1.
SF 1449
There is a reference that we should complete and submit an SF 1449, however there is not one within the documents. Will the government be providing this to us?
One has been attached. It can also be found by utilizing a search engine.

Past Performance
When submitting our past performance, we assuming that 3 past performances are required, and that they can be from both the prime and/or our team members. Please indicate whether our assumption is correct.
Yes
3.
Past Performance
There is no indication as to how far back our past performance should go, can we assume that it can go back 5 years?
Yes

Are there incumbent personnel at all sites?
There were when contract expired
5.
PWS
If there incumbents, how many and what types of positions are normally at each location?
2 nurses and 1 admin assistant

How many incumbents were historically on each of the two of the previously separate contracts?
13
7.
PWS
We see that the incumbent contracts ended in March 2018, was there another contractor or other arrangements made to continue services?
No
8.
PWS
Are you looking for specific “behavioral health” qualifications for the RN Case Manager?
No
9.
PWS

Are you looking for specific levels of education, qualifications, and years of experience for the Care Coordinators?
Yes
11.
PWS
Within the PWS there are “duties” for the Care Coordinator but what are the qualifications and the educational requirements for this labor category?
Preferably College degree, 3 yrs experience or work history that is medical and administrative.
12.
PWS
Is the Lead position a separate case manager and will there be a need for a Lead at each location, in addition to the Nurse Case Manager?
Just one lead. In the past it has been an additional duty. Only one for the state is required.
13.
PWS
We understand that the Government requires resumes and letters of commitment from key personnel. We regard the vendor program manager, vendor contract manager and the vendor alternate program manager as key. Please identify if the Government considers any other positions as key personnel for this program?
No
14.
Price
The documents provided for this opportunity do not include a pricing document. Will there be separate price document that will be provided to us with the CLINs defined?
SEE BELOW
*Also, see CLIN structure in the synopsis/solicitation.
15.
Price
We request that the government provide us with the number of annual hours that should be used as a basis for price determination; are the hours 2080,1920, or 1880?

16.
PWS 1.3.1.1
In the PWS there is indication of a QCP being required with the proposal submission, however it is not a part of the proposal instructions or evaluation criteria.
We assuming that this is a completely separate document, not a part of the technical 50 page limit. Please confirm.
Correct
17.
Submission
Instructions for submission of the hard copies are unclear. Please confirm our assumption that when we ship the (3) hard copies of the proposal, that all factors (Price, Technical, Past Performance and Administrative/Contract data can be combined into a single binder for each copy with tab separators/dividers.

All
19.
Price (Travel)
For the Travel CLIN we are considering that it is a cost reimbursable element, please confirm. Please also confirm that we should leave this CLIN blank for our proposal submission.
PWS 1.10
Travel is cost reimbursable. This CLIN should not be priced on proposals. Please refer to PWS.
20.
Factor A, #9
In subfactor 1, #9 it reads: “(9) An adequate approach to reducing staff when a reduction in force is necessary”
Please confirm that any “reduction in force” (RIF) would be the result of the Government taking action via a contract modification to change the scope of work. It is presumed that any RIF would only be the result of a Government initiated action.
If the above statement is not correct, please elaborate on the Government’s intention when asking for this information.
Yes, this would be a result of the Government taking action via a contract modification.
This would not be a change in the scope of the work, but it would result in a Bilateral modification.
