QUALITY ASSURANCE SURVEILLANCE PLAN
1. INTRODUCTION
1.1 This Quality Assurance Surveillance Plan (QASP) has been developed and designed for the Prime Power COTR duties in Afghanistan. This surveillance plan is site-specific and is designed to provide monitoring, inspecting and reporting of all contractor Operations and Maintenance (O&M) aspects of power generation systems.
1.2 A combination of methods for inspection will be used. They are:
a. Random Sampling b.100% Inspection
c. Scheduled Observations
d. Unscheduled Observations
1.3 The objective of this Surveillance Plan is to evaluate how the O&M is being performed by the contractor.
1.4 This QASP is based on the premise the government desires to maintain a quality standard in operating, maintaining, and repairing the power plant and that a specific contract to provide the service is the best means of achieving that objective.
1.5 The contractor, and not the government, is responsible for management and quality control actions to meet the terms of the contract. The role of the government in quality assurance to ensure contract requirements and standards are achieved.
1.6 The quality control program is the driver for product quality. The contractor is responsible for developing a comprehensive program of inspections and monitoring action. The contractor’s quality control program must be approved by the government at the beginning of the contract, and provides the measures to lead the contractor to success.
1.7 Once the quality control program is approved, careful application of the process requirements and standards presented in the remainder of this document will ensure a robust assurance program.
2. SAMPLING PROCEDURES
2.1 The scope of service is to provide reliable electrical service and sustain an effective Operations and Maintenance (O&M) Plan. The primary purpose of the contractor is to coordinate with the COTR to develop its O&M organization into a fully functional and effective mainstay in its daily operations while simultaneously performing effective O&M services at the power plant. This Surveillance Plan uses Random Sampling, 100% Inspection, Scheduled Observations and Unscheduled Observations as methods of inspection.
2.2 The preferred method of surveillance is random sampling of the required services each month. This plan contains the necessary forms and computation tables to monitor contract performance. The way the Random Sampling will be conducted is defined for each contract data requirement. Random Sampling will be used when some part, but not all of the contractor’s performance is to be evaluated.
2.3 One hundred percent inspection is the method used to evaluate the contractor when required by the contract Performance Requirements Summary (PRS). Using this method of inspection, all the parts of the service or requirement are monitored for evaluation. Such performance requirements cover equipment and practices deemed critical. Failure to properly O&M these contract data requirements would certainly have an adverse effect on the power generation and distribution systems.
2.4 Scheduled Observations are inspections previously scheduled and coordinated with the contractor’s site supervisor. These Scheduled Observations will include but are not limited to, the month’s trouble tickets, work orders, and corrective and preventative maintenance forms.
2.5 Unscheduled Observations will be conducted by the COTR at random. These Unscheduled Observations will include but not be limited to, unannounced visits to the Power Plant manager, customer premises and outside plant (OSP) work areas.
