DIVE OPERATION SPECIFICATIONS

DDC 16-07: Alamo Dam Bulkhead Removal, Bulkhead Slot and Bottom Sill Inspection and Repair 28 November – 02 December 2016

1. GENERAL: The services of a five/six (5/6)-person dive team (depending if options are awarded) are required to extract the bulkhead gate at the intake structure of Alamo Dam, inspect the steel bulkhead slots and bottom sill at the intake entrance to the tunnel, reinstall the bulkhead into the slot and re-secure it in its storage location. This work will take place on or about 29-30 November 2016 and as directed by the Government Representative on site.

All underwater activities shall be displayed on a monitor in real-time and video taped. The original video shall be adequately labeled and furnished to the GOAR and / or DDC in digital or DVD format upon completion of job.

Project can be accessed from the Alamo Dam Access Road off of US-60 E. Entry to the dive-staging area will be via boat from the Lakeview Dr. boat ramp.

The Dive Team shall be on-site at 29 November 2016, 8am at the Alamo Dam administration building parking lot.

The teams should expect up to two 10-hour shift of in-water and surface work, 10 hours total per person. All operations shall be conducted at the direction of an on-site, Government representative.

2. FACILITIES Alamo Dam is a multiple-purpose project that provides flood risk reduction, water supply and conservation, recreation, and fish and wildlife enhancement. It is located on the Bill Williams River where the Big Sandy River and Santa Maria River come together, about 35 miles upstream from Lake Havasu, and 75 miles northwest of Phoenix.

3. SUBMITTALS Prior to the commencement of operations the contractor shall provide the government representative/DDC/ADC with the required submittals in accordance with the regulations reference above. These submittals include the following:

Safe Practices Manual (per EM 385-1-1, paragraph 30.A.15) Dive Operations Plan(s) (per EM 385-1-1, paragraph 30.A.16) Accident Hazard Analysis (per EM 385-1-1, paragraph 30.A.17) Emergency Management Plan (per EM 385-1-1, paragraph 30.A.18) Dive Personnel Qualifications (per EM 385-1-1, paragraph 30.A.05-30.A.09)

Rigging Personnel Qualifications for diver (per EM 385-1-1, paragraph 15.B) All Equipment Certifications/Certificates (per EM 385-1-1, paragraph 30.F)

4. REQUIREMENTS: The dive shall conform to the requirements of the U.S. Army Corps of Engineers Safety Manual (EM-385-1-1, latest edition), 29 CFR 1910, Subpart T, the U.S. Navy Diving Manual (latest edition) as well as any other state or federal regulations that may apply. Copies of all operational dive logs shall be furnished to the District Diving Coordinator upon completion of the mission. If divers are required to perform rigging duties, they must be a qualified rigger and meet the personnel qualifications listed in Section 15.B.

5. DIVE TEAM: The Dive Team shall consist of a minimum 5 or 6-person crew (depending on dive) for all dives. The crew shall comprise of a dive supervisor, diver, stand-by diver, in-water dive tender (as needed), dive tender, and communications operator/stand-by tender. All dive team members shall have on file, with the Los Angeles/Buffalo District Office of Dive Safety, current work documentation (dive logs, etc) and certifications showing that they are qualified to perform their dive team responsibilities and / or underwater work. For inspection work a registered Professional Engineer (PE) diver is required to either conduct the inspection or be the dive supervisor in communication with the diver to ensure of technical competence during inspection. For dives that require a recompression chamber, a chamber operator and inside chamber tender shall be specifically identified and neither the operator of the chamber or the inside tender shall be one of the in-water divers or stand-by divers.

In-water diver(s) and stand-by diver(s) shall not have been exposed to hyperbaric conditions within 12 hours of the first and all subsequent planned dives. Each dive team member shall have documented training and experience consistent with the performance requirements of this scope of work. As a minimum, each team member shall have at least 1 year of military and/or commercial experience in the applicable position; divers shall have completed at least four (4) working dives with similar decompression techniques as in the contract, using the particular diving techniques and equipment to be used under the contract. Divers shall demonstrate that at least one (1) of the four (4) qualification dives was performed in the last 6 months prior to the contract award date. As stated in EM 385-1-1, Section 30.A.07.

6. GOVERNMENT PERSONNEL: The Government will provide a Diving Safety Inspector and other personnel as necessary.

7. DIVE TEAM UNIT FURNISHED EQUIPMENT: The Dive Team shall supply the following:

A) SCUBA and / or Standard Surface-Supplied Air with 2-way voice communication, approved air compressors and / or air cylinders shall be used for primary and backup SSA systems, up to two complete sets of minimum 175 ft. umbilicals shall be required to support the diver (s) and one standby diver.

