PERFORMANCE STANDARDS

PWS

Performance Work Statement (PWS)

Medical/Dental Case Management Support

Mississippi Army National Guard

1. PURPOSE. This is to provide Medical and Dental Non-clinical Case Management staffing in support of Individual Medical Readiness (IMR) to the Mississippi Army National Guard (MSARNG). This contract is designed to be flexible, responsive, and meets the ever-evolving needs of the MSARNG and the Office of the State Surgeon. This is a firm fixed priced contract.

2. SCOPE

a. The MSARNG Surgeon’s Office requires a highly responsive process whereby the MSARNG can quickly obtain needed support and services. This PWS reflects existing National Guard Bureau policies and regulations.

b. The Contractor shall furnish all labor and services required to provide a wide range of Medical and

Dental Non-clinical Case Management support services, including Periodic Health Assessment (PHA) support, Soldier Readiness Processing (SRP) support, Soldier Readiness Certification (SRC) support, Nonclinical

Case Management of Soldier’s found to be in need of advanced medical screening, Line of Duty

(LOD) non-clinical case management and/or referral to a MOS/Medical retention Board (MMRB), Medical Evaluation Board (MEB) or Physical Evaluation Board (PEB) .

3. PLACE of PERFORMANCE. The Contractor shall provide these services in multiple locations throughout the State of Mississippi as determined by the MSARNG Office of the State Surgeon, and specifically the Deputy State Surgeon. The Contractor may be required to deploy and execute any missions to support and augment the MSARNG consistent with all services described within the PWS as shown within. In the event of a mobilization or demobilization the Contractor shall ensure that adequate nonclinical case management support is deployed to locations to best support these activities (i.e. work at armories.) Additionally, the staffing levels and locations may change to remain responsive to the

MSARNG’s evolving case-management requirements.

4. BACKGROUND. The MSARNG Surgeon’s Office requires Registered Nurse (RN) Non-clinical Case

Managers. The MSARNG has a critical requirement for skilled medical personnel with expertise in the following areas: non-clinical case management, case management planning, analysis and review, patient risk analysis and service support. However, due to the combined significant need for medical professionals on the state level, an immediate and continuing need exists for contractor support through a contract mechanism. The most effective method addressing this requirement is to secure the services of a support contractor with functional expertise and available experienced registered nurses in order to provide program continuity and staff coordination to address requirements. For example, the MSARNG may have a need for a Nurse Practitioner to review the Registered Nurses overall Case Management but the need is only for part-time (less than 40 hours) per week. This contracting vehicle will allow for part-time and fulltime contract RN and NP Case Managers depending on the workload that may shift from month to month because of the needs of the MSARNG Surgeon’s Office.

The capability of a local contractor team to provide Registered Nurses or Nurse Practitioners for locations throughout the state would allow the MSARNG Surgeon’s Office to meet and support the medical readiness goals and the mission of the MSARNG.

5. PROGRAM OBJECTIVES

a. Non-clinical Case Management Support - Contractor shall provide Registered Nurses (RN) who have graduated from an accredited nursing school as Non-clinical Case Managers to provide pre-mobilization

IMR support to include identification and tracking of Soldiers who are not Fully Medically Ready (FMR) either for dental or medical concerns. Cases will be obtained from home station (HS) or mobilization station (MOBSTA) SRP events or those Soldiers determined non-medically or non-dentally ready through other means (e.g. during their annual PHA.) Case Managers shall assist Soldiers to become FMR by assisting with and tracking medical and dental appointments and maintaining communication as required with the Soldiers they are managing. All cases managed, regardless of type (medical or dental) or duration, must be documented in the Medical Non-Deployable (MND) Module of the Medical Operational Data

Appendix B

Evaluation Criteria

1.0 General. The Government will evaluate each Offeror’s proposal that is received by the established closing date and time. The Government will use the results of the source selection process to make an award. The source selection process will determine the overall merits of each proposal in terms of its potential to best satisfy the needs of the Government. Award will be made to the Offeror whose proposal conforms to the requirements of the solicitation and is judged to represent the best value to the Government. The best value represents an integrated assessment of the price and non-price factors.

