the following:
Both longitudinal and transverse edges shall be so treated.
1.6.2.6 SUBSECTION 411.04 K. Compaction
Append the subsection with the following:
As soon after being laid as it will bear the compaction equipment without undue displacement, the asphalt concrete shall be thoroughly and uniformly compacted. Unless otherwise directed by the ACO, compaction shall begin at the sides and proceed longitudinally parallel to the road centerline and gradually progress to the crown of the road. When paving in echelon or abutting a previously placed lane, the longitudinal joint shall be compacted first followed by the regular compaction procedure. The surface course shall be rolled a minimum of five coverages with a pneumatic-tired roller unless the required density is obtained with less coverages.
Compactors shall move at a uniform speed not to exceed 3 miles per hour (4.4 feet/second) with the drive wheel nearest the paver. Compaction shall continue until all marks are eliminated and the required density has been obtained.
The target density for compaction shall be 94 percent of the maximum theoretical density shown on the latest laboratory mix design report for the percent asphalt content recommended or approved by the Contractor-furnished laboratory. The specified density shall be obtained before the surface temperature for the lift being compacted drops below 180 degrees F. The average roadway density shall not be less than the target density. Individual roadway densities more than 2 percent below the target density are unacceptable. Laboratory density shall be determined by the Superpave method. The Contractor shall cut test samples from the pavement by sawing or coring at locations and times established by the ACO. The cost of cutting samples, placing new materials, and satisfactorily finishing areas where samples have been taken will be included in the price bid for mixture in place.
Displacement occurring as a result of the reversing of direction of a compactor, or from other causes, shall be immediately corrected by the use of rakes and addition of fresh mixture when required. Care shall be exercised during compaction not to displace the line and grade of the edges of the asphalt concrete. Along places not accessible to the compaction equipment, the asphalt concrete shall be thoroughly compacted with hot hand tampers, smoothing irons, or with mechanical tampers. All asphalt concrete material that becomes loose and broken, mixed with dirt, or is in any way defective shall be removed and replaced with fresh hot mixture, which shall be compacted to conform to the surrounding area. Areas showing an excess or a deficiency of bituminous material shall be removed and replaced at the expense of the Contractor.

All sampling and testing herein required or required by the STANDARD SPECIFICATIONS and the special technical provisions thereto, including furnishing the equipment and labor necessary to perform the tests, shall be performed by the Contractor at his expense. All tests and test procedures to be used by the Contractor's quality control personnel in determining suitability of materials and operations shall be as described or referenced in the STANDARD SPECIFICATIONS and the Special Technical Provisions. The frequency of tests specified in the following subparagraphs shall be considered the minimum, and where a frequency is not specified, sufficient tests to assure compliance with the contract, as directed by the ACO, shall be performed. If any test indicates materials or methods do not comply with the contract, the test shall be repeated and material already in-place tested to determine the extent of unacceptable material. The frequency of testing shall be increased as directed by the ACO until the ACO deems compliance has been re-established. Tests shall be performed by qualified testing personnel in a laboratory approved by the Corps of Engineers' Materials Testing Center for the required tests. Testing personnel may at any time be directed by the ACO to demonstrate thorough knowledge and proficiency in any testing method they perform. If testing personnel can not demonstrate such knowledge and proficiency to the ACO's satisfaction, they will be deemed unqualified. All test results shall be provided to the ACO within 24 hours of testing.
1.9.2.2 Asphalt Concrete
a. Use of Commercial Laboratory
The Contractor shall furnish HMA mix designs obtained through the services of a Government-approved laboratory that is staffed and equipped to design and test the asphalt concrete mixture used in the project. The mix design shall be submitted to the ACO for approval. A separate mix design shall be provided in the event multiple sources are used to supply materials. In the event of a change of sources during the project, the Contractor shall provide a new mix design for approval by the ACO and no further work shall be performed until such approval has been granted in writing by the ACO.
b. Quality Control Sampling and Testing
The Contractor shall sample and test all materials in the mixture to determine compliance with the requirements of SECTION 708 of the STANDARD SPECIFICATIONS. The Contractor shall submit the test results to the ACO before placement begins.
Bitumen content tests (Test OHD L-26) and sieve analysis tests shall be made at a minimum frequency of one test per 500 tons of mix or fraction
SECTION 32 00 00 Page 7 thereof placed each day. Specific gravity tests shall be made on each hot bin for each mix during initial plant operation and at least once each week thereafter provided the same materials are used with no reduction in quality. A minimum of one set of Superpave specimens (three specimens per set) shall be molded daily. A daily record of all tests shall be kept and provided to the ACO on the day following completion of the tests.
During construction the Contractor shall test each lift of the compacted asphalt pavement for the required density at a frequency of at least one set of test samples for each 500 square yards or fraction thereof placed each day. One set of test samples is defined as one density test in each joint and one within each lane. Density tests shall be performed with a thin-lift nuclear device (such as a Troxler 4640B or approved equal). At each fifth nuclear test location, a set of core samples shall be taken for a laboratory density measurement. The density value from a core shall supercede the density values from a nuclear device. If the density value from a nuclear device varies more than 2% from a core density value, then use of the nuclear device shall be suspended, and all density tests shall be performed on core samples. At the Contractor's option, density may be tested solely using core samples.
c. Thickness Compliance
The completed thickness of the asphalt concrete shall be the nominal thickness shown on the drawings. The thickness of the surfacing shall be measured at intervals of not more than 500 linear feet for each single lane width.