B) USACE required diving equipment as listed in EM 385-1-1, Section 30.E (latest edition). Please note new regulations as stated in Corps of Engineers Safety and Health Manual, EM 385-1-1, edition of November 2014, Section 30.

C) Underwater color video camera with adequate lighting apparatus, a digital and/or DVD format video recorder, and a color monitor suitable for on-site, real-time viewing.

D) Unless waived in advance of the scheduled starting dive date(s) by the Los Angeles/Buffalo Office of Dive Safety, divers working in water less than 46-degree-F for bottom times exceeding 45 minutes of accumulated bottom time or less than 36 degrees-F for bottom times exceeding 20 minutes of accumulated bottom time shall utilize hot water diving systems. Unless waived in advance of the scheduled starting dive date(s) by the Los Angeles/Buffalo District of Dive Safety, variable-volume dry suits with appropriate thermal protection shall be utilized by divers working in waters 60 degrees or less.

NOTE: Hot water suits may be REQUIRED for this mission.

E) An approved and certified dual-lock, multi-place recompression chamber (EM 385-1-1, Section 30.C). This chamber must be certified and all pertinent documentation available on site. In accordance to industry practices the chamber will be pressurized to approximately 120 fsw and be fully ready to accept and treat an injured diver. The chamber shall be located such that the diver can be put to treatment depth within five (5) minutes of surfacing.

NOTE: The designated Chamber operator must possess current certifications and / or training documentation. The operator of the chamber shall not be one of the divers or standby divers.

NOTE: A Recompression Chamber may be REQUIRED for this mission.

F) Common hand and inspection tools including, but not limited to, hammer, measuring tape, underwater steel thickness gauge, and probing rod.

NOTE: Debris and sediment has accumulated on the bottom sill and will need to be removed to allow proper inspection.

H) Toilet facilities in accordance to Section 2.C.06, EM 385-1-1. Appropriate toilet facilities may be required at the floating work site.

I) Lighting in accordance to Section 7, EM 385-1-1.

J) USCG inspected dive vessel of appropriate size to safely accommodate a minimum of a six-person dive team and associated dive and support equipment, dive and government officials (DSI and GCOR), floating work platform of sufficient size to support required tasks.

8. GOVERNMENT FURNISHED EQUIPMENT: None.

9. PRE-DIVE CONFERENCE: A pre-dive safety and work conference will be conducted prior all diving operations and will be conducted just prior to dives in the vicinity of the dive site. All personnel, Government and Contractor, are required to attend.

10. ANTICIPATED DIVING CONDITIONS:

Average Working Depth: < 60-100 feet

Maximum working depth: < 100 feet.

Water Visibility: < 04 feet.

Water temperature: < 60 degrees F

Current velocity: (Impact clearances to be determined) 0 ft/sec

11. EMERGENCY SERVICES FOR ALL LOCATIONS

A. EMERGENCY SERVICES (EMS):

For all emergencies contact The Alamo Dam Control Room at via external telephone 541-374-8338, via internal telephone Extension 2223, or by VHS radio on Channel #14.

B. Air Evacuation:

The Dive Team shall be in constant radio communications with the Control Room Operator via the DSI or government representative. Project radio system is the primary and the Project phone system is the secondary means of activating emergency services in the required Emergency Management Plan submittal.

11. SAFETY CLEARANCES. Safe clearances for the control of hazardous energy will be obtained by an authorized Project employee appointed by the Operations Project Manager and shall be jointly inspected daily by the Project Clearance Holder, the Diving Safety Inspector and the Dive Supervisor prior to commencement of dive operations and the dive safety meeting. Safe clearance procedures shall comply with ER 385-1-31 and the Project’s Safe Clearance Procedures.

12. OPERATIONAL PROCEDURES. See detailed requirements in the main Performance Work Statement.

13. DIVE PLAN PACKAGE. In accordance with EM 385-1-1, 30.A.16, the Dive Team shall submit a dive plan package for complex projects involving more than one work task, location, and/or dive team require each separate facility consisting of

a) Dive Plan;

b) Mission-specific Activity Hazard Analysis;

c) Mission-specific Accident Prevention Plan;

c) Site-Specific Emergency Management Plan;

d) Safe Practice Manual - If not on file with LRB/SPL Office of Dive Safety;

e) Other documentation requested by the LRB/SPL District Office of Dive Safety.