2.0 Evaluation Factors

2.1. The evaluation factors and subfactors are as follows:

Factor 1 Technical Approach o Subfactor 1 – Understanding of the Requirement o Subfactor 2 –Process and Approaches to Meet Requirements

Factor 2 Management Approach o Subfactor 1 – Staffing/Management Plan o Subfactor 2 – Key Personnel o Subfactor 3 – Quality Assurance Plan

Factor 3 Past Performance o Subfactor 1 – Corporate Capabilities o Subfactor 2 – Past Performance Questionnaires

Factor 4 Price

2.2. When evaluating offers, the Government considers Technical Approach to be more important than Management Approach. Management Approach is considered more important than Past

Performance. When combined, all non-price factors shall be considered significantly more important than Price.

2.3. Under the Technical Approach Factor, the Understanding of the Requirement Subfactor is considered to be more important than the Process and Approaches to Meet Requirements

Subfactor.

2.4. Under the Management Approach Factor, the Staffing/Management Plan Subfactor is considered to be more important than the Key Personnel Subfactor. The Key Personnel Subfactor is considered more important than the Quality Assurance Plan Subfactor.

2.5. Under Past Performance, both subfactors are considered to be equal in weight.

3.0. Rating Scales

The Government will evaluate each proposal to identify strengths, weaknesses, significant weaknesses, risks, potential clarifications, and deficiencies. The consensus evaluations will be supported by written narratives justifying the ratings.

3.1. All proposals will be evaluated in accordance with the Factors and Subfactors set out in this solicitation. All proposals will receive adjectival ratings at the Factor and Subfactor level.

3.2 Definitions for Adjectival ratings used in evaluating Factors 1 and 2 and their Subfactors:

Adjectival Rating Definition

Excellent

Offeror’s proposal demonstrates an excellent understanding of the requirements and the approach is of superior quality. There are major strengths. There are no deficiencies. Exceeds many of the Government’s expectations.

Very good

Offeror’s proposal demonstrates a very good understanding of the requirements and the approach is of good quality. Strengths clearly outbalance any weaknesses that exist. There are no deficiencies. Exceeds some of the Government’s expectations.

Good

Offeror’s proposal demonstrates a good understanding of the requirements and the approach is of satisfactory quality. There may be strengths or weaknesses and the strengths balance the weaknesses. Meets the government’s expectations.

Unsatisfactory

Offeror’s proposal does not demonstrate an understanding of the requirements and the approach is of unacceptable quality. Significant weaknesses or deficiencies exist. Does not meet the Government’s expectations.

4.0 Technical Proposal Contents (Volume 1). The Government will evaluate the offeror’s proposed technical and management approaches for delivering the requirements expressed in the performance work statement (PWS) and appendices, the requirements within the applicable regulations referenced in the PWS, and any specific examples or sources provided by the offeror which support the proposed technical solution as being the most relevant, low risk, and best value approach. All sections of the Technical Proposal must reflect technical competence, and must demonstrate that the requirements of the PWS can be accomplished. The Government will evaluate the following Factors and Subfactors to rate the Technical Proposal:

4.1. Factor 1 - Technical Approach - The government will evaluate the following sub-factors, to evaluate the technical approach:

4.1.1. Factor 1, Subfactor 1 – Understanding of the Requirement. The offeror’s understanding of the requirement. Technical solutions that demonstrate a clear understanding of the requirements within the PWS and relevant regulations shall be rated more favorably than offers that merely restate the requirements and quote the regulations. Specific attention will be given to the details regarding the complete understanding of each contract location requirements located in the appendices.

4.1.2. Factor 1, Subfactor 2 – Methods/Processes to Meet Requirements. The processes or technology within the offeror’s proposal. Proposals which convincingly explain how their unique processes or technology will overcome performance risks will be rated more favorably than those which merely list processes or technology.

4.2. Factor 2 - Management Approach - The government will use the following sub-factors, listed in descending order of importance, to evaluate the management approach:

4.2.1. Factor 2, Subfactor 1 – Management/Staffing Plan - This sub-factor will be evaluated based upon the degree that the proposal addresses the need and the proposed technical approach.

Offers with more efficient organizations, which offer higher minimum levels of experience and criteria, will be rated more favorably than those which merely meet the minimum requirements.

4.2.2. Factor 2, Subfactor 2 – Key Personnel - The Government will evaluate the following resumes considered “Key Personnel,” (1) the resume of the Offeror’s responsible agent for the contract within the company, (2) the Program Manager’s resume, and (3) the Assistant Program

Manager’s resume. The resumes will be evaluated to determine the adequacy and desirability of the individuals assigned to these positions to perform the work proposed to accomplish the PWS.

Resumes proposing more relevant experience and higher education levels will be rated more favorably than those which merely meet the minimum requirements.

