It is included in the solicitation (see Past Performance Questionnaire Form) for the Offeror or its team members to submit to the client for each project the Offeror includes in its proposal for Factor 2, Past Performance. (Ensure correct phone numbers and email addresses are provided for the client point of contact.)
1. Completed PPQs should be submitted with your proposal. If the Offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the Offeror should complete and submit with the proposal the first page of the PPQ which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires.
2. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other solicitations. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. Also include performance recognition documents received within the last five years such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition.
3. The Government may call references to verify Past Performance information. The Government also reserves the right to use other information, e.g. personal knowledge, preaward survey, past performance information systems, etc. during the evaluation. While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.
Evaluation criteria:
An Offeror's proposal will be rated as "ACCEPTABLE" for this factor if, based on the Offeror's performance record, the Government has a reasonable expectation that the Offeror will successfully perform the required effort, or the Offeror's performance record is unknown.
(*See note below.)
An Offeror's proposal will be rated "UNACCEPTABLE" for this factor if, based on the Offeror's performance record, the Government has no reasonable expectation that the Offeror will be able to successfully perform the required effort.
: In the case of an Offeror without a record of relevant past performance or for whom * Note information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Offeror may not be evaluated favorably or unfavorably on past performance (FAR 15.305(a)(2)(iv)). Therefore, the Offeror shall be determined to have unknown past performance. In the context of acceptability /unacceptability, "unknown" shall be considered "ACCEPTABLE."
Factor 3 - Price Proposal
It is the Government's intention to award to the offeror whose quote receives "ACCEPTABLE" ratings for Factor 1 - Recent Relevant Experience and Factor 2 - Past Performance and whose quote has the lowest total evaluated price.
Offerors shall provide a complete price proposal in Attachment 4 that includes all information requested below. To be complete, the Offeror must provide all data that is requested and necessary to evaluate prices. Pricing submitted by the offeror shall become binding upon award of the contract.
The Government may determine that a proposal is unacceptable if the prices proposed appear to be materially unbalanced. Unbalanced pricing exists when, despite an acceptable total evaluated price, the price of one or more priced items is significantly overstated or understated as indicated by the application of cost or price analysis techniques. A proposal may be rejected if the Contracting Officer determines that the lack of balance poses an unacceptable risk to the Government.
The SF1449 shall be filled out completely by the Offeror and signed by an official that is authorized to bind the company. The Offeror shall also provide and acknowledge all amendments in accordance with the instructions on the SF1449. The Offeror shall complete the required Annual Representations and Certifications through the System for Award Management (SAM) at http://www.sam.gov/. This tab will not be rated, but will be evaluated for responsiveness and completeness. A rating of "UNACCEPTABLE" will be given to those proposals with incomplete data in this section.
Binding Price Schedule

The Government will evaluate price based on the total of all line items included in the Binding Price Schedule, Binding Price Schedule: Found in Attachment 4, the offeror shall complete the Binding Price Schedule with five-year pricing for each of the items on the schedule. The quantities on this sheet are estimates based on past history and anticipated usage. The rates included in the