This dive plan package shall be specific to location and operation and shall include the following statements:

1) NOTE: “This Dive/Work plan and related dive and/or other underwater operations shall strictly conform to all regulations set forth in EM 385-1-1, November 2014 Revision. Failure to do so may result in the immediate termination of the dive operation, cancellation of the dive contract and / or other sanctions as deemed appropriate by the USACE Los Angeles/Buffalo District Office of Council.”

2) NOTE: "If for any reason the dive plan is altered in mission, depth, personnel, or equipment, the DDC shall be contacted in order to review and accept the alteration prior to actual operation."

3) NOTE: If a complete dive plan package is not submitted with satisfactory documentation, and in compliance with the aforementioned regulations, at least 72 hours prior to the contract scheduled start date; the Government may terminate the contract for default/cause.

A copy of the dive plan package shall be emailed to Adam.W.Hamm@usace.army.mil at the Buffalo District Office of Dive Safety while the original dive plan package shall be submitted to the on-site Government Diving Safety inspector upon request at the dive site.

PERFORMANCE WORK STATEMENT

ALAMO DAM BULKHEAD GATE INSPECTION SUPPORT

PERFORMANCE WORK STATEMENT

1. General: The Contractor shall perform the following duties:

1.1. Extract the bulkhead gate at the intake structure of Alamo Dam near Wenden, AZ from its storage position.

1.2. Place the bulkhead on an unsubmerged surface, such as a floating plant or boat ramp, and accommodate access for U.S. Army Corps of Engineers (USACE) inspectors to perform a structural inspection of the gate, including both visual and non-destructive testing (NDT) techniques.

1.3. Provide a Remote Operating Vehicle (ROV) inspection of the bulkhead slots and sill at the intake entrance to the tunnel.

1.4. Reinstall the bulkhead gate into its storage position after the inspection is complete.

1.5. As an option to the government, install the bulkhead in its operation position in front of the tunnel to assess the seal condition.

1.6. As an option to the government, if it is determined that repairs are needed to the bulkhead gate, transport the bulkhead to a land facility for storage where repairs can occur.

1.7. As an option to the government, perform a full visual “hands-on” inspection of the submerged bulkhead slots and sill at the intake entrance to the tunnel with divers.

2. Duration and dates:

2.1. The inspection of the bulkhead gate is scheduled for 29-30 November 2016. It is anticipated that the USACE inspectors will require two (2) consecutive days of access to complete their work. Ten hour workdays are to be assumed.

2.2. The Contractor shall coordinate the time of inspection of the bulkhead slots and sill with

Alamo Dam personnel. The inspections may occur simultaneously with the extraction and inspection of the bulkhead gate as long as all safety requirements are met.

3. Detailed Requirements: The Contractor shall complete the following tasks:

3.1. BASE: Bulkhead Gate Extraction: At the time on inspection, the lake surface elevation is expected to be approximately 1095 ft Above Mean Sea Level (AMSL). The bulkhead gate is currently stored in the intake structure hoisting frame which bottom is on top of a concrete deck at elevation 1070 AMSL and the bulkhead is submerged beneath the lake surface with the top of the gate at an elevation +/- 1067.0 ft AMSL. The bulkhead is secured with slings to the hinged gate guide pin support bracket by 1 ¾” diameter 12” long pins to the hoisting frame at elevation 1070 ft AMSL. The Contractor shall extract the bulkhead from its storage position to an unsubmerged location so that USACE inspectors are able to perform visual inspection and NDT on it. The bulkhead gate weighs approximately 23,000 pounds. The Lake Side boat ramp parking lot is the preferred location for this inspection. It should be assumed that the Cholla boat ramp will not be available for use by the Contractor.

3.2. BASE: Bulkhead Gate Inspection Access: The Contractor shall provide access for three

(3) USACE inspectors to perform a full visual “hands-on” inspection of the gate. The gate shall be oriented with its skin plate face down to allow access to the girder flanges initially. Once the USACE inspectors have completed the inspection of the first side of the gate, the Contractor shall flip the gate to its other side, with the skin plate oriented face up for inspection of the skin plate. The contractor shall supply the inspectors with standard 120V electrical power for their inspection equipment for the duration of the inspection. It is anticipated that the inspection will take two (2) consecutive ten hour days. The bulkhead gate measures 15’-8” W x 20’-7” H x 1’-6” D. Access to the entire gate, including the edges is required, as well as a six-foot clear walking surface around the entire perimeter.