4.2.3. Factor 2, Subfactor 3 - The Quality Assurance Plan (QA Plan) – The Government shall evaluate more favorably plans that offer more effective, appropriate and advantageous mechanisms for addressing the government metrics of success; plans that identify additional quality metrics, offer plausible auditing procedures and providing enforcement mechanisms

(incentives and decrements). Plans with vague assurances or which merely promise compliance with the PWS requirements will be considered as unsatisfactory and evaluated as higher risk.

4.3. Factor 3 - Past Performance - The Government will conduct a performance risk assessment based upon the Past Performance of the offeror as it relates to the probability of successful accomplishment of the work required by the solicitation. The evaluation will consider the offeror's Past Performance considering currency, relevancy, sources, context, and trends.

Specific details of similar analytical, technical, instructor, training, flight and/ or other requirement details in the PWS and appendices will be analyzed. Past Performance includes current on-going performance. Past performance assessed with less than satisfactory sub-factor relevance shall not result in a total Past Performance factor rating of higher than satisfactory.

4.3.1 PAST PERFORMANCE RATINGS ARE:

Excellent

Performance met all contract requirements and exceeded many to the Government's benefit. Problems, if any, were negligible and were resolved in a timely and highly effective manner. Excellent probability of success with overall low degree of risk in meeting Government's requirements.

Very Good

Performance met all contract requirements and exceeded some to the Government's benefit. Problems, if any, were negligible and were resolved in a timely and highly effective manner. Very good probability of success with overall low to moderate degree of risk in meeting Government's requirements.

Satisfactory

Performance met contract requirements. Adequate quality. Minor problems were identified however; contractor took satisfactory corrective action to resolve where appropriate. Good probability of success with overall moderate to degree of risk in meeting the government's requirements.

Neutral No relevant performance record is identifiable upon which to base a meaningful performance risk prediction. A search was unable to identify any relevant Past performance information for the offeror or key team members/subcontractors or their key personnel. This is neither a negative or positive assessment.

Less than

Satisfactory

Performance did not meet some contractual requirements. There were problems, some of a somewhat serious nature. Marginally effective corrective action was sometimes noted. Fair probability of success with a moderately high, to a high degree of risk in meeting the government's requirements.

Unsatisfactory Performance did not meet some contractual requirements. There were problems, some of a serious nature. Contractor's corrective action was sometimes marginally effective to ineffective. Probability of success is questionable with an unacceptably high degree of risk in meeting the government’s requirements.

4.3.2. Currency, Relevancy, Trends: The Government will consider the currency, relevancy and trends of the performance information while conducting its Performance evaluation. For the purpose of this solicitation, currency is performance occurring within the last five (5) years.

Within this period, performance occurring later in the period may have greater significance than work occurring earlier in the period. For example, performance information for work occurring during January 2007 may have greater importance than performance information for work occurring during November 2004. In assessing relevancy, the Government may evaluate an offeror's references for similarity of the contract scope to their previous performance on other contracts.

4.3.3. In assessing performance confidence, the Government may: (1) evaluate present and past performance information through the use of questionnaires completed by the offeror's references;

(2) use data independently obtained from other sources; (3) rely upon personal business experience with the offeror; and (4) consider any information the offeror submits which explains any problems encountered on the identified contractor and the offeror's corrective actions. The

Government may use data provided by the offeror, and data obtained from other sources. The evaluation may take into account Past Performance information regarding predecessor companies, key personnel who have relevant experience or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to this acquisition.

Past Performance information on contracts not listed by the offeror, or that of planned subcontractors, if any, may also be evaluated. The Government may contact references, use data bases containing performance information, and contact parties other than those identified by the offeror and information received may be used in the evaluation of the offeror’s Past Performance.

While the Government may elect to consider data obtained from other sources, the burden of providing current accurate and complete Past Performance information rests with the offeror.

4.3.4. Offerors lacking Past Performance will receive a neutral rating.

4.3.5. Offerors may be given the opportunity to clarify certain aspects of their proposals (e.g., the relevance of an offeror’s Past Performance information and adverse Past Performance information to which the offeror has not previously had an opportunity to respond or to resolve minor clerical errors.)

4.3.6. Offerors are cautioned that all information provided, regardless of the format shall be considered as proposal information, unless specifically accepted, and used in the evaluation process.

4.3.7. The following sub-factors shall be evaluated to rate the offeror’s Past Performance:

4.3.7.1. Factor 3, Subfactor 1 - Corporate Capabilities. The Government will evaluate the entire technical proposal to ascertain the currency, relevancy, reliability of the source, context of the data, and general trends within the corporate past performance in relationship to the efforts required under the PWS. Additionally, proposals that demonstrate the capability to manage performance risks and deliver services to a nationwide scope shall be rated more favorably than those without evidence of these capabilities within their proposals. Offeror which provide information on problems encountered on the identified contracts and the corrective action used to improve the problems and manage risk will be rated more favorably and be assessed with lower risk than those without this information.

4.3.7.2. Factor 3, Subfactor 2 - Past Performance Questionnaire – Each element within the questionnaire shall receive equal weighting for this sub-factor. An offeror shall receive a

“neutral” rating if it claims no past performance or if no past performance questionnaires are received in response to this solicitation. A neutral rating in past performance shall not be considered positive or negative. The offeror is encouraged to include any relevant past performance it may have as another entity, of its corporate staff, the Project Manager, etc.

5.0. Factor 4 - Price (Volume 2) - The offeror’s cost/price proposal will be evaluated by the

Contracting Officer for reasonableness and completeness to determine if the offeror's proposed prices are compatible with the requirements and the proposed technical solutions. The Price

Panel (NGB-AQ) will evaluate the labor rates and the pricing for the sample task order using the following factors, which have equal importance:

5.1. Reasonableness. The offeror's fixed price is fair and reasonable to both the offeror and the

Government for the level of effort proposed. Compensation that is unrealistically low or not consistent with the GSE levels listed above may impair the Contractor’s ability to attract and retain competent service employees; and may be viewed as evidence of failure to comprehend the complexity of the contract requirements. It is solely the offeror’s responsibility to convince the

Government that any compensation levels that are lower than the GSE levels listed will not impair the contractor’s ability to attract and retain competent employees for this contract.

5.2. Completeness. The offeror's price proposal responds to all solicitation requirements and the price estimates are traceable to those requirements.

5.3. Realism. The offers submitted must have labor mixes and levels of effort (number of hours) that are considered appropriate and realistic.

5.4. Balance. Unbalanced pricing occurs when, despite an acceptable total evaluated price, the price of one or more periods of performance is significantly different (much greater or much less). Unbalanced pricing may increase performance risk and may result in the payment of unreasonably high prices.

6.0. Evaluation Process.

6.1 Individual Evaluation: The purpose of the individual stage of the evaluation is for each evaluator to rate each proposal with a single rating for each factor and sub factor and according to the evaluation criteria listed in the solicitation and the guidance within the source selection plan. Each evaluation shall be made of each proposal independently and shall not be influenced by other members of the SSEB.

6.2. Consensus Evaluation: The purpose of the consensus stage of the evaluation is to determine a single rating for each factor and subfactor that will be used in the best value comparison stage in the evaluation. The SSEB shall compare the ratings from the evaluations prepared by each member and then reach a consensus concerning the SSEB rating for each offeror’s factors and subfactors , without comparing one proposal to another.

6.3. Best Value Comparison Evaluation: (Technical solution) While cost/price is not a significant factor in the evaluation, the importance of cost/price as an evaluation factor will increase with the degree of non-cost factor equality of competing proposals. When the proposal with the lowest evaluated cost/price is other than the proposal(s) with the highest non-price factor rating, a cost/technical tradeoff analysis is necessary to decide whether the technical superiority of the other proposal(s) warrants payment of the additional price or cost. The source selection authority may select the best value that may not be the lowest price or the highest technically ranked offeror. This decision will involve a comparison of the combination of non-cost strengths, weaknesses, and risks and cost/price offered in each proposal and judgment as to which provides the best value. The government may be willing to pay a higher price for better capability, skills, reduced risk, or other non-price factors, if the added benefits are worth the premium.

7.0. Award without Discussions. It is the government's intention to award a single contract without discussions; however, the government reserves the right to conduct discussions if it is determined to be in the best interests of the government to do so. Therefore, the offeror's initial proposal should contain the offeror's best terms from a cost/price and technical standpoint. If it becomes apparent that discussions are necessary, the government may hold communications with some offerors to determine whether or not to include a proposal in the competitive range.

Communications will be held with any offeror who will be excluded from the competitive range because of adverse past performance information to which the offeror has not had a prior opportunity to respond.

8.0. Notification of Unsuccessful Offerors. Upon written request, unsuccessful offerors will be notified in compliance with FAR 8.405-2(d).

9.0. Release of Information. After receipt, all proposals shall be treated as source selection information will not be available to other firms or released under the Freedom of Information Act unless directly incorporated into the terms and conditions of the final contract.