3.3. BASE: Bulkhead Slots and Sill ROV Inspection: The Contractor shall perform an initial

Remote Operating Vehicle (ROV) inspection with high-resolution color video/lights, 2D multi-beam, scanning sonar to inspect the submerged bulkhead slots and sill at the intake entrance to the tunnel. There is suspected damage to the right side sill due to a previous dropping of the old bulkhead approximately 60 ft free fall. The old bulkhead that was taken out of service showed damage in the lower right corner. The Contractor shall provide the ROV video, a written inspection report with photos and sketches that includes types, locations, sizes, etc. of all deficiencies. Final, signed reports shall be provided to the Government within thirty (30) calendar days of the completion of the work.

3.4. BASE: Bulkhead Gate Storage: Once the USACE inspectors have completed their work, the Contractor shall replace the bulkhead in its original stored position on the hoisting frame at the intake structure.

3.5. OPTION 1: Bulkhead Gate Test Installation: As an option to the contract, the

Government may decide to have the bulkhead installed in its operation position in front of the intake tunnel at elevation 990 ft. This installation will be for a short time to assess how well it seals the tunnel for a future inspection. Once the situation has been observed by Government inspectors, the Contractor shall then replace the bulkhead in its original stored position on the hoisting frame at the intake structure.

3.6. OPTION 2: Bulkhead Gate Transport: As an option to the contract, the Government may decide to have the bulkhead transported to a land facility if it is determined that repairs to the bulkhead are required. If this option is exercised, the Contractor shall be responsible for transporting the bulkhead to the site instead of replacing it in its stored position on the hoisting frame. The bulkhead gate shall be transported to and stored at the parking lot of the Lake Side boat ramp. The bulkhead shall be supported on timber blocking with its skin plate facing up.

3.7. OPTION 3: Bulkhead Slots and Sill Dive Inspection: If significant damage is found during the ROV inspection, the Government may decide to exercise an option to have the Contractor perform a full visual “hands-on” inspection of the submerged bulkhead slots and sill at the intake entrance to the tunnel. The Contractor shall provide dive video(s), a written inspection report with photos and sketches that includes types, locations, sizes, etc. of all deficiencies. Final, signed reports shall be provided to the

Government within thirty (30) calendar days of the completion of the work.

4. Submittals: The Contractor shall submit a safety plan, pick plan, dive plan (see attached dive SOW/requirements), and inspection plan at least ten (10) days prior to the inspection to be approved by the Government.

5. Government Furnished Materials: Drawings and other information of the project and structures involved are included as an attachment to this PWS.

6. Limiting Criteria: All work must be in accordance with the Corps of Engineers Manual EM

385-1-1, entitled Safety and Health Requirements Manual, dated 2014. Contractor personnel performing the inspections of the bulkhead slots and sill shall be Professional Engineers holding a license in the United States or Puerto Rico.

7. Security: The Contractor shall meet the following requirements:

7.1. All contractor employees, to include subcontractor employees, requiring access to Army installations, facilities, controlled access areas, or require network access, shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. Upon request, the contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer (if a COR is not assigned), within 5 calendar days after completion of training by all employees and subcontractor personnel. AT Level I awareness training is available at the following website: http://jko.jten.mil/courses/atl1/launch.html; or it can be provided by the RA ATO in presentation form which will be documented via memorandum.

7.2. All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation/facility access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any installation or facility change, the Government may require changes in contractor security matters or processes.

7.3. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05 / AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative, as NCIC and TSDB are available), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

7.4. All contract employees, including subcontractor employees who are not in possession of the appropriate security clearance or access privileges, will be escorted in areas where they may be exposed to classified and/or sensitive materials and/or sensitive or restricted areas.

7.5. The Contractor must pre-screen Candidates using the E-verify Program

(http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the COR no later than 3 business days after the initial contract award.

8. Points of Contact:

8.1. USACE Dive POC (for all dive related issues):

52.222-42 STATEMENT OF EQUIVALENT RATES FOR FEDERAL HIRES (MAY 2014)

In compliance with the Service Contract Labor Standards statute and the regulations of the Secretary of Labor (29 CFR part 4), this clause identifies the classes of service employees expected to be employed under the contract and states the wages and fringe benefits payable to each if they were employed by the contracting agency subject to the provisions of 5 U.S.C. 5341 or 5332.
